~ History of Willow Creek Childcare ~

Willow Creek Childcare started as a small family daycare opened by Jennifer

Maillet in 2005. In 2012 the daycare grew to a group childcare center. By May 2014

Willow Creek Childcare relocated to a small church building in Terrace. June 2014

Willow Creek Childcare became incorporated. Scott Mitchell who is Jennifer’s father

became a business partner and a key staff member.

February 2015, Willow Creek Childcare relocated to a closed school, with the

support of fifteen staff member’s,Jennifer , Andy, and Scott are happy to provide

childcare services to Terrace and surround areas.

~ Our Philosophy and Goals ~

Children who attend Willow Creek Childcare Center:

• Are valued and respected as the unique individual that they are.

• Have the freedom to explore their environment and learn through challenging play

experiences.

• Have access to a safe environment that nurtures creativity, curiosity and free

expression.

• Feel secure within a flexible but familiar routine with realistic limits.

• Have the opportunity to involve their family and share experiences.

• Will be recognized as an individual within a family unit and to have that family unit

acknowledged as the primary influence on their development.

• Will explore the community through excursions and inviting community participation

at Willow Creek Childcare program.

• Can share their happy, sad and proud experiences with people who will recognize and

respond to my feelings.

~ Our Programs ~

Willow Creek Childcare has two childcare center locations to meet the needs of

children aged 0 to 12. Both locations are wheel chair accessible.

At 3302 Sparks Street, current Programs include: Group Childcare Ages 30 month to

School Age. Preschool, Pre K, Group Childcare for School aged children and Multi-age

childcare.

At 4715 Merkley Rd, current Programs include: Group Childcare for School Aged

Children.

~ Our Staff ~

The quality of our programs is a direct reflection of our staff. Andy and Jennifer

Maillet and Scott Mitchell (Jennifer’s father) are the owner’s of Willow Creek Childcare

Inc. Jennifer has a background in nursing. Other staff include, ECE certified, ECE

assistants, certified school teachersand janitorial. Jennifer’s Dad, Scott Mitchell who the

children refer to as Papa also works full time at the childcare centre.

~ Days & Hours of Operation ~

Daycare Program

Willow Creek Childcare operates Monday to Friday from 7am to 530pm.

Different programs within the centre have different schedules and operating times.

Willow Creek Childcare will be closed for the following holidays:

New Year’s Day BC Family Day Good Friday

Easter Monday Victoria Day Canada Day

BC Labour Day Thanksgiving Remembrance Day

Christmas Day Boxing Day

Enrolment Policy

Children of employees have first priority at each of our centres. After that,

returning children are the next priority and then siblings of former children. After that,

community children may enrol on a first come first served basis.

Waitlist Policy

If either centre is full, you may put your name on a waitlist and as spots become

available, priority will be given in the same order as in our Enrolment Policy.

~ Fees ~

Infant/ Toddler(Under 36 Months) 3yrs-12yrs

-$ 45.00/day - $40.00 /day

-$28.50/ half day (4 hours or less) - $25.00 /half day (4 hours or less)

- $20.00 /before & after school

Fees Policy

Fees are due on the 1st of every month. Families receiving BC’s Childcare

Subsidy must have the subsidy authorization in place or the first months fees paid before

a child may start. If fees are paid by the parent and subsidy then covers the fees, Willow

Creek Childcare will happily reimburse your payment.

Registration Fee Policy

Families interested in registering their child will be required to pay a $25

registration fee. If the child attends, your registration fee will be applied to the second

month’s bill.

Late Payment Policy

If prior written arrangements are not made with the owner, fees will be considered

overdo if not paid in full by the 1st day of the month care will be provided for. For each

day each month bill is due a $5.00 day late fee will be applied. For example: If fees are

due January 1st, and a parent pays them on January 6

th. Over due charge of $5x5 days will

be added to the bill. Childcare fee’s over due more than one month will result in child not

being allowed to attend until outstanding fees are paid in full.

Child Care Subsidy Policy

Some families are eligible for childcare subsidy, (based on income and family

size) from Ministry of Child & Family Development. Parents are responsible for the

entire cost of care even if applying for subsidy. Staff will remind parents when they need

to renew their subsidy; however it is the responsibility of parents to keep their subsidy up

to date.

Withdrawal Policy

1 month’s written notice is required prior to withdrawing a child from the

centre, or reducing hours/days of care.

If no notice is given, parents will be billed for an additional month of care.

Parents have the option of paying the monthly fee in lieu of notice.

