A lot has happened across the MeNA region since we launched iNsiGHT back in september. We kicked off the start ofAutumn with an incredible two day event in Dubai - Dentsply sirona Desert Fest. We welcomed 6 speakers from allaround the globe to deliver free of charge education and hands-on training, allowing attendees to learn about a widerange of topics including digital impression taking, root-to-crown and digital integrated implantology. We also, for thefirst time, showcased with dedicated Dentsply sirona booths at the Moroccan Dental Meeting that took place inMarrakech in October and the Pan Arab meeting that took place in Cairo in December – it was a real pleasure for us toget closer to our customers in Morocco and egypt and give them an opportunity to meet some of our wonderful teamwho are devoted to the MeNA region. During the past few months we have also been busy launching some excitingnew products. in November we held our Astra Tech implant system® eV launch with the help of Dr. Fernando Rojaswho ran incredible introductory trainings for our customers in uAe. We also continued the launch of Celtra® Duo inuAe with an evening of education from Dr. Mohamed Hassenien in November and we finally got to run our first Celtra®Press launch trainings in Tunisia, Jordan and uAe – you can read more about all of our major events on pages 22-25. inDecember we welcomed Prof. simone Grandini back to uAe to continue our celebration of the Class ii procedure at anextremely special event at the Roxy Cinema in Dubai. 80 customers joined us to hear more about why the Class iiprocedure shouldn’t be ignored and to learn about tips, tricks and protocols to get the most out of this procedure. Theevening was continued with live demos and fun with our Dentsply sirona photo booth and finished with a privateviewing of the movie The Mountain Between us – you can read more about it on page 20.

if our first edition of iNsiGHT celebrated the Class ii procedure then our second celebrates the magic of CeReC. in thisedition you can read all about how there is more to CeReC than just offering patients single visit dentistry. On pages4-5 you can meet our dedicated service engineers who are on hand across the MeNA region to partner with our CeReCand equipment users, offering direct technical support and service, to enable them to focus on delivering improvedpatient outcomes. The findings of the Futuredontics® dental patient survey offer dental offices a practical guidefor improving patient acquisition and retention. As this report illustrates, accommodating yourpatients’ insurance requirements should be a primary concern of your practice.

Today’s patients also expect a greater degree of convenience and comfort from dentists ineverything from appointments to advanced treatment technology. Again, it is critical that youare proactive in addressing these issues.

introductionAnterior single-tooth restorations can place high demands in terms of treatment skills and dental materials.The challenge above involved adapting the tooth as precisely as possible to the adjacent teeth, requiring ahighly rational procedure, a high level of craftsmanship, and excellent materials. Light, shade, shape, andthe three-dimensional appearance had to be imitated in a natural way.

ConclusionThe patient case presented here shows the great potential of the interaction of the zirconia-reinforced lithiumsilicate (ZLs) Celtra® Press (in this case used as a framework material), and the low-fusing, leucite-reinforcedfeldspathic material Celtra® Ceram veneering ceramics.

The special microstructure of Celtra® Press, its 3-point bending strength of more than 500 MPa and its biaxialstrength of more than 700 MPa, coupled by a fine crystalline structure and a high proportion of glass, lended theframework excellent optical properties. The symbiosis of translucency and opalescence provided by Celtra® Pressalso allowed a surprisingly natural chameleon effect in the case shown, an ideal shade basis for the highly delicateveneer.

The cut-back framework, which had been reduced only in the vestibular area, was veneered with incisal ceramicsonly. Thanks to its excellent material properties, Celtra® Ceram exhibits no shrinkage, so that the aesthetic goalwas achieved in just a few steps, maximising efficiency. The prepared tooth, framework, and ceramic veneerpresent a harmonious interplay of shades. The “anterior single crown” challenge was mastered perfectly withouta complex ceramic build-up.

AbstractPatient: 39-year-old patient with a fractured ceramic crown on her upper leftlateral incisor.

The Challenge:

The fractured crown was to be replaced as quickly as possible by along-lasting and aesthetic restoration.

The Treatment: Minimum repreparation of the tooth and an impression using theputty/wash technique. A Celtra® Press crown framework wasfabricated and individually veneered with Celtra® Ceram at the dentallaboratory. The new crown exhibited an impressive fit and greataesthetics.

