Welcome to the Fall/Winter edition of Healthy You! While many of us are taking advantage of the last days of warm weather before the comforts

of the fall season give way to the chilly bite of winter, it’s important to remember not to neglect our orthopaedic health as we enjoy brisk walks outdoors, play games of touch football in the backyard, or train for marathons that may require a few extra layers of clothing.

With potential sports-related injuries in mind, we here at Murray-Calloway County Hospital are pleased to announce changes that will only enhance the strength of our orthopaedic services. Charette, Morgan, and Compton Orthopaedics has changed its name to Murray Orthopaedics and expanded.

Dr. John Yezerski, a surgeon specializing in hands and a longtime practitioner in Murray, has officially joined the group. Learn more about Dr. Yezerski on page 3.

In addition, two new physicians have opted to make Murray Hospital their home, and we are very fortunate to welcome Dr. Ghanshyam Shastri, Pulmonologist, and Dr. Nicholas C. O’Dell, Internal Medicine Physician. On page 6 you can read about their backgrounds and what they hope to contribute to our community.

DR. JOHN YEZERSKI JOINED fellow orthopaedic surgeons Ray Charette, Stephen Compton, and Derek Morgan on August 1 to form Murray Orthopaedics. Dr. Yezerski—a former Chief of Surgery and Chief of Staff at Murray-Calloway County Hospital—specializes in hand surgery and relishes the intricate and varied nature of the work.

“Because the hands are so complex and delicate, hand surgery is somewhat more labor intensive than other orthopaedic work in terms of monitoring how wounds heal and rehabilitation progresses,” Dr. Yezerski says. “I could go from fixing a small nerve in one case to putting plates and screws on a bone in another case. I like that variety.”

One of the most innovative treatments Dr. Yezerski offers is Xiaflex® injection to relieve Dupuytren’s contracture of the hand—a change in tissue texture that draws the small and ring fingers toward the palm.

“The gold standard of Dupuytren’s contracture care is still surgical removal of the scar tissue-like material in the hand,” Dr. Yezerski says. “During the past five or six years, the Xiaflex injectable enzyme has proven to break up the contracture and make the hand more flexible, decreasing the number of patients requiring surgery.”

ALSO KNOWN AS PHYSICIAN extenders, mid-level providers are licensed health professionals whose clinical care duties are more limited than those of physicians, but

who still play crucial roles in medical practices.At Murray Orthopaedics, Certified Physician

Assistants Diane Miller and Jennifer Walker and Nurse Practitioner Sabrina Karraker help

reduce the burden on orthopaedic surgeons’ time, allowing the physicians to concentrate on procedures and patient consultations.

“We coordinate with the physicians to provide pre- and postoperative education to patients,” Miller says. “Patients should feel free to ask us questions because talking with us, as physician extenders, is much like talking with the surgeons.”

Although educating patients about what to expect from surgery and rehabilitation is one of their primary

tasks, Miller, Walker, and Karraker also perform plenty of hands-on clinical work, including assisting in surgery.

“We’re the first assistants during procedures, so we hold instruments, close incisions—whatever the surgeons need us to do,” Miller says. “Even holding a patient’s arm in a certain position so the surgeon can fix a fracture is part of our job.”

The mid-level providers of Murray Orthopaedics also fit patients for casts, care for wounds, accompany surgeons on hospital rounds, and organize discharge plans.

“Whatever patients need, we’re here for them,” Miller says. “That’s the most important thing they need to know about us.”

IN OCTOBER OF 2013, the right shoulder that had helped Justin through many years of competitive sports began to hurt.

“I’ve been an athlete for almost my entire life,” Justin says. “I was a pitcher and catcher in baseball, played quarterback for 11 years through elementary and high school, and today I’m an outdoorsman. I tried to ignore the ache, but it never really went away and eventually began to get worse.”

When he began having trouble at his job as a lineman with Murray Electric System, Justin called Dr. Stephen Compton, orthopaedic surgeon at Murray Orthopaedics. The trustworthy orthopaedic specialist at Murray-Calloway County Hospital was an old friend.

“I grew up with Dr. Compton—he’s ‘Stephen’ to me,” Justin says. “We played basketball and football together and spent time hanging out as kids. I knew after talking to him about my shoulder that he would take care of me—I could have gone to anyone else, but I chose to see him.”

An X-ray showed that Justin had an injury at his acromioclavicular (AC) joint, where the collarbone (clavicle) meets the shoulder blade.

Wear and tear from years of playing sports had worn away the cushioning cartilage in the joint, allowing the bones to rub together.

“Stephen explained all my options, including short-term solutions and surgery,” Justin says. “We decided I would have steroid injections to control the pain rather than undergo surgery.”

LASTING REPAIR After a few months of steroid shots, however, Justin’s discomfort

could no longer be controlled. “The shots would wear off, and I would be in pain,” Justin says.

“Stephen told me that after the surgery, I would feel as good or better than I did right after I got the shot. I said, ‘If you can guarantee that’s what will happen, then the surgery is the best option.’”

Justin had clavicle resection surgery at the end of March 2014. During the procedure, a small piece at the end of the clavicle is removed. Over time, the area fills with scar tissue that pads the joint. Justin’s procedure was performed arthroscopically, using multiple small incisions rather than one large cut.

Getting Over Overuse A cut, a bruise, or a broken bone are all examples of acute injuries that usually occur in an instant. An overuse (or repetitive stress) injury, on the other hand, develops over many years. When too much stress is put on the joints or bones without giving the body time to heal, an overuse injury can occur.