Changes in Prearranged Schedules

A child’s space is only guaranteed for hours and days prearranged (as per the

registration form) with the childcare owner. For children enrolled less than full time,

alternative childcare arrangements must be made for any additional hours required.

Requests for a change in the hours of care must be submitted in writing to Willow

Creek Childcare owner for consideration. There is no guarantee that the centre will be

able to accommodate such requests due to staff/child ratios.

Termination of Service

Services will be terminated when:

Fees for service are not paid by month’s end. In extenuating circumstances,

alternative arrangements for payment may be agreed upon.

The centre is unable to satisfactorily resolve an issue with an enrolling

parent/guardian.

Returned Cheque Policy

A $45 administration fee will be applied to all cheques that are returned as non-

sufficient funds.

~ Arrival & Departures of Children ~

Arrival

When you arrive, it is important to take the opportunity to speak with staff about

your child’s needs for the day, sharing information about major changes at home. This is

necessary for staff to respond appropriately to children’s emotional wellbeing. Take the

time to settle your child in for the day and say goodbye.

If your child will be absent, please call the centre and let staff know.

Transportation Policy

Parents are responsible for transporting their children to and from Willow Creek

Childcare. Transportation is offered to and from Uplands Elementary and Mountain

Ecole via Willow Creek Childcare Bus or alternative approved business use vehicles.

Releasing a Child

Child Care Regulations require that staff can only release a child to the parent(s)

and others whom you have listed underthe registration form. Any other person must have

written or verbal permission from the parent and picture identification.

You must inform staff of any changes to information on your child’s registration

form (eg. Custody agreements, orders, addresses, phone numbers, place of

employment/school).

If the person picking up your child appears to be impaired, or otherwise incapable

of safely caring for the child, staff will encourage the individual to make alternate

arrangements for the child. Staff will attempt to contact the parent/guardian. Staff will not

attempt to stop the removal of the child but may be legally obligated to notify the

appropriate authorities.

Pickup

At pickup time, please speak directly to staff before leaving with your child. This

is also a good time to find out about his/her day.

Late Pickup Policy

If children are picked up late, parents will be charged a fee of $15 for

every 15 minutes or part of 15 minutes past closing time. This will be added

to your bill.

If No One Comes For the Child

Staff will make every effort to find the late parent by calling the parent’s school,

place of work, home and other places the parent might be.

When the parent is late and not known to be coming, the alternate listing will be

called.

If no alternate can be contacted within ½ hour, or is unable to pick up the child,

the staff will call Ministry of Child & Family Development at 250-638-2311

(after hours 250-310-1234). The child will be released to the Ministry’s social

worker after seeing identification and recording his/her name and the time the

child was released.

~ Program ~

Lunch/Snacks

Willow Creek Childcare parents are asked to provide nutritious lunches for your

child(ren). Please ensure your children have adequate food for their day. We

have a microwave available to warm food however must be in a microwavable

container. Ask staff for suggestions or materials on nutritional ideas.

Water is provided all day for the children.

Rest Time

Rest time is an important break for all children to rest their bodies at some point

in the day. Children are not required to sleep, only to give their bodies an

opportunity to relax and rejuvenate.Rest time is a soothing time: a time for staff

and children to connect and spend some quiet time together.Those children who

require a nap and do fall asleep are left comfortably to rest while others find

quiet activities.

Toys from Home

Staff prefers that your child leave toys at home to ensure toys do not get lost or

broken.

We do, however, understand the transition from home to daycare can be

difficult and your child may need to bring something special from home to help

ease the transition. Although we do have a rule that if the child wants to play

with the toy in the daycare they have to be willing to share or else it stays in

their backpack.

Open Door Policy

Willow Creek Childcare has an open door policy. This means that parents are

always welcome.

We encourage parents to visit anytime, share lunch time with us, share your

talents with all the children or just come and see what the children do all day.

Please be aware that the child care staff may not have an opportunity to chat

with you as they are busy with the children.

If you need to speak with the staff please set up a meeting time that is mutually

convenient.

Clothing

A spare set of clothing is necessary and to be left at the centre.

Clothing must be labelled. If unlabelled, staff will label.

We encourage children to come in play clothes as we engage in many messy

activities throughout the day.

Please provide indoor shoes for your child.

Outdoor

Licensing requires that all licensed childcare facilities take children outside

everyday. Children are required to have all necessary clothing, including mitts, hats,

coats and boots. If children do not have the appropriate outerwear, staff will share

clothing from our spare clothing box.

