Background: • January 2019 launched the Health Equity Organizational Assessment (HEOA) – developed by the CMS PfP Health Equity Affinity Group―Purpose: Assess the current state around seven (7)
categories identified through national research as critical components of an organization’s ability to identify and address disparities
(not a required survey under HIIN, but all hospitals were strongly encouraged to complete the survey).

METHODS• 2017-2018 focus on care of patient in ED & critical care

• Best Practice Alerts (BPAs) in EMR revised and simplified for providers; Communication out to all providers early in the year
• Education provided to all ED providers, hospitalists, and appropriate nursing staff
• Began monitoring and reporting at all ED department meetings and posting in the ED staff rooms
• Providers and staff coached when sepsis BPAs not acknowledged and/or acted upon
BACKGROUND
OSF Healthcare St. Francis Hospital, EscanabaImproving Sepsis Care and Outcomes
Sepsis is the #1 cause of inpatient deaths in U.S. and accounts for billions in healthcare costs.
OSF St Francis Performance 2014-17 (average):
-Sepsis mortality rate: 4.65
-1st lactic acid in 3 hrs: 70%
-Antibiotic in 3 hrs: 72%
-Blood Cultures before Antibiotics: 81%
OBJECTIVE • Promote safe, quality care for patients diagnosed with
severe sepsis and septic shock
• Decrease mortality and LOS
• Increase sepsis bundle compliance
• Reduce sepsis readmissions
RESULTSImprovement experienced in all performance areas!
CONCLUSION• Improvement in sepsis care continues to be a major
priority for OSF St. Francis Hospital.
• Care management post-discharge now area of focus.
• New Complex Care Manager position created to work with high risk populations including post-sepsis
• Continue to monitor and facilitate improvement in all metrics
