HealthFlex: A Comprehensive Approach to Healthy Living HealthFlex is much more than a health plan. It offers a comprehensive approach to a healthier lifestyle—with features that support disease prevention, physical vitality and emotional well-being.

The HealthFlex focus on prevention helps clergy, lay employees and their families prevent disease and improve their overall wellness. Early Detection and Disease Prevention

If you think health care benefits matter only when people get sick—think again. Preventing illness and keeping people healthy are primary goals of HealthFlex. Don’t wait to take advantage of HealthFlex’s depth of resources aimed at preventing illness and chronic health problems.

Step 1: Blueprint for Wellness—The Quest Diagnostics Blueprint for Wellness® screening is the first HealthFlex step toward a healthier you. This screening involves a single blood draw, followed by lab analysis for more than 20 health indicators, including blood glucose, cholesterol, hemoglobin, PSA (men only) and much more. Test results provide valuable information about heart disease risk, diabetes risk (or diabetes management for those who have diabetes), kidney health and liver health. The Blueprint for Wellness screening may be more comprehensive than your doctor’s standard blood tests—and is available to you at no out-of-pocket cost. PleaseshareyourBlueprintforWellnessresultswithyourdoctor.

You may be able to take the Blueprint for Wellness screening at an on-site event if offered by your plan sponsor—or you can go to a local Quest Diagnostics facility.

To register for a Blueprint for Wellness screening, call 866.908.9440 (employer/program: “United Methodist Church” or “HealthFlex”). You also can register through the HealthFlex/WebMD website. Go to www.gbophb.org, click on “HealthFlex/WebMD” and enter your username and password (or choose “New Users—Register Here” if this is your first time on the website). Then select “Blueprint for Wellness (Quest)” under “HealthFlex Vendor Links.” (Registration key for 2011: UMC2011. Have your medical insurance card handy for online registration.)

Step 2: HealthQuotient—The WebMD HealthQuotient (HQ) online health risk assessment is your next step toward preventing disease and managing health risks. This online tool is a fast and easy way to evaluate how personal health factors, lifestyle habits and family medical history may raise your risk for serious conditions like heart disease or diabetes. The HQ does not diagnose conditions—but it does provide information about how habits or family history may increase your likelihood of developing certain health conditions. It may also help you prioritize which risk factors deserve your attention first.

The HQ is easy and confidential—simply enter your information through a secure website. If you took the Blueprint for Wellness screening, some of your health data will be conveniently and securely uploaded into your HQ.

Your HQ answers are summarized to generate a report that includes an overall HQ score as well as an assessment of your potential risk for conditions

such as diabetes, heart disease, certain cancers or depression. HQ takes things a step further, with practical suggestions for improving your scores—and your wellness—in target areas.

To take the HQ, log in to the HealthFlex/WebMD website at www.gbophb.org and click on “HealthFlex/WebMD.” (If you cannot recall your username or password and cannot retrieve them by clicking on “Forgot your username or password,” contact WebMD’s customer service team at 866.302.5742, option 1.)

Step 2: HealthFlex Benefit: Wellness Exam—All HealthFlex medical plans include a provision for an annual wellness exam that includes an office visit plus all coordinating lab tests and screenings—at no out-of-pocket cost to you. Clinical studies suggest that receiving recommended screenings and identifying conditions early are critical to preventing chronic conditions or catching problems at the earliest, most treatable phase. A wellness exam is like a “tune-up” for the body—it’s preventive maintenance that can help you avoid bigger problems later.

To use your HealthFlex wellness exam benefit, ask your physician or physician’s office to code your exam under “wellness” or “preventive care.”

Step 2: Measure Your Risk

Your Privacy Is ValuedWe are committed to keeping your personal health information private. Your church, annual conference, employer, HealthFlex, the General Board of Pension and Health Benefits (General Board) and your insurance company cannot access your lab results, answers to health questionnaires or other personal information. HealthFlex partner vendors—such as WebMD and United Behavioral Health—are bound by federal HIPAA privacy laws.