Dr. Roze and Associates Dental Clinic is located in Jumeriah 3, Dubai. Theclinic comprises of predominately Dentsply sirona equipment as part oftheir commitment to offer the best possible service to their patients, withCAD/CAM technology at the heart of their practice. Dr. Hubert has beenworking at Dr. Roze for 4 years, specialising in CeReC. We caught up withhim to find out how CeReC has benefited his workflow, his patients andthe practice.

Q. Please briefly explain your background in digitaldentistry.

A. My university in the south of France, Montpellierfirst introduced me to digital dentistry. They had aCeReC BlueCam machine which they allowedstudents to practice on and discover the workflow. Assoon as i used this machine, i believed that CAD/CAMtechnology would be the future of dentistry – andnow it’s everywhere! in order to stay relevant in themarket, you have to keep up-to-date with the latesttechnology otherwise you will fall behind yourcompetitors. We want to offer our patients the best,and CeReC is the best.

Q. Why did you choose CeReC over otherCAD/CAM systems?

A. i chose CeReC naturally as this is what we had atthe university and were taught with. But even when ihad the chance to discover other systems, i felt morecomfortable with the CeReC workflow. everything isso smooth, well synced, and works well together. sowhy would we need to use a different system otherthan CeReC?

Q. Can you explain your experience with the support youreceive from Dentsply sirona?

A. We have strong support from Dentsply sirona from A-Z. Ourclinic is actually predominately Dentsply sirona - treatmentchairs, imaging units, CeReC, and consumables.

Our service engineers and our CAD/CAM consumables salesrepresentative Joseph Magdy really help to ensure a smoothand easy life with CeReC.

Q. How do you find the CeReC workflow?

A. Honestly it’s perfect. it does take more time than the regularprocess - for one restoration it can take around an hour and ahalf, or two hours if its highly aesthetic. But the end result isworth it – i’m happier as i deliver a better result and the patientis happier as everything is completed within a single session.

Q. What are the types of cases you do with CeReC?

A. everyone in our practice uses the Omnicam for scanning theteeth, especially our 2 orthodontists. some are not comfortablewith milling or shading and glazing, but i spent a lot of timeusing it in the past so i’m comfortable with the wholeprocedure. i mainly do single restorations, because for onerestoration you have to anticipate at least an hour maybe morewith the patient. everyone is using the Omnicam at the clinic,from our general dentist with the single visit restorations, to our2 orthodontic specialists scanning the arches for the invisalign®1

cases or for creating in-house immediate digital surgical guideswith our 2 surgeons.

Q. What would you say are the advantages to yourworkflow and the practice by using CeReC?

A. it changes your way of working definitely. if youdon’t have the CAD/CAM technology in house,usually you see one patient every 30 minutes butnow with the CeReC i have fewer patients butbigger sessions. so i can take more time to perfectmy work and have less stress - no patients waitingin the waiting room. At the end of the day it’sbeneficial to everyone.

Q. How has CeReC, and single-visit dentistry,affected the satisfaction of your patients?

it’s very good as the patient sees the wholeprocess from start to finish, even the chairsideworkings. it’s nice to share what we are doing withthe patients when they are in the chair, they loveit. They enjoy seeing the milling machine working– this is why we keep it in the patient waiting roomso they can take pictures!

By doing everything in one session we avoid usingmultiple anesthesia and temporary crowns. Wealso get more accuracy with digital impressions asCeReC trains us to do good prep. Aesthetically, wecan achieve a better colour by working chairsidewith the patient. if i use a lab, i have to sendpictures to my technician – the quality of thepictures is never as good as in real life.

Q. in your opinion, can you achieve a high returnon investment with CeReC?

A. i am not involved directly in the figures but iknow that there are 6 doctors here using theequipment – 2 orthodontists and 4 generaldentists. There is a huge demand for anotherOmnicam machine now! in terms of the full CeReCsystem, i know that in the long term we ourreducing our lab costs overall.

