Case 1ผูปวยชายอายุ 25 ป ปวดหลังมา 1 เดือนปวดราวไปที่ขาขวาลงไปถึงนองและเทา ปวดมากเวลากมตัวหรือน่ัง เวลานอนดีขึ้น อาการปวดมาก รบกวนชีวิตประจําวัน ไมสามารถทํางานได ชาบริเวณนิ้วหัวแมเทาขวา อุจจาระปสสาวะปกติ ไดรับการรักษารพ.ใกลบาน กินยาแลวอาการไมดีขึ้น VAS 9/10

compression แตหลังจากใหการรักษาแบบอนุรักษ 2-4 สัปดาห แลว

อาการปวดหลังสวนลางไมดีขึ้น

Treatment

Indication for emergency surgery

1. Cauda equina syndrome ซึ่งมักสัมพันธกับ acute large central disc herniation2. Progressive neurological deficit3. Severe neurological deficit or multiple nerve root involvement

10 RCTs were reviewed: 8 out of 10 suggested only 2 days of best rest is enough. Rest 2 days vs 7 daysRest 2 days is better functional ability and return to work faster.
