How am I selected for Care Coordination?You are selected to participate if you meet one or more of the following:

■ You have had more than three crisis contacts in the previous 12 months

■ You received inpatient detoxification, crisis, or hospital admission

■ You have high behavioral health care and physical health care needs

■ You are a child or adolescent in a Level III or Level IV group home placement or Psychiatric Residential Treatment Facility

■ You are a child or adolescent discharged from a Youth Development Center or a Youth Detention Center

Who works with me?You work with a Care Coordinator who is familiar with the behavioral health system. The Care Coordinator works with you, your family, legal guardian, and service providers to ensure that you are receiving the services you need. It is up to you to decide who you want involved to help you achieve improved safety, health, and recovery.

How does Care Coordination help me?If you choose to participate in Care Coordination, we work with you for an average of 90 days. During this time, Care Coordination:

■ Links you to needed or requested services

■ Monitors your Individual Service Plan (ISP) and/or Person Centered Plan (PCP)

■ Helps you establish an individual Wellness Recovery Action Plan® (WRAP ®)

■ Helps you transition back into the community

■ Partner with Community Care of North Carolina (CCNC) and your medical professional

■ Connects you to transportation and crisis services

Am I required to participate in Care Coordination?You are not required to participate in Care Coordination. However, participation has helped many achieve their objectives and goals. Your services or benefits will not be affected if you choose not to participate in Care Coordination.

What is Mental Health/ Substance Use Disorder (MH/SUD) Care Coordination?Care Coordination is designed to help you develop a plan to meet your goals in recovery and life. We also help you access the services related to your behavioral and physical health care needs. You and your care coordinator work together to identify what is important to you and create a plan to achieve it.