Q. What would you say was your goal with CeReC,and would you say you have achieved this yet?

A. First and foremost my goal was to deliver highlyaesthetically pleasing restorations to my patients.i have only reached my goal because of CeReC.When i work with the labs either in France or Dubaithere is always something that needs changingwith the restorations. With the CeReC machine itallows us to design the tooth ourselves, so we cancreate the shape, colour and details that we want.There’s no comparison – why send the work to thelab who doesn’t know how we work or what wewant!

introductionAs a practicing endodontist and a clinical endodontic educator for more than 30 years, the most frequent question i am askedabout technique is: “is it predictable?” While every dentist wants his or her endodontic treatment to be easier, more efficient, simpler,and profitable, in the end, it is predictability that trumps all considerations.

When i am asked the question “rotary vs reciprocation: how do i choose?” my answer is “rotary and reciprocation,” because thepredictability of both methods is similar; they are just different in sequence, purpose, and motion. This article should help to explainthese 3 critical distinctions of rotary vs reciprocation so that the clinician is aware of these differences, understands his or heroptions, and makes the best choice for his or her particular practice and patient needs. it is an invitation to be self-educated aboutyour best way to produce endodontic preparations that can be easily obturated.

The Greatest Variablein all of dentistry, the greatest variable is always the clinician. While product and operatory infrastructure play a significant role,the answer to rotary vs reciprocation depends mostly on technique, and technique depends on the clinician’s skill, care, andjudgment.

Q: How Do i Choose?A: Take the ChallengeHere is a simple and revealing test for each clinician to determine his or her own preferred “rotary vs reciprocation” choice. speakto your local Dentsply sirona sales representative and explain to them that you want to do this “challenge” test: Purchase enoughrotary files (ProTaper Next® or ProTaper Gold™) and enough reciprocation files (WaveOne® Gold) to treat 10 endodontic patientswith rotary and 10 endodontic patients with reciprocation. You could treat every other patient alternately with rotary andreciprocation, or you could treat 10 patients in a row with rotary and then 10 with reciprocation. Reverse the order if you prefer. Youcan use this same telltale test for comparison with your current preferred system. Take good notes about what worked and did notwork. Your answer for rotary vs reciprocation will be right in front of you!

Closing Commentsusing predictability as your critical benchmark distinction, your own testing will reveal your best choice of “rotary vs reciprocation.”The result: clinical confidence, consistency, and control. The Primary W1G (which has similar external geometries to PTG F2) cuts theentire length of the radicular preparation using a multipass technique.

The “crown-down” approach with a single file is asking that single file to dothe work of 2 or more files. The motion is simple and similar to the RotaryFinishers in “Follow and Brush, Follow and Brush, Follow and Brush” to length.

The big difference, however, is that with each “Follow and Brush,” or “pass,”the Reciprocation file should be removed to avoid blockage, and cleaned ofdentin filings. The canal should be irrigated, patency should be confirmed withthe No. 10 manual file, and the canal should be irrigated again. Then proceedwith “pass” No. 2 and so on until preparation physiologic terminus length isreached. Fit the WaveOne® Gold cone or Verifier to confirm the shape.

The Concept for Class II RestorationDelicate Preparation, Rapid Procedure andReliable Results

Composite restorations have always been an attractive option. They provide aesthetic, tooth-coloured restorations and allow for minimal invasive treatments that preserve as much naturaltooth substrate as possible. Thanks to advancements in material properties (universal adhesives,bulk fill composites, aesthetic composites), composite restorations are continuously getting morereliable and convenient. All-in-one treatment solutions (e.g. Dentsply sirona’s Class ii solution™)further increase convenience and efficiency, as the following patient case illustrates.

The "Class ii solution™" represents a safe and rapid complete system, true to the motto "Class ii– everything from a single source". As such, major success and high levels of patient satisfactioncan be achieved in daily routine.

Case PresentationThe 23-year-old male patient came for a routine examination. Followinga visual examination, in order to rule out proximal caries, bite wing imageswere made from both sides. The radiological assessment of tooth 15confirmed the clinical diagnosis which, on the basis of a distal opacity ontooth 15, suggested a carious lesion and resulted in lucency up to themiddle of the dentin (Fig. 1). The logical therapeutic step was a minimallyinvasive restoration. After being provided with visual and oral information,the patient agreed to the treatment.

