Hunterdon Radiological Associates, P.A. (HRA) is our Diagnostic Radiology
Corporation; it consists of the Brems Imaging Center and the Imaging Department of Hunterdon Medical Center. This Company performs all of the Bone Densitometry, CT Scan, Mammography, Ultrasound and Xray services.
Hunterdon Imaging P.A. (HIPA) is a separate company than HRA. HIPA
consists of the latest diagnostic imaging technology (MRI). HIPA has two locations one in Flemington and the other located in Annandale.
HRA and HIPA are made up of ten Board Certified Radiologists, they are: 7 owner/partners and two employee Radiologists, they are as follows: Mark Malzberg, MD Radiologist/Partner Andrew Loesberg, MD Radiologist / Interventional Radiology /Partner

All employees are paid on a bi-weekly basis.
Payroll runs from Monday being the first day of payroll and Sunday being the last day in a
two week period.
Payday is on the Friday after the last Sunday of the period.
Direct Deposit is available
Direct contributions on an employee’s behalf to the Federal Credit Union at the Hunterdon
Medical Center is available.
Employees are permitted to contribute to their 401K after their 90 day probationary period
has been completed.
Open Enrollment for Medical Benefits is March of every calendar year. At that time, changes
and additions can be made to your insurance coverage, unless you experience a qualifying
event during the year defined as either a marriage, divorce or legal separation, birth or
adoption of a child, a change in your spouse’s employment and insurance status, a dependent
ceasing to meet eligibility requirements and a change in residence that affects coverage.
**please note that should you experience a qualifying event and need to make a change in your coverage, you must contact the HR
Department within 30 days of the event and complete the appropriate paperwork, If you fail to notify your insurance carrier within 30
days of the event, you will not be able to make a change and will be required to wait until the next open enrollment or another
qualifying event, whichever comes first.
All employees working 1250 hours per a twelve month period are eligible for FMLA
benefits, consisting of 12 weeks of unpaid leave. This leave must be approved by
management prior to the leave. All necessary completed doctor forms must be returned
within 15 days of receipt. The twelve month period is a rolling calendar year.
All employees working an 8 hour shift are entitled to 45 minute lunch break, all employees
working 4 hour shift are entitled to a 15 minute break. ** Please note, HRA/HIPA DOES
NOT recognize any other breaks, i.e. morning or afternoon break.
In order for eligible employees to utilize doctor time, a note from your doctor must be
presented at the time of payroll. If no note is available the time will be counted as sick time.

Employees can access their own personnel file with HRA/HIPA through our HR Online
software through Paychex. This site provides employee’s copies of W2’s direct deposit
information, vacation requests and totals, demographic information and W4. To access this
website, staff members should go to services.paychex.com/secure/. Employee User Names
are the first letter of first name and full last name all in lower case no spaces. The Company
ID is 0418 in box 1 and N793 in box two. During your initial login, the employee’s
password: FIRST LETTER IN FIRST AND LAST NAME CAPITALIZED last 4 digits of
social security number; upon entering the employee will be prompted to change the password
to one of their choice.
All HRA/HIPA employees are eligible for all the following benefits once their 90- day probationary
period has been completed. HRA/HIPA offers the following benefits:
Medical Insurance – Horizon BC/BS – Omnia 11 with Blue Card
The Horizon BC/BS OMNIA plan is OPEN ACCESS; provides coverage on an in-network basis
only; and does not require the selection of a Primary Care Physician or obtaining referrals for
visiting specialists. The cost for this coverage is a required employee contribution of $80.00 for
single coverage or $170.00 for family coverage bi-weekly. Accessing care from OMNIA Tier 1
Providers will result in the best financial benefit for you and your family members. To locate
participating providers, visit www.horizonblue.com and select “Find a Doctor”. When searching for
Omnia providers select the Omnia” Plan. The provider search will identify those that are in Tier 1
and those that are in Tier 2.
Dental Insurance – Horizon BC/BS – Dental Choice Plan and PPO Access Plan
Taking care of your teeth is as important as taking care of the rest of your body. That is why we
offer dental plan that covers routine check-ups and additional services needed for your dental health.
Your dental plan offers coverage for four main types of expenses:
Preventative & Diagnostic Care (routine exams, cleanings, fluoride treatments, sealants & z-
rays)
Basic Treatment (simple fillings & extractions, root canals, oral surgery and gum disease
treatment)
Major Treatment (crowns, dentures and bridges)
Orthodontic Services

TREATMENT PER LIFETIME
The cost for the Dental Choice Coverage is a required employee contribution of $8.00 for single,
employee and spouse, employee & children or family coverage or $16.00 bi-weekly. For the
Dental PPO Access coverage is a required employee contribution of $5.00 for single, employee and
spouse, employee & children or family coverage or $10.00 bi-weekly.

Life Insurance – America One Life Insurance (AUL)
All full time employees are eligible for Basic Life/AD&D insurance. This coverage consists of 2X
Annual Base Salary to a maximum of $200,000. Guaranteed issue amount is $200,000. Benefits
reduced by 50% at age of 70. AD&D is equal to life insurance benefit. This coverage is NO COST
to the employee and is paid by the company at 100%. Also, if the employee is no longer employed
with HRA/HIPA the employee has the option to continue the coverage directly.
Long Term Disability Insurance – UNUM
All full time employees are eligible for 50% of monthly earnings to a max benefit of $5000 per
month. With a 90 day elimination period. Benefits paid to age 65 (reducing benefit duration).
Additional Life Insurance – UNUM
All full time employee are eligible for $5,000 benefit amount, benefit reduces to 65% at age 70; and
50% at age 75.
Voluntary Life Insurance Program
All full time eligible employees who wish to supplement their life insurance benefits may purchase
additional coverage. Premiums are paid by the employee via payroll deduction. Additional benefits,
exclusions and limitations apply. – Employees – minimum $10,000 base amount to an overall; maximum of the lesser of
$500,000 or 5 X annual salary, limited to 50% of the employee’s coverage. Guarantee Issue amount is $1000,000. Spouses –
minimum $5,000 base amount to max of $250,000- Children – life amounts of $2,500, $5,000 or $10,000. AD&D amounts equal
to Life Amounts. Benefit reduces starting at age 70.
401(K) Retirement Plan – Mass Mutual
HRA/HIPA offers all employees working at least a minimum of 1200 hours in a twelve month
consecutive period a 401K Retirement Savings Plan through Mass Mutual. Employees must be at
least 21 years of age. Employees who meet the criteria are able to start contributing after their 90
day probationary period has been completed. The 401K Plan provides a convenient, tax advantage
way to save for retirement. These elective deferral contributions can be made with per-tax and/or
Roth dollars.
Mass Mutual offers a diverse array of investment options covering objectives from conservative to
aggressive.

FSA (Flexible Spending Account) - Paychex
HRA/HIPA offers two different FSAs administered by Paychex, that helps you save money on
routine expenses
The Health Care FSA allows you to use money that is not subject to income tax to pay for
most-out-of-pocket medical expenses – including copays, prescription drugs, vision and
dental expenses. You can contribute up to $2,750 (2020) to your Health Care FSA.
The Dependent Care FSA allows you to use money that is not subject to income tax to pay for
child care and adult day care while you and your spouse (if applicable) work or go to school.
You can contribute up to $5,000 per year to your Dependent Care FSA.
Company may make a Safe Harbor non-elective contributions of
3% of pay. (immediate vesting and/or discretionary
payments(subject to vesting).
