• Hypoxia

• Metabolic imbalances such as hypoglycaemia

• Falls and trauma to head

• Unresponsiveness

• Neurological disease processes Eg. stroke, brain tumor,& epilepsy

• Post-anaesthesia

• New admission to form a baseline assessment.

• Severe, with GCS ≤ 8

• Moderate, GCS 9 - 12

• Minor, GCS ≥ 13.