The next steps are self-evident: A delicate preparation which sacrificesthe minimum amount of healthy tooth substance as possible (Fig. 2). Anexcellent adaptation and a well-shaped anatomical marginal ridge wereaccomplished using the sectional matrix system, Palodent® V3. The matrixand wedge were applied safely by simply using the associated forceps.The wedge was inserted as far as it would go to ensure a good marginalseal and an adequate contact point. The interproximal distance wasdetermined perfectly using the ring to result in good cervical adaptation(Fig. 3).

The cavity was then etched with 36% phosphoric acid, starting in theenamel area (approx. 20 to 30 seconds) with the areas close to the pulpconditioned for the shortest amount of time (approx. 10 to 15 seconds)(Fig. 4). A suitable amount of bonding agent, Prime&Bond universal™,was applied into the cavity, creating a homogeneous adhesive layer,which was air-dried and light cured. Prime&Bond universal™ with thepatented Active Guard™ Technology is versatile in use for all etchingtechniques and indications. The good balance between hydrophobic andhydrophilic properties ensures an ideal surface tension and thus facilitatesthe active, complete penetration and coverage of the cavity surfaces.

Class ii Clinical Case

Following polymerisation of the adhesive layer, the cavity had a glossy appearance and the filling could beplaced (Fig. 7). The cervical margin of the cavity was lined with sDR® Plus, which is easily achieved due to themetal application tip (Fig. 8). This section was finally polymerised (at least 20 seconds) to produce a gap-freecervical seal. in the next step, the cavity was restored in one go using ceram.x® sphereTeC™ (Fig. 9). ceram.x®sphereTeC™ is a nano-ceramic, light-curing, radiopaque universal composite based on the innovative sphereTeCfiller technology with granulated spherical fillers in combination with an optimised resin base for direct andindirect restorations. The material is characterised by a pleasant consistency and excellent modeling propertiestogether with first-rate stability. The so-called CLOuD concept covers the entire ViTA®1 Classical system withjust five shades. The good colour adaptation of the filling to the adjacent teeth is down to the pronouncedchameleon effect.

Discussion and Conclusionin this patient case, the combination of an anatomically shaped matrix system, a homogeneous, and easy tocreate adhesive layer, an excellent cervical adaptation and well-adapted and aesthetic occlusal layer, lead to anexcellent final restoration. The check-up two days later demonstrated perfect colour match and a barely visiblefilling – to the great satisfaction of the patient (Fig. 10).

in terms of handling, the universal composite used is particularly worth mentioning. This product offers a uniquecombination of good modeling properties and excellent consistency.

The advances made in adhesive technology have also played a key role in establishing composite fillings as anessential part of everyday routine. universal adhesives that combine the three treatment options (etch&rinse,self-etch, selective enamel etching) have further spurred this development; and in some cases go even further,offering additional benefits like active moisture control and active spreading in the cavity.

Overall, the patient case demonstrates just how efficient, safe and reliable posterior tooth fillings can be created– particularly if all the components are combined into a complete system for Class ii restorations.

The aim of the event was to bring our customers together,not only for a fun-filled evening, but to also demonstratethe benefits of the Class ii solution™ and highlight thestrengths of the portfolio of products for successfuloutcomes, with a lecture from Prof. simone Grandini andlive demonstrations.

We welcomed over 80 customers to the Roxy Cinema inDubai where the evening began with a walk up the redcarpet for a short drinks reception, which was closelyfollowed by a 60 minute lecture in the cinema auditoriumby Prof. Grandini. After the lecture there was a break, inwhich dinner and drinks were served in our own privatelounge, where we hosted live product demonstrations that

allowed attendees to get further details of the productsthat were covered in the lecture and ask any questionsabout the importance of the Class ii solution™.

The evening ended with our guests picking up theirpopcorn and drinks before settling back down in thecinema for a private viewing of a block buster movie.

On the following day we hosted 2 hands-on workshopswith Prof. simone Grandini at our Dentsply sironaAcademy in Dubai. 25 attendees were taken through bestpractices and Prof. Grandini shared his tips and tricks whenusing the products in the Class ii solution™.