Early Detection and Disease Prevention

WebMD Health Coaching for adopting a healthier lifestyle—When the HQ online risk assessment identifies certain risk factors, HealthFlex offers health coaching services from WebMD to help you reduce these risks before they progress into more serious health problems. Health coaching is provided by telephone at no cost to HealthFlex participants and covered spouses in active plans—and is completely confidential.

Health coaching can address common risk factors, including:• high body mass index (BMI);• high blood sugar;• inactivity;• high blood pressure;• high cholesterol;• unbalanced nutrition habits;• emotional strain, such as depression or stress; and• smoking.

Health coaches are trained health care professionals—including dietitians, exercise physiologists and psychologists—who provide useful tips and feedback to help you develop a plan that works for you.

Coaching is always confidential—your participation in the program and any information you share with a health coach will not be disclosed to your employer, conference, insurance carrier, HealthFlex or the General Board.

To learn more about WebMD health coaching services or to start working with a health coach, call 866.302.5742, option 2. You must complete the HQ online risk assessment to be eligible for coaching.

Optimal Health condition management for living with a chronic condition—When risk factors progress to a chronic health condition, taking care of all the details necessary to manage that condition may feel like a second job. Remembering medications and appointments, changing habits and living with symptoms can become overwhelming. The Optimal Health service can help you manage an existing health condition to prevent complications. Optimal Health’s registered nurses can help you understand your doctor’s recommendations, follow them properly and learn ways to control symptoms. They will share evidence-based recommendations to best manage your condition. These services are available at no cost to participants and covered spouses in HealthFlex active plans.

Optimal Health condition support services can help with the complexity of managing chronic conditions such as:• diabetes,• heart disease,• chronic obstructive pulmonary disease (COPD),

Consultation is offered by telephone and is confidential—information shared with an Optimal Health nurse will not be disclosed to your employer, conference, insurance carrier, HealthFlex or the General Board.

on nutrition, fitness, smoking cessation and specific medical conditions;• connect seamlessly to Blue Cross and Blue Shield of Illinois,

UnitedHealthcare and Medco websites without entering additional usernames or passwords to access personalized information about your health coverage and claims;

• connect to Web pages for HealthFlex partners including Ceridian, Cigna, Vision Service Plan (VSP), United Behavioral Health (UBH), Weight Watchers®, Virgin HealthMiles and Quest Diagnostics;

• research a medical procedure or test; and• sign up for electronic health newsletters.

Optum NurseLine—Any time, any day, you can ask a registered nurse about symptoms, self-care and prevention for many common health conditions. If you are trying to decide whether to call your doctor, go to the emergency room or the best way to handle your symptoms, contact the Optum NurseLine for knowledgeable advice. The Optum NurseLine is a helpful resource after hours when your doctor is not available.

And being active can help you maintain a healthy weight, which lowers the risk of common conditions such as heart disease, diabetes and arthritis. Weight Watchers®—Maintaining a healthy weight is good “medicine”—it lowers the risk of heart disease, stroke, diabetes, high blood pressure, arthritis and some types of cancer. If your weight isn’t ideal, losing just 5% to 15% of your body weight could pay off with lower health risks and more energy to enjoy your work, relationships and activities.

Yet if losing weight was easy, obesity wouldn’t be a national concern. For many, a structured program and a supportive group can put successful weight control within reach. That’s why HealthFlex offers a discount of 50% off national prices for Weight Watchers—to help people reach their goals for weight control. Weight Watchers meetings can be found in your community or even in your church or workplace if enough individuals are interested. HealthFlex also provides a 50% discount for online and at-home Weight Watchers programs.

