at 397 Louisiana Street in Buffalo. It is a collaborative initiative aimed at ensuring vulnerable patients – those who otherwise might fall through the cracks – have access

to nutrition education, free health screenings, wellness programs, and other resources

focused on preventing heart disease.

WHO CAN JOIN? The program is free and open to the public. Participants do not need to be current Catholic Health or Mercy Comprehensive Care Center patients.