Celebrations

We honour all cultures and religions. Please share with us how your family

celebrates holidays and special events. We try to keep most celebrations low key to

respect the various religious practices. Children’s birthdays are celebrated with a song.

Parents are more than welcome to bring a treat on their child’s birthday to share with

their friends at snack time.

~ Parental Consent ~

Photo Taking

With your permission, staff will take pictures of your child to be used at the

Centre for the enjoyment of children, families and staff. Please ask for a copy of your

favourite ones. We will request written permission if we want to use the photos of your

child for other purposes.

Field Trips

An integral part of our program is planning for experiences and activities outside

the Centre and in the community. Families are required to complete the “Field Trip

Consent Form” before their child participates. If possible, parents/guardians will be

informed in advance and a written permission slip will be provided, outlining our plans.

Transportation

Transportation Form must be signed before a child is transported.

At all times possible we will utilize our 18 passenger school bus with Class 4

drivers licence driver, and clear abstract.

We also own 2 other business vehicles (van and truck) that may be used when

necessary. All children not yet 9 years old will be restrained in a car seat/booster seat

dependent on the Motor Vehicle Act and ICBC recommendations. Care provider will

have a valid driver’s License. Cell phone will not be used while operating the vehicle.

~ Guidance ~

Our goal is to provide an environment in which each child feels safe and secure,

develops friendships and social skills. Each child is encouraged and supported to develop

positive adult and peer relationships.

Families can expect staff to:

Model appropriate, respectful behaviour at all times.

Promote social skills development including self-esteem and self-discipline,

self-help skills, shared responsibility and decision-making.

Encourage children to cooperate, be empathetic and to meet centre expectations.

Supervise children at all times.

Staff will:

Establish clear, consistent and simple expectations and implement appropriate

consequences if required.

Work with the children to establish additional group expectations.

Acknowledge children’s feelings.

Discourage aggressive play and assist children to learn and practise resolving

conflict.

Offer choices that are developmentally appropriate.

Demonstrate respectful affection and caring to each child.

Use verbal direction and redirections rather than physical prompting.

Develop individual care plans for children who require additional support.

~ Supervision ~

Willow Creek Childcare staff has a duty to ensure all children are cared for and educated

in a safe and secure environment. We achieve this through active supervision that is

essential for creating such an environment, allowing staff to observe and respond

appropriately to all children in their care. Through thoughtful arrangement of the centre’s

environment we can reduce the likelihood of accidents and injury. Also through active

supervision staff are able to ensure that children’s learning and play are promoted and

opportunities to enhance experiences are

Families will:

Actively pass their child on to staff upon arrival, not just drop at the door.

Staff will:

Ensure the ratio of children to care providers is consistently met as per licensing

requirements.

Conduct frequent head counts throughout the day.

Actively supervise children through interaction and not policing.

Arrange furniture and activities to promote active supervision and safety for all

children.

~ Health & Safety ~

Immunization

As immunizations are one of the most effective ways to preventing the spread of

communicable diseases, we recommend that families have their child’s immunization up

to date prior to starting in the Centre. Once enrolled, we must have an up to date record of

your child’s immunizations on file. If parents choose not to immunize their child (ren),

the child will be required to not attend in the case of an outbreak of communicable

disease happens.

Illness

Parents are required to keep a child home if the child:

• Has a communicable disease.

Has a fever over 38 C or 100 F

Has a skin infection or an undiagnosed rash.

Is not well enough to participate in all program activities including outdoor

play.

Child has been on antibiotics for less then 24 hours

Parents must notify the Centre if a child has a communicable disease so that other

families can be notified. Children must be kept at home for at least 24 hours after their

last episode of vomiting or diarrhea.

Staff will notify all other parents attending the centre if a communicable disease has been

reported.

If your child becomes ill during the day, we will attempt to contact you. If you are

unavailable we will try to reach one of your emergency contacts. We will provide a quiet

resting place, close to staff supervision until you, or one of your emergency contacts can

pick up your child. If the situation becomes urgent, we will follow the emergency

procedures outlined below.

Head Lice Policy

Parents must keep children at home if they find head lice in their child’s hair.

Parents will be contacted if staff notices lice and will be expected to pick up

their child as soon as possible.

Children may not return to daycare until all nits have been picked out of their

child’s hair.

Administering Medication

Staff will only administer medication prescribed by a doctor.

All over the counter medications must have written instructions from a doctor

before the staff will administer them.

The parent must complete a Permission to Administer Medication form,

which is signed by both the parent and staff.

All medications must be in the original container and be clearly labelled.