Over-the-counter medication, rest, steroid injections, and surgery are all used to treat repetitive stress injuries. In addition, overuse injuries can be prevented through:

• Proper technique. Work with a coach or trainer while exercising or learning a new sport—proper training goes a long way toward keeping your body healthy over time.

• Crosstraining. Make sure your workout routines incorporate all major muscle groups, not just the ones used most in your preferred sport.

• Warming up. Proper stretching before exercise keeps your joints, muscles, and tendons flexible and less likely to be injured.

• A slow approach. Putting in the work to train your body takes time. A good rule of thumb is to increase your training program or activity only 10 percent each week. This allows your body to adapt and rest.

• Paying attention to pain. Discomfort is the body’s way of letting you know something is wrong. Don’t ignore pain for a few more minutes of play—your body could pay the price down the road.

RETURN TO LIFEAfter taking all of April and half of May off work, Justin returned

to Murray Electric and is feeling better than ever. “By the time I went back to work, I was at 100 percent,” Justin says.

“The procedure went really well. Dr. Compton explained before the surgery what the positives and negatives would be, how much time I would need to take off work, and how I would feel afterward. He was right on the money.”

Justin was also able to return to hunting and fishing with his 5-year-old daughter and 2-year-old son.

“Justin works hard to be a good father, husband, and employee,” Dr. Compton says. “The pain in his AC joint was holding him back in all those areas. If you have pain, we can find a permanent fix to improve your quality of life and get you back to the things you love doing without those limitations.”

DR. SHASTRI RECEIVED his bachelor of medicine and bachelor of surgery degrees from Grant Medical College and Sir J.J. Group of Hospitals in Mumbai, India. Afterward, he completed a three-year internship and residency in internal medicine at Cooper University Medical Hospital. He is also board certified in internal medicine and pulmonology.

Dr. Shastri’s interests include treating complex diseases related to the lungs, such as asthma, emphysema, and pneumonia, as well as sepsis, septic shock, and interstitial lung diseases, and managing chronic obstructive pulmonary diseases.

“Pulmonology is an important service line at Murray Hospital, and Dr. Shastri is a good fit,” says Jerry Penner, CEO of Murray Hospital. “He brings a wealth of training that will help our region breathe easier.”

When he’s not providing care, Dr. Shastri can be found playing tennis or cricket and loves listening to music, as well as drawing and painting.

“My wife, Parul, and I recently moved here with our 2-year-old daughter,” Dr. Shastri says. “We are excited to be a part of the community, and I hope to help patients better manage symptoms related to respiratory conditions.”

Dr. Shastri’s office is located in Suite 307E of the Medical Arts building at Murray Hospital.

PURSUING A CAREER in medicine was an easy decision for Dr. O’Dell.“When I was in high school, I considered becoming a minister, but I also had a strong desire

to study science,” Dr. O’Dell says. “As a college junior, I decided that being a physician was the best way to combine my love of science and my desire to help others.”

Dr. O’Dell received both his undergraduate degree and medical degree from the University of South Carolina. He completed an internal medicine residency at the University of Kentucky Medical Center, where he was recognized as Intern of the Year 2011–2012 and was recently named Chief Resident. He currently serves as a captain in the Medical Corps of the United States Army National Guard. Outside of medicine, he enjoys gardening, cooking, and spending time with his family.

He’s also happy about his new role at Murray Medical Associates. “I’m eager to contribute to the health of the community—it’s a real gem, and I hope I can

continue to make it a great place to live, in my own way,” Dr. O’Dell says. “I’m looking forward to building relationships with patients and helping them however I can.”

DRS. CHARETTE AND MORGAN practice with Drs. John Yezerski and Stephen Compton at Murray Orthopaedics.

Dr. Charette has been practicing orthopaedic medicine here in Murray since 1980, making him one of the longest-tenured orthopaedic surgeons in the area. Initially trained in general orthopaedic surgery, Dr. Charette took a break from active practice in 1996 to pursue subspecialty training in joint replacement surgery at Indiana University’s Methodist Hospital in Indianapolis. Since then, he has performed thousands of joint replacements and revision surgeries right here at Murray-Calloway County Hospital.

“When I came to Murray Hospital 34 years ago, there was a feeling that you went to the big city for a big problem and the small-town hospital for small problems,” Dr. Charette says. “Now we have a team of highly specialized orthopaedic surgeons who can handle virtually any orthopaedic issue. People no longer have to see big-city doctors to get the best orthopaedic care.”

FAMILIAR FACES, NEW TECHNIQUESDr. Morgan has been practicing orthopaedic

surgery at Murray Hospital for six years. He completed fellowship training in adult knee and hip reconstruction at Stanford University in California. Dr. Morgan is well versed in a

number of advanced orthopaedic procedures and techniques, including the anterior approach to hip replacement surgery, a minimally invasive option that requires no cutting of muscle tissue around the hip and provides a significantly faster and less painful recovery. He also performs reverse total shoulder replacement, an advanced option for patients who wouldn’t normally qualify for total shoulder replacement due to a torn rotator cuff.

“The surgical technologies and techniques now available to us are more advanced. One of the most gratifying aspects of my work is being a part of the healing process,” Dr. Morgan says. “I aim to treat my patients as I would want to be treated. That means not only offering advanced services, but also doing so in a compassionate, caring way.”
