Supervisors:Dr.MikeJones&Dr.CorrinaCoryProject:BiomechanicalAssessmentofRibFracturesinChildrenwithSuspectedAbuseTheaimofthestudywastoconductabiomechanicalevaluationofribfracturesinchildren,byapplicationof engineeringprinciples.A literaturebased surveyof thoracic injury toleranceof thehumanthorax andtheoretic biomechanical analysis of mechanismsof injury and forces acting on the thorax to producedifferentpatternsof rib fractures in childrenwere studied.Our results showed that ribfracturesof theposteriorarcarethecommonestpatternnoticedinchildrenwithsuspectedabuse.Theyresultfromforcesproducedbecauseof shakingandantero‐posteriorcompressionofthethorax.This leadstoleveringof therib over the transverse process resulting in a fracture. These fractures are usually missed on plainradiography and requireahigh index of suspicionfor diagnosis.Cardiopulmonary resuscitation doesnotleadtoproductionofribfracturesinchildren,as theposteriorribarcis notabletoleveroverthetransverseprocesstoproducesuchafracture.

Supervisors:Prof.KABalasubramanian&Prof.GKangProject:OxidativeStressResponseinGramNegativeGutBacteriaTheeffectofoxidativestressintheformofexogenousH2O2onthesurvivalof fourspeciesofShigella andtwo nonpathogenic Gram negative gut bacteria and the role of catalase as an antioxidant enzyme,neutralizing theeffect of H2O2wereexamined.A significantdecrease in thenumber of colony formingunits(CFUs) afterexposure to exogenousH2O2(122+/‐ 37), comparedto control bacteria(218+/‐ 63,P<0.001)was observed.TherewasaninductionofcatalaseactivityafterexposuretoexogenousH2O2andthespecificactivityofcatalaseinH2O2exposedbacteriawassignificantly increased(2.88+/‐ 1.25),com‐paredtocontrolbacteria(1.5+/‐0.44;P<0.05).Adirectcorrelationwasobservedbetweenthedecreaseinbacterialcountsand increaseincatalaseactivityafterexposureofH2O2(regressioncoefficient0.56).Gutbacteriaappeartobesusceptibletooxidativestressandinduciblecatalaseactivitymayformanimportantpartoftheantioxidantdefensemechanismagainstoxidativestress.

TheVancouverclassificationsystemwasusedfortheaudit.Practiceswerecomparedtotertiarycentersandchangesrecommended.

3.Mortalityrelatedtofractureoftheneckoffemur‐2001.OldchurchHospital,RomfordThis auditprojectlookedatthemortalityrelatedtotheneckof femurfracturesandfactorsresponsibleforthesame.Theriskfactorswereidentifiedandachangesuggested,decreasingthemortalityrate.

4.SuccessofMRCSDiplomates’infindingaSpRposition–2000.RoyalCollegeofSurgeonsofEngland,LondonThis projectwasrun inassociationwiththeRoyalCollegeofSurgeonsof Englandonthepositionof SHOsafterpassingtheMRCSexams.ThepurposewastoassesswhethertheDiplomates’havemadeittotheSpRgradeorarethey“stuck”inthesystem.AfurtheranalysisonfactorsresponsibleforgettingaSpRpositionwasalsocarriedout.

5.AssessmentofankleinjuriesbasedontheOttawaanklerules–1999.NewhamUniversityHospital,LondonAtwo‐stagedauditwassetupinaDGHinLondon.Thefirststageinvolvedlookingatthenotesofpatientswith ankle injuries who presented to A/E over a period of three months with reference to clinicalassessment and basis for requesting radiographs. Thesecond stage involved teaching theOttawaanklerules tothejuniors intheDepartmentandre‐auditing thenotesafterthreemonthstoelicitadifferenceifany.

6.Roleofthromboprophylaxisinhipandkneereplacementsurgery–1998.NewhamUniversityHospital,LondonThepercentageof patientsdevelopingDVTafterhipandkneereplacementsurgerywascalculatedovera6‐monthperiodandriskfactorsleadingtothesamewereanalyzed.Athromboprophylaxisprotocolfortheunitwassuggestedbasedontheresultsoftheanalysis.

CAREERPLANS

I am keen on continuing to develop my career as a Clinical Academic Orthopaedic Surgeon with asub‐specialtyfocusonLowerLimbArthroplastyandSportsSurgery.My fellowships and initial years as a Consultant in Lower Limb Arthroplasty and Sports Surgery havestimulatedaresearchinterestinarthritisoftheyoungandearlyinterventioninthesame.My main aim in themedium term is to look at the modeling and epidemiology of femoroacetabularimpingement and outcomes following surgical intervention in this condition. In the longer term I aminterestedinthedevelopmentofabiologicalsolutiontochondralandlabraldamageinthehip.IamalsoverykeenonmedicaleducationandtrainingandtothateffecthavebeeninstrumentalinsettingupcoursesfortheFRCS(Orth)exams;andNationalandInternationalHipandKneeMeetings.Finally,Iseemyself leadingateamofwell‐motivatedOrthopaedicSurgeonsspendingapartofmytimeinclinicalwork,apartinclinicalresearchandapartinundergraduateandpostgraduateteaching.
