Chronic Pelvic Pain (CPP)

Chronic Pelvic pain
• Chronic pelvic pain can be defined as intermittent or constant pain in the lower abdomen or pelvis of a woman of at least 6 months in duration, not occurring exclusively with menstruation or intercourse and not associated with pregnancy.
• It is a symptom not a diagnosis
CPP
• Difficult to diagnose
• Difficult to treat
• Difficult to cure

CPP
•Affects 15-20% of all women in the reproductive age group
•Accounts for 20% of all diagnostic laparoscopies in women
•Accounts for 15% of all hysterectomies in women
Why?
•There is frequently more than one component to chronic pelvic pain
•Physical, psychosocial , emotional and social factors

Causes of Pain
Causes of Pain• Hormone related:
▫ Endometriosis▫ Adenomyosis
• Adhesions• Irritable bowel syndrome• Interstitial cystitis• Musculoskeletal• Nerve entrapment• Psychosocial issues
Assessment•History
▫Appropriate referral to urologist, gastroenterologist, physiotherapist, psychologist , psychosexual counsellor etc.
•Examination•Investigations
▫Screen for infections▫USG▫MRI▫Diagnostic Laparoscopy
Red Flag Symptoms• Bleeding from the rectum• New Bowel symptoms after
age 50 years• New pain after menopause• Lump in the abdomen• Irregular bleeding after age
40 years• Post coital bleeding• Excessive weight loss• Suicidal ideas
Treatment
•Treatment of specific gynaecologic disorder if any
•Hormonal therapy for cyclical pain•Antispasmodics for irritable bowel
syndrome / interstitial cystitis•Lifestyle changes•Pain specialist
