Spring 2011
Alberta Cross joined the PACE CNY program when she moved into her daughter Ann Latocha’s home. Ann suggested her mother join PACE CNY because she wasn’t as active and was losing her social connections. Ann also needed help coordinating her mother’s care.
Ann says, “PACE CNY has changed my mom’s life in just two months. It really has helped me overcome my fears of taking care of my mom because PACE provides wonderful care, friends and most importantly, love.”
Prior to PACE CNY, Ann was scheduling appointments with a variety of doctors, transporting her to appointments and was having trouble working full time and caring for her mother.
Since enrolling in PACE CNY, Alberta is very active. She socializes with people her age and enjoys a change of scenery. She loves going to the Day Center and the activities it provides including a dog show, a trip to Lights on the Lake and live entertainment. Not only does Alberta benefit socially, but she receives medical care
and coordination from an interdisciplinary team of doctors, nurses, social workers, nutritionists and therapeutic recreation specialists.
Ann says, “When my mom attends the Day Center, she appears more vibrant. She selects her outfits the night before and gets excited to go back to her second home.” Together, Ann and PACE CNY strive to keep Alberta as independent as possible in the home she loves with the care she needs.

PACE CNY Flu Shot Clinics Serve 300 People in 2010
The flu season is upon us and hopefully you have received the influenza vaccination. Despite vaccination, the winter seems to be filled with a variety of respiratory illnesses, so it’s important to practice good hygiene to reduce the risk of acquiring or transmitting the influenza or other respiratory viruses to each other. The flu virus, colds and other upper respiratory viruses are transmitted through nasal, oral and lung secretions that can travel approximately six feet through the air after a cough or sneeze.
We can all help reduce the risk of transmitting respiratory infections by frequent hand washing, covering your mouth and nose when coughing or sneezing, and staying home to avoid unnecessary close contact if you have a cough or runny nose.
Rest assured, you can continue normal activities and reduce the risk of passing along any respiratory infection by washing your hands frequently, carrying tissues and
covering your mouth and nose when you cough or sneeze.
If you have difficulty breathing, high fever associated vomiting, and change in mental status, please call your doctor as soon as possible. If there is any question that you are experiencing serious symptoms related to a respiratory infection, do not hesitate to call the medical clinic or your physician. PACE provides on-call services of a team of nurses, aides and physicians to answer questions and help manage illnesses 24 hours a day.

I’m very pleased to announce that PACE CNY will offer services in the Town of Van Buren at Loretto’s O’Brien Road Senior Apartments.
At the new site, PACE CNY participants will enjoy therapeutic recreation activities, a mid-day meal, a light snack and companionship in a safe and nurturing environment. The program is open to adults 55 and over in the area who are enrolled in PACE CNY.

Mary Santorelli is a 9-year PACE CNY participant. She enjoys playing physical therapeutic recreation games including Cornhole and Ladderball. She says, “I like to play games because it keeps me active and my brain stimulated.” She also likes to play blackjack with her friends and PACE CNY staff members. When Mary isn’t playing games, she takes a break and visits the Day Center hair stylist.
Yvonne Howard joined PACE CNY after her husband passed away. She lives independently with supportive care services from PACE CNY aides that come to her apartment. Yvonne says, “I love PACE CNY because it gives me a place to go to during the day and I’m cared for and safe.” Yvonne also enjoys outings to restaurants, shopping and the theater. She feels blessed to be part of a program that helps keep the elderly and disabled in their homes as long as possible.
The Papaiannis recently joined PACE CNY and are thankful for the program because their lives are now more organized. They are very pleased with the medical care and physical therapy at the Day Center. Joe jokes with his wife and says, “I need to do physical therapy so I can continue to chase my wife around the house.” The Papaiannis love to play games and socialize with their friends.
Delores is a 2-year PACE CNY participant. She likes attending the Day Center because she enjoys sharing her sense of humor with her friends and staff. She also enjoys physical therapy, art therapy, spending time on the Day Center patio in the summer and outings in the community. Delores says, “PACE CNY is a nice place to be and a great place to make friends.”

Medical Care and ManagementPrimary care physicians and nurses work in both PACE CNY clinics. Daily medical management of specialty care includes dental, podiatry and vision. Emergency, hospital, nursing home and end of life care are also available. A nurse and doctor are on call 24 hours. Home Care ServicesNurses provide coordination of care, skilled nursing assessments and medication management. The aides assist with personal care including bathing, dressing, meal preparation and medication reminders.
Social WorkersSocial workers help by listening to participants and providing emotional support and guidance. They alert the PACE health care team to potential participant and/or family concerns.
Day Center (Dining and activity room pictured at right)Recreation therapists provide person-centered programs, activities, social opportunities, events and outings. A hot lunch and bag supper are also available. Aides can help with transferring, toileting and bathing.
Nutrition Staff Dietitians provide assistance and educate participants about healthy food choices and manage dietary restrictions. Meals on Wheels home food delivery service is available.
Rehabilitative ServicesPhysical, occupational and massage therapists work with participants to help increase strength, flexibility and mobility. Pictured to the right is participant Janet Hill with Cherie Duvall, Rehab Aide.
Transportation Drivers pick up participants and take them to the Day Center, clinic and to outside doctor’s appointments. Aides accompany those who need assistance.
Medical Equipment/Lifeline Emergency Response SystemPACE CNY has a wide variety of medical equipment available. Theinterdisciplinary team determines the type of medical equipment to beused based on each participant’s specific needs.

One beneficial aspect of PACE CNY is our interdisciplinary team approach to long term care of the elderly. Each day, the team gathers to discuss any findings or concerns about
