As part of the 8th grade social studies curriculum, we will betraveling to Cantigny in Wheaton, Illinois. The students will havethe opportunity to explore both the historic house museum andthe First Division Military Museum.

COST: $7.00

ATTENTION:Students must bring a brown bag lunch with a drink, wearclosed-toed/tennis shoes and wear comfortable outdoor

clothing. Dress for the weather because we will go rain or shine!Chaperones Needed!!We will need one chaperone for every ten students to accompanyus on our field trip. If I wish for my student to receive any medication on this trip, I must eithersubmit a Field Trip Medication Information Form or Extended Trip Medication Form to my studentsschool.

I hereby waive any claims against the Board of Education and its individual members, employees,agents, and assigns arising out of my students attendance on this student trip and agree to holdharmless and indemnify the Board of Education and its individual members, employees, agents, andassigns, either jointly or severally, from and against any and all liability, claims demands, damages, orcauses of actions or injuries, costs, and expenses, including attorneys fees, resulting from or arisingout of my students attendance on this student trip.
