At a minimum of every six months, the leadership team uses their data to develop a new action plan, with at least three goals, to guide their implementation of the Pyramid Model. These sample action plans are examples of how the plan guides the work of the leadership team. The team reviews and updates the plan at each meeting and revises accordingly.