Astra Tech Implant System® EV Launch.Presented by Christian erneklint, from sweden and Dr.Fernando Rojas, from spain, these events launched Astra eVinto the Dubai and Abu Dhabi markets to over 125 participantsin total. The event started with lectures on the background towhy a new system was developed and why this led toevolution. it went into details of the BioManagementComplex, and how it applies to the eV system, alongside thephilosophy behind the eV system and the Crown downapproach concept. Then different cases were presentedillustrating a wide variety of surgical and restorative protocols,where the use of the different abutments is presented in detail.

3 City Endodontic Tour.Dr. Francesco Mannocci, from the uK, joined us in Kuwait first where,reflecting to the needs of the market, the focus was on ProTaper® Gold.Overall it was a very popular and successful event with a total of 27attendees.

The next day, Prof. Francesco Mannocci visited Dubai to deliver a coursein our Academy. M-Wire® rotary files have become extremely popular inrecent years, and as such this lecture illustrated the properties and theclinical results obtained using continuous rotation M-Wire® rotary files.

Prof. Mannocci also visited several clinics in Abu Dhabi and trained thestaff directly on ProTaper® Next as well as conducting a lecture andworkshop in our Abu Dhabi showroom. We had a total of 19 attendeesin Dubai and 19 in Abu Dhabi.

Pan Arab Conference.

Prof. Giuseppe Cantatore, from italy, joined us for the Pan ArabConference where he presented 2 lectures on the “Latest Trends inendodontics using ProGlider®, ProTaper® Next and 3D Obturation withGuttaCore®.” We had more than 100 attendees for both lectures, and alsoconducted 2 workshops each on the ProTaper® Next and WaveOne® Goldsystems. We had 20 participants for each of the workshops. At the sameconference, we also had a 15x6m stand dedicated to showcase Dentsplysirona products from imaging, CAD/CAM Technology and Celtra® Duo,Prosthetics, endodontics and Restoratives.

Class II Perfection and CompositeTips and Tricks Workshop.

Following on from the Cinema event (read more on page 16), Prof. simoneGrandini, from italy, conducted 2 hands-on courses in our Dentsply sironatraining centre to 25 participants – half who had travelled from saudi Arabiato attend. This focused on the Class ii solution™ products (Palodent® V3,Prime&Bond universal™, sDR® Plus, ceram.x® sphereTeC™), whilst alsoadding in composite tips and highlighting some of the anterior tricks as well.

CEREC Masters Club Event.

We proudly hosted the first CeReC Masters Club event for ourexisting customers in the uAe. We welcomed 64 dentists to theConrad Hotel in Dubai where Dr. Andreas Kurbad, from Germany,presented insightful lectures and demonstrations.

The CeReC Masters Club provides a forum for CeReC users toshare ideas, best practice, receive the latest updates fromDentsply sirona and benefit from the most up-to-date trainingsfrom some of the best global CeReC trainers.

True Opalescence with theCeltra® Press System.

On 16th and 17th December, we educated 15 labs on ‘TrueOpalescence with the Celtra® Press system’ through two full-day trainings in Dubai and Jordan.

Lino Adolf, from the uK, presented this training andparticipants were taken through three indications – crown,veneer and a 3-unit bridge. The crown and veneer were pre-milled in wax, enabling us to start the workflow from thesprueing stage. in Tunisia, Yamen Chaban, from Germany, took 20 participantsthrough a detailed hands-on course focusing on Celtra® Ceramand the layering technique. He detailed how and when to usethe more aesthetic shades to create true opalescence and aresult which is undistinguishable from surrounding teeth.

Celtra® Duo Launch.On November 16th and 17th 2017 we proudly hosted a Celtra®Duo launch training to 35 CeReC users in the uAe. Theevening started with a technical overview of Celtra® Duo, whichthen led into a clinical demonstration and workshop, allpresented expertly by Dr. Mohamed Hassanien, from egypt.The event was a chance to remind our CeReC customersabout the benefits of Celtra® Duo and how it could help themdeliver a final restoration with complete confidence withrespect to clinical, functional, and aesthetic outcomes. As theattendees were already somewhat familiar with Celtra® Duo,we focused on additional aspects of the restoration process,such as polishing techniques and staining and glazing, whichare sometimes a challenge for users.
