Local Solutions In 2014/15 - Improving Access to Psychological

Therapies (IAPT) enhanced service pilot for patients with Long Term Conditions.

Help patients and carers prevent avoidable hospital admissions.

In 2015/16 - Expanded to include people with dementia – working to reduce anxiety, low mood and stress within the household.

Across 2014/15 - 6% of all referrals to the IAPT pilot were for people with dementia. By month 5 in 2015/16 - 16% of all referrals were for patients with dementia

To consider the whole person, including other chronic and long term conditions and the potential impact of medication