HealthFlex Benefit: Registered Dietitian consultation—Making healthy food choices is helpful and healthful for individuals with chronic conditions, special dietary needs and those who simply want to improve their health. HealthFlex benefits include visits with a registered dietitian for a low co-payment or co-insurance. Registered dietitians provide professional and individualized guidance, including:• information about recommended caloric intake and nutrient balance;• sensible approaches to weight management; and• education about food groups, special dietary needs and adhering to

Busy schedules, daily demands and the energy required to serve others can weigh heavily on anyone’s shoulders. When stress and emotional strain interfere with work, family life, social interactions or your ministry, take advantage of the stress management resources available through HealthFlex. All services are strictly confidential; your participation in these services is not shared with your conference or employer.

HealthFlex participants in active plans have access to many of these services at no cost. Choose the method that feels most comfortable— in person, on the phone or online. A broad range of services aimed at improving life’s balance and peace of mind are provided by United Behavioral Health (UBH), a HealthFlex vendor partner. All services are completely confidential.

Remember: Asking for help is an act of courage, not weakness.

Employee Assistance Program (EAP)—The EAP helps participants resolve personal concerns related to work, relationships and everyday situations, as well as major life changes and personal issues, such as depression, substance abuse, grief or living with illness. The EAP can help participants cope with the stress and uncertainty of common challenges, such as:• welcoming a new baby, • caring for a sick or aging family member, • adjusting to relocation and purchasing a home, • setting boundaries between work and personal life, • going through a divorce, or • facing financial uncertainty.

Make the EAP your first call if you’re feeling overwhelmed, unmotivated, or out of touch and in need of friendly and confidential support. HealthFlex participants in active plans receive eight free in-person visits with a local counseling professional per concern. If more visits are needed, HealthFlex provides an easy transition to regular behavioral health benefits. You may choose to receive support in person or on the telephone.

To obtain EAP services, first call 800.788.5614 for an authorization number for the first eight visits at no out-of-pocket cost. Once you have the authorization number, call the provider directly to arrange your appointment. Be sure to tell the provider that your first eight visits are covered completely by HealthFlex with no out-of-pocket cost to you.

EAP WorkLife Services—Minimize common stressors before they lead to chronic stress or illness. WorkLife Services offers resources by phone or e-mail for:• managing financial concerns, including balancing the household budget

or paying off debt;• handling legal issues, such as wills and real estate transactions;• identifying services available in a new community after relocation; and• finding resources close to home for elder care, child care or even pet

care when you’re away.

WorkLife Services also provides one free in-person or phone consultation with a legal or financial professional and discounted rates on follow-up services. Additional WorkLife support for other needs is available by telephone and can guide you to appropriate in-person resources.

Live and Work Well website—Find an abundance of helpful information and resources at your fingertips through the Live and Work Well website, offered by UBH. Resources on this website are an excellent complement to the in-person and telephone-based EAP support services offered by UBH.

The Live and Work Well website can be helpful even when life is in balance. This site is a portal to the following resources:

• LifeStageCenters—Learn about living with chronic medical conditions, handling major life changes, coping with stress, parenting kids of all ages and caring for elderly family members.

• MentalHealthConditionCenters—Read about anxiety, depression, abuse and other behavioral health concerns.

• CommunicationTools—Find tips that can improve relationships with family, coworkers or parishioners.

Access online behavioral health resources through the HealthFlex/WebMD website by clicking “EAP–Live and Work Well” under “HealthFlex Vendor Links.” If you go directly to the website (www.liveandworkwell.com) instead of through the HealthFlex/WebMD website, you will need to enter an access code (“GBOP”).

If you’re a pastor, call the Pastoral Consultation Line to speak with an ordained clergyperson who also is an experienced counselor—someone who understands the stresses that come with serving others. The Pastoral Consultation Line offers a confidential platform to help clergy:• balance multiple demands for time and attention within ministry;• address difficult situations in the congregation; • help parishioners facing personal challenges; and• identify appropriate resources for parishioners struggling with marital
