The gangway platform got detached from the vessel, causing the platform and the gangway to drop approximately 0.5 to 1 meter until it left hanging in the securing ropes and the safety net. The securing pin slid out of the gangway platform because of the varying force and was not fitted correctly. Vessel was moored by use of a Yokohama fender and the wind was pushing the vessel against the quay. Securing Ropes proved to be effective. The slot of the lever is positioned at the bottom and at the outside of the gangway securing point; therefore it is hard to see if the lever is properly fitted into it Use of a Yokohama fender in combination with the wind caused more movement of the vessel. Corrective Actions Taken The gangway was installed in the proper way again and an extra securing rope was installed at both pins. It was added to Risk Assessment for deploying the gangway that the securing pins have to be double checked for their proper installation while the gangway is deployed. Additional pin is used to prevent the securing pin from moving. Specific mooring situations or requirement to be advised prior to vessel’s berthing so same can be included in the Risk Assessment.
