HOW TO UPLOAD DOCUMENTS

Getting Started

Sign into your CareDox account and click Start or Continue Enrollment.

Care Plan for Medical Condition

To add a Care Plan for a Medical Condition, click the Medical Conditions section and answer YES to the question, “Does the student have any medical conditions?”

Then enter the name of the condition (e.g. Asthma). Click the name of the Care Plan to download it. Once completed, you can upload it below by scanning the pages to your computer or taking pictures of the document on your mobile device and clicking upload.

Name your Care Plan, and choose a single document from your computer or multiple images from your mobile device or camera and press Okay to upload.

Once the document has been uploaded, you will see the name and date on which you uploaded the document.

To add a Care Plan for Allergies, click the Allergies section and answer YES to the question “Does the student have allergies?”

Then enter the type of allergy (e.g. Shellfish). Either click the required care plan or add your own care plan by clicking the Add Care Plan button.

Choose a single document from your computer or multiple images from your mobile device or camera and press Okay to upload to the form.

Care Plan for Diet Restrictions

To add a Care Plan for Diet Restrictions, click the Diet Restrictions section and answer YES to the question, “Does the enrollee have any special diet needs?”

Choose the diet restrictions (e.g. Vegan). Click the name of the Care Plan to download it. Once completed, you can upload it below by scanning the pages to your computer or taking pictures of the document on your mobile device.

Immunization Cards

To upload an Immunization Card, go to the Immunizations section and click the Upload Card button.

Add a brief description, and then select a single document from your computer or multiple images from your mobile device or camera and press Okay to upload.

Insurance Cards

To upload an Insurance Card, go to the Immunizations section and answer YES to the question “This enrollee is covered by family medical/hospital insurance or Medicaid?”

Insurance Cards

Then enter the necessary information and click the Upload Insurance Card button to upload a picture of the insurance card.

Medication Permission

To upload a Medication Permission Document, click the Medications section and answer YES to the question “This enrollee is covered by family medical/hospital insurance or Medicaid?”

Then click Add Medication and select a medication type: Over the Counter (OTC) or Prescription. Enter the necessary information and then select who will administer the medication.

Download the Medication Permission document by clicking the name of the document. Then upload it by clicking Upload Document for Medication Permission.

OTC Medication Permission

To upload an OTC Medication Permission Document, go to the OTC Medication section. Click the blue Download button to download the document. Click the green Upload button, and choose a single scanned document from your computer or multiple images from your mobile device or camera.

Physical Examinations

To upload a Physical Examination, go to the Screenings Section (or Forms section) and click Add New Physical Examination.

Enter the date of the screening. Then click Upload Screening File (Physical Examination).

Download the form by clicking the name of the document. Then upload it by choosing a single scanned document from your computer or multiple images from your mobile device or camera and pressing Ok.
