INTRODUCTION

This Division has been newly created in the year 2015 restructuring of Water Resources

Department. This office is temporarily functioning with hiring of private building at

Baramasi Danda road, College square, Nayagarh. This office has recently established

Quality Control testing Laboratory. Sanction has been accorded of functioning of office

the Executive engineer, Quality Control Division, Nayagarh vide letter no.5824 dated

15.03.2017 of Joint Secretary, Department of Water Resources.

Jurisdiction of this Division

Jurisdiction of this division is as follows-

1. Nayagarh Irrigation Division, Nayagarh

2. Nayagarh Minor Irrigation Division, Nayagarh

3. Kandhamal Minor Irrigation Division, Phulbani

4. Boudh Minor Irrigation Division, Boudh

ORGANISATION SET UP

Following two no’s AEE’s have joined to this division on dated 20.06.2016

1. Biswadeep Singh

2. Markandeya Dash

However one no.of AE (Civil) has been joined in this division on 01.08.2015 and

another one no.of AE has been joined in this division on 03.04.2017 and 5 no’s peon are

working in this division. Several correspondences have made with higher authorities for

posting of minimum no’s of ministerial staffs and technical staffs for smooth functioning

of this Division.

The work distribution of AEE(C) have been made as follows

Inspection Vehicle

Two no’s Inspection vehicle has been provided to this division on hiring basis for

inspection of developmental work of above four no’s Divisions. One vehicle for Executive

Engineer and other is for two no’s A.E.E.s for inspection of site work.

The undersigned has been requested to the Director, Reasearch and Quality

Control, Bhubaneswar to provide one more private vehicle on hiring basis.

FIELD VISIT

Inspection Report of Executive Engineer, Quality Control Division, Nayagarh Date 11-01-2018 Inspection the different work site along with Sri M.Dash, AEE and Sri S.C.panda, AE of QCD, Nayagarh and Concerned SDO and AE of Boudh M.I. Division. 1 .Name of the Work:- Construction of gandini nallah checkdam no.III near village champamal in Harabhanga Block in boudh district. Observation and Suggestion

- Upstream cutoff wall has been completed. - Down stream cutoff wall is under progress. - Sand and coarse aggregate available at site are good quality. - Cement used at site ACC Brand. - Q.C. register maintained by field engineers. - Advised to do needful minimum Q.C. testing equipment kept available at site for

instant testing. 2 . Name of the Work:- Construction of canal lining from RD 3284m to 4380m RMC, RD 00m to RD 240m in RMC,RD 100m to RD 500m in rambhikata-II, RD 100m to RD700m in Rambhikata-I of of loigaon on MIP under CLSRP grant 2017-18. in Boudh district. Vide agt no.-94p1/17 Observation and Suggestion

- RD 4780m to RD 4880m C.C. lining has been completed. - Bed concrete work RD 4880m to RD 4960m is under progress. - Slump test conducted at site result come to be 45mm. - Cement used at site KONARK Brand. - Sand and aggregate available at site are good quality. - Q.C. register maintained by field engineers. - Quality Control test report are not received to till date. - Advised Curing should be done properly. - OK card is not maintained at site.

3. Name of the Work:- Construction of canal lining from RD 1780m to 1820m, RD 3110m to RD 3284m of main canal of loigaon MIP in right main canal

under CLSRP grant 2017-18. in Boudh district. Observation and Suggestion

- Canal lining from RD 1080m to 1280m bed concrete work has been completed and

lining wall is under progress.

- Quality Control test result yet to be received. - Old centring and shuttering plates used. - Q.C. testing equipment not at site - Old centring and shuttering plates to be replaced.

- Q.C. register maintained by field engineers. - Ok card is not maintained. - Advised curing should be done properly.

Dt.30.01.18 Inspection the different work site along with Sri N.N.Nayak, Director, Research and Quality Control Division, BBSR, Sri M.Dash, AEE and Sri S.C.Panda, AE of QCD Nayagarh and concern SDO, AE of Nayagarh Irrigation Division ,Nayagarh

1. Name of the Work:- Improvement of rough destroy along with renovation of 5nos canal siphon and c.c. lining of kuanria irrigation project for 2017-18. RD 900m to 17445m working RD 2600m to 2800m and RD 3200m to 5130m vide agt.no.-34p1/17-18.

Observation and Suggestion - At the time of inspection no work was going on at site. - C.C. lining has been completed in all respect from RD 2600m to 2658m of rough

destroy of kuanria project. - Quality Control test of cement, sample for gradation of CA and FA have been reported

with EE, QCD, Nayagarh.