Medication will be stored in a locked container following the storage

instructions.

~ Emergency Procedures ~

If a child is injured or becomes ill while at the Centre, Staff will immediately assess the

situation to decide what action/attention is required. Outlined below are three

procedures that may be followed.

First Aid Treatment Only

If first aid treatment is required, staff will:

1. Provide first aid treatment.

2. Acknowledge the child’s feelings.

3. Provide close supervision to ensure the child does not require further first aid or

medical treatment.

4. Complete an incident report form and process.

5. Inform the family when they pick up the child or call if family has requested to

be informed on the Minor Incident Notification Form.

Non-Emergency Medical Attention Required

If medical attention is required, a staff will:

1. Contact the family/guardian.

2. If the family/guardian is not available, staff will contact of the emergency

contact(s).

3. If the family/guardian or emergency contacts cannot be reached, contact the

child’s doctor.

4. If the child’s doctor is not available, proceed as if it is an Emergency Medical

situation.

Once the family/guardian or emergency contact has been contacted, a staff will:

1. Provide information to the doctor and family/emergency contact.

Emergency Medical Attention Required

If emergency medical attention is required, qualified staff will administer First Aid until

the ambulance attendants arrive. Staff will:

1. Call and request an ambulance.

2. Contact the family/guardian and arrange to meet at hospital.

3. If the family/guardian is not available, staff will get hold of the emergency

contact(s) andarrange to meet at the hospital.

4. Provide doctor and family/emergency contact with information about the

emergency.

5. Complete an Incident Report and process.

Fire Drills

Evacuation procedures will be explained to the children and fire drills will be

practiced monthly and recorded at each centre. Evacuation plans are posted at the exit

doors within the daycare facility.

Evacuation Procedures

The safe evacuation of children is the first priority.

Children will never be left without adult supervision.

1. Call Out FIRE, FIRE, FIRE

(If safe to do so notify all people in building)

2. Exit Business (Take emergency kit if possible)

- If children can not walk, childcare providers are to carry children out of

building, or pass safely out window.

3. Relocate to Medical Building (one staff if safe to do so will stay behind to

give further information to fire department upon arrival)

4. CALL FIRE DEPARTMENT- Lead Staff will Call

5. Account for all children and staff

- Use attendance sheet from clip board if you were able to take with evacuation.

If in the event this is not possible. Each program leader will check for

attendance from memory.

6. Update Fire Department on status.

7. Check all children for any signs of medical/emotional distress. Call 911 for

medical attention if needed.

8. Call Facility Emergency Contact Personnel to assist with children.

9. Notify all parents, and ask to come get children immediately.

10. Check children for signs of medical/emotional distress every 5 minutes.

11. Once all children are removed contact Northern Health Licensing Office

and notify Licensing Officer (250)-631-4222 of current situation. Owner to fill

out incident report. If in the event owner is away acting manager will complete.

Centre Closure & Emergency Disaster Procedures

In case of fire, extended power outage, water/heat failure, extreme weather

conditions or an evacuation due to safety of the facility, the Centre may have to close.

The staff will care for children until families/emergency contact(s) have picked them up.

Evacuation Procedures will be posted at the exits and carried out as written.

If the centre needs to be evacuated, staff will begin the emergency evacuation

procedure. Willow Creek Childcare will evacuate to Scott Mitchell’ s 4623 Soucie. Staff

will then call families for pick-up instructions.

If the centre is closed for only a day, parent’s fees will still be taken to cover staff

wages. If the closure lasts for more than day, parents will receive a credit on their next

month’s bill.

~Emergency Preparedness Plan~

Extreme Heat

In the event that Terrace has an extreme heat wave the daycare will remain open and we

will follow these steps to ensure child safety.

• We will take children outdoors in the early morning hours when it is coolest.

• We will have plenty of shaded areas available for the children to play.

• We will apply sun screen to those children that have parental consent.

• We will offer and have many opportunities for children to hydrate.

• We will keep the children indoors from noon to closing as this is the hottest part

of our days.

• We will turn on air conditioner.

• Parents will be responsible for all school pick up and drop offs.

Extreme Cold

In the event that Terrace has an extreme cold warning the daycare will remain open and

we will follow these steps to ensure child safety.

• We will limit the amount of time the children are outdoors.

• We will require all children to come with adequate outdoor clothing.

• We will provide extra clothing for outdoors if needed.

• We will keep the children indoors when the weather advisement states to do so.

• We will keep our home warm indoors.

