4. In accordance with 10 CFR 30.36(b) and/or license condition, notify NRC, promptly,in writing, and request termination of the license:

when you decide to terminate all activities involving materials authorized undera.the license; or

b. if you decide not to complete the facility, acquire equipment, or possess anduse authorized material.

5. Request and obtain a license amendment before you:

a. change Radiation Safety Officers;

b. order byproduct material in excess of the amount, or radionuclide, or formdifferent than authorized on the license;

add or change the areas of use, or address or addresses of use identified in thec.license application or on the license; or

d. change ownership of your organization.

6. Submit a complete renewal application with proper fee or termination request at least30 days before the expiration date of your license. You will receive a reminder noticeapproximately 90 days before the expiration date. Possession of byproduct materialafter your license expires is a violation of NRC regulations. A license will notnormally be renewed, except on a case-by-case basis, in instances where licensedmaterial has never been possessed or used.

In addition, please note that NRC Form 313 requires the applicant, by his/her signature, toverify that the applicant understands that all statements contained in the application are trueand correct to the best of the applicant's knowledge. The signatory for the application shouldbe the licensee or certifying official rather than a consultant.

You will be periodically inspected by the NRC. Failure to conduct your program inaccordance with NRC regulations, license conditions, and representations made in yourlicense application and supplemental correspondence with NRC will result in enforcementaction against you. This could include issuance of a notice of violation, or imposition of a ]civil penalty, or an order suspending, modifying or revoking your license as specified in the .

Radiation Safety Program

1. Radiation Safety Officer

A. Greg W. Banner has been designated as SE Technologies' Radiation SafetyOfficer and will assume the duties and responsibilities that include the fotowing:

1. To ensure that all terms and conditions of the license are being met andthat the information contained in the license is up-to-date.

2. To ensure that the equipment has been leak tested in the required timelymanner and that the leak test is performed in the manner prescribed bythe equipment manufacturer.

3. To ensure that the use of the equipment is only tey individuals that have ,

been authorized by the Radiation Safety Officer and that all users wearpersonnel monitoring equipment when utilizing the equipment. Personnelmonitoring equipment will consist of TLD's supplied by Troxler RadiationMonitoring Services on a quarterly exchange period.

4. To maintain the records as required by the license and the regulations.These records shall include personnel exposure records, leak test recordsand training certificates for all users.

5. To ensure that the equipment is properly secured against unauthorizedremoval at all times when it is not in use.

6. To serve as a point of contact and give assistance in case of emergency'

such as equipment damaged in the field or theft and to notify the properauthorities in case of emergency.

. 7. To ensure that all users have read and understand the radiation safetyoperating and emergency procedures.

2. Operating Procedures

A. Transportation of Equipment

1. All possible means will be provided to ensure that the equipment securedin the transporting vehicle and the equipment is away from the passengercompartment. When transporting in an enclosed vehicle (van), the vehicle .i

will be locked. When transporting in an open vehicle, the gauge will be qsecurely fastened and locked to the truck bed. ;

2. The gauge will be transported in the custom transportation case providedby the manufacturer. The Department of Transportation requires that the !gauge be transported in a properly labeled carrying case. j

i

3. At all times during transport, the operator will have a properly completedBill of Lading for each gauge.

B. Utilization Procedures1

1. When the gauge is in the field, SE Technologies will maintain control over jthe gauge at all times. The gauge will never be left unattended. 1

2. When not making measurements, the gauge will be placed in thetransportation case and returned to its permanent storage area as soonas possible. The gauge will be used for its intended purpose only. Bydoing so, SE Technologies will be able to maintain any radiation exposureto as low as reasonably achievable (ALARA).

3. When using the equipment, SE Technologies' technicians will wear thepersonnel monitoring device that has been assigned to us by Troxler.When not using the equipment, the monitoring device (s) will be stored inthe radiation-free area that has been designated in our office.

C. Maintenance and Leak Test Procedures

1. Periodic maintenance will include cleaning the gauge. During anymaintenance, our personnel monitoring device will be worn. The acceptedcleaning and lubrication procedures developed by the manufacturer will befollowed.

2. No maintenance will be performed in which the radioactive source isremoved from the gauge. For this type of maintenance, the gauge will bereturned to the manufacturer.

3. The leak test will be performed using the Troxler Model 3880 Leak Test Kit.The leak test will be performed using the manufacturer's instructions.Again, the personnel monitoring device will be employed. Gauges will beleak tested at intervals not to exceed six (6) months.

D. Record Keeping in Regards to Facility Decommissioning

Records of information important to the safe and effective decommissioning of thefacility will be maintained in an identified location, until the license is terminatedby the Commission. The file will be entitled " Facility Decommissionina File." Therecords will include the following:

1. Records of any leakage involving the spread of contamination, where thecontamination remains after cleanup procedures have been exhausted,and/or if the contamination is inaccessible.

2. Drawings or sketches of arcas in the facility where radioactive materialsare used and/or stored. These drawings willindicate locations of any non-removable contamination.

3. Records of the cost estimate for the decommissioning of the facility.

3. Emergency Procedures

A. In the event of physical damage to a gauge, the following will be performed:

1. The area around the gauge will immediately be cordoned off. An area, aminimum radius of 15 feet, will be maintained.

2. Should a vehicle be involved, it will be stopped until the extent ofcontamination,if any,is established.

3. A visual inspection of the gauge will be made to determine if the sourcehousing and/or shielding has been damaged.

4. At the earliest possible time, when the situation is under control,SE Technologies' personnel will contact Greg W. Banner at (412)221-1100, extension 604. SE Technologies' personnel will describe thepresent conditions and follow the instruction of the Radiation SafetyOfficer.

B. In the event the gauge is lost or stolen, the Radiation Safety Officer will be notifiedimmediately.

Item 11 - Waste Manaaement

Disposition of the gauge will be by transfer to either another licensee specifically licensed topossess the radioactive material or to a licensed disposal facility. The manufacturer will assistin locating a properly licensed disposal facility.
