CONNECT YOUR COMMUNITIES WITH PATIENT CENTERED MEDICAL HOMESSeveral MEDITECH sites have received some level of NCQA certification for Patient-Centered Medical Homes, and we anticipate many of our customers will want to modify their organizations to fit this new model in the years ahead. Our focus is on providing our customers with a balance of integrated and interoperable solutions to connect providers and patients across care communities, while also providing robust tools for reporting outcomes and securing reimbursement. Our focus is on developing solutions to help healthcare organizations better coordinate their patients’ ongoing treatment and maintain a continuous plan of care between various inpatient and outpatient settings.

FOR CONTINUOUS CARE, THERE’S NO PLACE LIKE HOMEUpon referring patients to home health or hospice environments, your organization can instantly exchange pertinent information between care settings—such as demographic, contact and insurance information; medications; allergies; orders; and documentation—resulting in a smooth and accurate transition. MEDITECH’s Home Care automatically triggers employees to complete tasks based on timeframes or the completion of prior tasks, thereby preventing bottlenecks. Visiting providers can document OASIS assessments using agency-defined policies and procedures to ensure protocols are met. SHARING INFORMATION CREATES OPPORTUNITY You don’t need to be on the same system as your ACO counterparts just to share information. The ACO model is about creating opportunities, not putting up boundaries. To exchange information our customers send and receive CCD documents to and from other vendor systems, and store data discretely in their system. We also collaborate with Forward Advantage to provide our Data Express interoperability suite, which is designed to receive Ambulatory orders inbound and transmit electronic patient results and report data from MEDITECH hospitals to any number of affiliated care providers, insurance companies, and medical suppliers. For Direct Messaging, MEDITECH supports the Direct protocol without requiring a vendor-specific HISP, an interoperability “subscription,” or transaction fees.

Our customers are also contributing to Health Information Exchange (HIE) projects across the globe as well as submitting data to public health agencies. We currently exchange immunization data with public health agencies in 50 states or territories. We are also demonstrating our Interoperability leadership through our involvement in the Argonaut Project, a collaboration to speed the development and adoption of HL7’s Fast Healthcare Interoperability Resources (FHIR) Specification across the healthcare industry. In addition, as a member of the CommonWell Health Alliance, we continue to position our customers, as well as the healthcare industry, to successfully exchange data across the continuum of care. 6