• Parents will be responsible for all school pick up and drop offs.

In the event that this extreme cold causes a power outage we will follow these steps to

ensure child safety.

• We will call the parents to tell them of our situation.

• We will keep children for 2 hours.

• We will layer the children in extra clothing.

• We will attempt to start our generator that powers our whole house.

In the event that we are unable to power our center we will follow these steps to ensure

child safety.

• We will call the parents to tell them of our situation.

• We will keep children until the temperature drops to an uncomfortable level for

children to play.

Flood

In the event that the Skeena River floods its banks in our city we will follow these steps

to ensure child safety. Keeping in mind the location of our home is not in jeopardy.

• We will remain open for care but offer extended hours to those families that will

have to take alternate routes to get to our home.

• We will provide opportunities for the children voice their concerns about the

flooding.

• In the event that the water in our city is undrinkable we will provide bottled water,

juice boxes or milk as alternative beverages.

• We will follow any order given by the city when it comes to water safety.

Wind Storm

In the event that the City of Terrace has a major wind storm we will take the following

steps to insure child safety.

• The children will be kept indoors to prevent injury from flying debris, falling

branches, trees, and fences.

• If the power is cut off due to one of the above, we will attempt to power the house

by gas generator.

• If we are unable to do so we will remain open until it is too cold for the children

to play comfortably.

• We will provide extended hours of care to allow time for the children to picked up

safely by their parents.

• Parents will be responsible for school pick up and drop offs.

Chemical Spill

In the event that the City of Terrace has a chemical spill we will follow these steps to

ensure child safety.

• We will listen to the local radio for instructions on air quality and safety.

• We will keep the children indoors until we are notified of air safety.

• We will evacuate only under direction from the City, Fire Department or RCMP.

Forest Fire

In the event that our city has a Forest Fire we will follow these steps to ensure child

safety.

• We will remain open for regular daycare hours.

• We will continue to have regular outdoor play unless air quality states we should

keep children indoors.

• We will listen to local radio for safety updates.

Earthquake

In the event that an Earthquake strikes outside of daycare hours we will close our daycare

to ensure of building safety.

In the event that our City has a catastrophic Earthquake during daycare hours we will

follow these steps to ensure child safety.

• We will remain at our home, unless our building has become insecure.

• If we are in danger to stay at the building we will post a sign on the front door and

evacuate to 4623 Soucie, Scott and Laurie Mitchell. 250-635-3079.

• If instructed to evacuate sooner we will proceed to the location given to us by the

City, RCMP or Fire Department, we will post the location in which we asked to

relocate to on the front door.

• We will attempt to call or text the parents of the children to inform them of our

evacuation location.

• We will post on our door where we have relocated to.

• We will provide for all children until they are released to their parents.

~ Personal Safety ~

Visitors to the Centre

Centre has “open door” policy for parent/guardians. Parents are welcomed to

visit anytime although staff may not have the time to interact with parents if the

centre is busy.

To ensure the safety of children, families and staff, visitors must get permission

from the program coordinator prior to visiting the Centre.

Community resources/partners and immediate family members of children

enrolled are welcome to visit as long as prior arrangements have been made.

Unauthorized visitors will be asked to leave the premises.

Suspected Child Abuse

As required under the Child, Family and Community Services Act, if staff has

reason to believe that a child has been or is likely to be physically or

emotionally harmed, neglected, sexually abused or sexually exploited by a

parent or other person, the staff will “report” to the Ministry of Children &

Family Development (MCFD)

If unsure, staff will call the Ministry and speak to the intake worker to “get

information”. (The decision to investigate is always in the hands of the

ministry).

Staff will document and date all observations which will be kept in the child’s

file.

If a child discloses being at risk or likely to be at risk, staff will contact MCFD

and report. Staff will not interview children, but reassure them.

Suspected Child Abuse while at the Centre

Staff will take steps immediately to protect children from the person alleged to

have or likely to abuse them.

Staff will contact MCFD and Community Care Facilities Licensing (CCFL)

immediately as required by law.

Staff will also contact the Terrace Child Care Society Board liaison.

Staff will work with the Board, MCFD and CCFL to ensure the safety of the

children.

Apprehension by MCFD

The Child, Family and Community Services Act define when a child is in need

of protection. Decisions related to how, when and where MCFD apprehends a

child rest with the Ministry.

If and when a Social Worker from MCFD intends to apprehend a child in the

centre, prior to the child being removed, the centre staff will verify with the

Ministry’s Protection Team Leader that the social worker has the authority to

remove.

