IntroductionLymphoedema is a chronic condition which, if untreated, has the potential to deteriorate. One of the elements of successful treatment is the use of quality compression garments.
medi UK offers the most comprehensive range of sizes and styles off the shelf (OTS) of compression garments. However, we are aware that some patients require bespoke garments that address their individual physical requirements. Therefore we are able to offer a comprehensive made to measure (m2m) range.
This m2m booklet is designed to guide you through the process in order to choose the most effective m2m garment for your patient. In the back of the booklet you will find tables for your final choice of product summarising the available options and codes.

Overview of tailor made compression hosieryOur aim Our aim is to provide you with top quality made-to-measure garments that are both comfortable and effective for your patients. Each garment is tailor made specifically to your patients needs. Every m2m garment from medi UK is manufactured in Germany and we aim to get it to you as soon as possible. The time taken to do this will depend upon the complexity of the garment and the colour choice; our average turnaround is six working days for round knit garments, eight working days for flat knit – trend colours may take a little more time due to the extended dying process.
What we need from you to help make the perfect garment
Measurements A measurement form with all relevant parts completed. Accurate measurements are a very important part of this process and it is a combination of length and circumference measurements that will allow us to make a comfortable and effective garment. Medi m2m pads are supplied with a measuring guide, if you would like further details on how to measure accurately please contact your medi Account Manager or our team in Hereford.
Product details In addition to the measurements we need to know the fabric, style of garment , compression class and colour of the garment before we can start making it for you. If one or more of these details are missing we will have to come back to you for clarification and this may delay the manufacturing process. Our m2m forms are laid out to help make sure that these essential details are provided.
Optional extras These are extra items that may help your patient with their garment – for example a zip, a silicone top band or a silk lining. Sometimes it helps with complex requests if you send in a diagram or photograph with the order form; this will be passed to our manufacturing team to help us to get it right first time.

RAL compression classesmedi m2m garments comply with the strict standards that are laid down by the 'Reichs-Ausschuss für Lieferbedingungen' (RAL) Institute. This is the gold standard for compression garments. All medi m2m garments are guaranteed to provide effective compression for 6 months of daily use if used and laundered correctly.
Selection of the correct compression class (ccl) will help to ensure that your patients receive a garment that is suited to their condition:
RAL class 1 (18 – 21mmHg)Prevention or early treatment of lymphoedema, venous disease and for use on palliative limbs.
RAL class 2 (23 – 32mmHg)Mild / moderate lymphatic conditions, pronounced varicosis with oedema present and after healing minor ulcers.
RAL class 3 (34 – 46mmHg)Lymphoedema, significant varicosis or after healing long standing ulcers; use if condition fails to respond to class 2.

Note
m2m hosiery is tailor made for your patient. In order to help our specialist m2m department tailor the garment to your patients requirements including photos and diagrams can help this process. Photos can help with manufacturing of complex garments (these photos are only used for the manufacturing process).
At times compression is not always appropriate. In the following situations mediven compression is contraindicated.
Contraindications:According to the guidelines of the German Phlebological Society (published in 1996, German Journal ’Phlebologie‘)
Absolute contraindications:Advanced, peripheral arterial occlusive disease, decompensated heart disease, septic phlebitis,phlegmasia coerulea dolens.
Relative contraindications:Suppurative dermatoses, intolerance of compression stocking fabric, sensory disturbances of the limb, advanced peripheral neuropathy, primary chronic arthritis. medi assumes no liability as a result of any contraindicated use of this product.

Tips
medi always produces flat knit products with an oblique border.
The standard is ‘flat oblique’ = approx. 5% of the circumference. A ‘steep oblique’ = approx. 10% of the circumference and can be selected as an option. The obliques optimise the fit around the top of the thigh.

Tips
medi produces all flat knit garments with an ‘oblique foot border’. This guarantees the best possible fit to follow the anatomical line of the metatarsophalangeal joints. It also prevents constriction of the forefoot. The ‘oblique border’ is standard for both open and closed toes. The oblique border of the compressive knit is given by the difference between the medial foot length lAi and the lateral foot length lA. The largest possible oblique is 4 cm.

The incorporation of Y- knitting marks at the ankle holds the foot in about 90° flexion.

The long sole optimises the fit in the forefoot region. It can be advantageous where foot deformities such as hallux valgus or hammer toe are present.Knitting the compressive knit along the sole of the foot gives the stocking a circular grip at the metatarsophalangeal joints.This stops the foot section slipping back towards the heel.The length of the long sole depends on cA and is produced automatically.

Tips
The ellipse form enhances comfort for people who spend most of their time sitting down. It compensates for the longer distance at the knee when sitting and thus reduces the pressure on the patella. Always give the measurement ‘l (length)back of the knee’. This is measured close to the body from the fold in the back of the knee to the heel with the patient standing. Note that lE must be at least 1 cm longer than ‘l (length) back of the knee’.Attention! For patients with voluminous knees (lipoedema) or a large cE measurement, the incorporation of ellipses increases wrinkling in the knee region. Wrinkles may also form below the patella. Do not use ellipses here.

For AD stockings the incorporation of a zip on the inside of the leg is recommended – this zip fastener is similar to the zip on a boot. For AF-AG garments no zip should be sewn on the inside of the thigh. Zips on a pant section can be positioned either on the lateral seams or centrally on abdomen.

During pregnancy, the circumference measurements of the legs show almost no changes if appropriate and optimum treatment is continued. In the abdominal area, however, sizes often change very rapidly. The gentle compression from the soft front panel is designed to accommodate this.

Correct fitting of compression stockingsIn order to make an m2m garment we need to know the circumference measured at various points on the limb and exactly where they were measured (the length from a known point). Circumference and length measurements are taken in a distal to proximal direction. The measuring points on the leg must be marked so that the circumference and length measurements are taken at the same location. Our measurement forms are designed to make this process as simple as possible. Other details that we require as a minimum are the compression class and colour.