2. Name of the Work:- Improvement to Kuanria left near village Luni of Dasapalla block RD 4750m to Rd 4804m vide agt.no-29p1/17-18.

Observation and Suggestion - At the time of inspection no work was going on at site from 4750m to 4804m of river

kuanria except collecting of stone boulder of size 30cm and above as per specification of agreement.

- A good no. of small stones have been found which needs to be removed prior to completion of this work. Stones having Skin needs to be removed immediately.

- Voids are to be filled with small stones vertically. - The test report of stone has been reported with EE, QCD Nayagarh for testing of its

properties. 3. Name of the Work:- Construction of office building of newly created nayagarh irrigation division vide agt.no.-133p1/16-17. Observation and Suggestion

- At the time of inspection , putty filling work over walls of office building under construction was under progress with NEROLAC WALL PUTTY.

- 1st layer putty filling is to be achieved as per smoothness and fineness required prior to painting job.

- The interstices of C.P done is to be filled up with putty with proper care and supervision for achieving good finish of painting.

- Verified the test report of Design mix , steel bar , flyash brick , cc cubes etc. and found

satisfactory.

Inspection the different work site along with Sri N. N. Nayak, Director Research and Quality Control Division, BBSR, Sri M.Dash, AEE, QCD, Nayagarh, Sri S. C. Panda, AE and concerned SDO, AE of Drainage sub Division, Nayagarh Name of the Work:- Construction of checkdam at RD 3900m pathargada (Chingudipada) vide agt.no.-30p1/16-17. Observation and Suggestion

- The work has been completed in all respect .

- There is slide of stone pitching work in the D/S of check dam which needs restoration.

- Verified the Q.C. test reports and found satisfactory. - The damaged ground posts 3nos needs to be repaired immediately. - The jungle is to be cleared in the U/S and D/S of check dam. - Cement of Brand has been reported for the work and design mix report has been

obtained from a private lab and found satisfactory. -

2) Name of the Work:- Construction of check dam at RD 2750m pathargada (Chingudipada) vide agt.no.-31p1/16-17. Observation and Suggestion

- The work has been completed in all respect . - There is slide of stone pitching work in the D/S of check dam which needs

restoration. - Verified the Q.C. test reports and found satisfactory. - Design mix of cc M25 A20 with ACC brand of cement has been tested in a private

lab and found satisfactory. Dt. 02-02-2018 Inspection the different work site along with Sri N.N.Nayak, Director Research and Quality Control Division, BBSR, Sri M.Dash, AEE and Sri S.C.Panda, AE of QCD Nayagarh and concerned EE, SDO and AE of Boudh MI Division, Boudh.

1) Name of the Work:- Construction of canal lining of sagaranallah MIP in Harabhanga block of boudh district vide agt.No.-92p1/17-18.

Observation and Suggestion

- At the time of inspection C.C. lining of bed of canal with M15 A20 with KONARK (PPC) Brand of cement from RD 1100m to 1150 m was under progress.

- The cantering and shuttering for construction of protection wall is to be made rigid to avoid bulging of concrete.

- Conducted gradation test of CA and FA at site. 20mm and downgraded chips contains

higher size materials have advised for mixing of 12% of 10mm and downgraded chips to

meet the required specification as per agreement.

- The sand contains silt content of 2.08 and bulkage of sand is found to be 17.64% and

belongs to zone-III.

- Plate vibration is to be used for proper compaction of C.C. henceforth for lining work. - 6 nos. of cc cubes have been casted at site for determination of compressive strength

at 7th and 28th days. - Instruction of OK card system is to be made for CC work under execution, henceforth.

2) Name of the Work:- Construction of canal lining from RD 500m to 2200m in madhusudanpur, RD 00m to RD 1000m at harekrushnapur,RD 00m to 1200m in Rambhikata-II minor canal of loigaon MIP under CLSRP grant 2017-18.in Boudh District.

Observation and Suggestion - At the the time of inspection ,CC with M15 A20 with Ultratech Brand of cement from

RD 1250m to 1350mt. of Madhusudanpur Minor was under progress. - Two nos. of concrete mixture and one no of plate vibrator have been deployed by

the agency. - Verified the Q.C register and found satisfactory.

- Design mix and sample for gradation of CA and FA have been deposited with EE, QCD Berhampur under S.M.I.C, Berhampur .

- Advised for immediate introduction of OK card system for cc work, henceforth.