MCFD is responsible to make all reasonable efforts to notify the child’s family

of the apprehension.

If no custody agreement exists and conflict between parents and/or their

families is evident, the centre may refuse to care for the child unless both

parents sign a written agreement confirming the details of pickup and access to

information about the child.

Conflict Resolution

Families are encouraged to discuss questions or concerns regarding all aspects of

the program with staff. If an issue arises, the goal is to resolve differences of opinion and

find acceptable solutions.

The following steps will be takes:

Step 1 Enrolling parent/guardian will meet with staff to define and state point of

view.

Step 2 Solutions and/or appropriate resources are identified, if appropriate.

Step 3 Both enrolling parent and staff will agree to a plan.

Step 4 If all parties cannot agree to a plan, other childcare arrangements may be

required. The enrolling parent may appeal in writing to the Board only if

Steps 1, 2 &3 were followed. If there is no resolution with the Board,

other childcare arrangements will be required.

Staff will maintain a bulletin board in the pickup/drop off area to share

On request, staff will meet with parents privately and at mutually convenient

Staff will evaluate the program minimally once a year.

Feedback from parents will be included in the evaluation.

MINOR INCIDENT NOFICATION FORM

Changes to Child Care Licensing Regulations (section 55) require childcare staff to notify

parent parents or emergency alternate immediately if their child becomes ill or is injured.

Some parents do not want to be notified every time their child sustains a minor injury,

however, to meet childcare standards we must notify you unless you provide us with

written instructions stating otherwise. If you do not want to be contacted for a minor

incident until the end of the day, please indicate below or provide written instructions

stating when you want to be notified.

IMPORTANT:

Parents or emergency alternatives will be notified immediately if the minor incident

involves the head as the slightest bump could be serious.

$15 per 15 minutes for extended hours beyond scheduled time will be added to

your bill unless previous arrangements are made with manager of Willow Creek

Childcare.

Fees are due on the 1st of the Month for the following months care; unless

previous arrangements have been made with manager of Willow Creek Childcare.

Fee’s are preferably paid by postdated checks given at time of registration for 6

months at a time. Alternatively fee’s can be paid by check, email money transfer

to [email protected], money order, or cash.

Half-Day fees will be charged if you cancel care due to child’s illness beyond 5

days per year. 10 days per year no charge for booked holidays/ leave of absence.

CARE ARRANGEMENT/CONSENT GUIDELINES

I will not send my child to daycare if my child is ill and I will notify childcare

provider if child is contiguous. I will not send my child to daycare if my child has

any sign of lice or nits.

I give permission for the staff of Willow Creek Childcare to apply child safe

sunscreen and child safe bug repellant as deemed necessary by staff.

I give permission for staff will take pictures of my child to be used at the Centre

for the enjoyment of children, families and staff. We will request written

permission if we want to use the photos of your child for other purposes.

FIELD TRIP CONSENT

An integral part of our program is planning for experiences and activities outside

the Centre and in the community. If possible, parents/guardians will be informed

in advance and a written permission slip will be provided, outlining our plans.

I give permission for my child to participate in spontaneous walking trips with

care provider, as long as information on location is posted on childcare centers

door. I also, give permission for my child to use off site playgrounds as defined in

parent handbook. I understand I will be notified in advance.

POOL CONSENT

I give permission for my child to play in a (less than 1.5 foot) pool, while

attending Willow Creek Childcare. There will be no more than five children in the

pool at one time, this will also depend on children’s size and comfort level while

in pool.I understand that children will be closely supervised.

I understand the pool water status will be continually observed (pool will be

shocked with ½ tsp of chlorine at the end of each day). And water changed no

more than every two days

TRANSPORTATION CONSENT

I give permission for my child to be transported by Willow Creek Childcare

authorized business vehicles for the purpose of transportation for field trips or

pick up to and from school.

At all times possible I understand the 18-passenger school bus with Class 4

drivers license driver, and clear abstract will be used.If in the event of a smaller

outing or transportation need I understand that a alternative business vehicle may

be used.

All children not yet 9 years old will be restrained in a car seat/booster seat

dependent on the Motor Vehicle Act and ICBC recommendations. Care provider

will have a valid driver’s License. I understand in the bus a car seat/booster seat is

not used as defined by the Motor Vehicle Act and ICBC recommendations.

I give permission for my child to be transported by Willow Creek Childcare as

long as driver holds a valid driver license, with applicable Classification, and a

clear drivers abstract. Also, that there is additional liability insurance for the use

of the vehicle for this purpose.
