1.1 Structure Description and Inspection Summary This structure is a five barreled, corrugated metal pipe culvert with flashboard control. This structure has five corrugated metal pipes that have had 5-ft long extensions added to the downstream ends. The end sections are corrugated metal arches, not full pipes. The headwall is a grout bag wall with grout bags between the barrel extensions. The structure is located at the outlet of Lake Jackson in the Upper Kissimmee River Basin and was constructed in June 1994. The purpose of the structure is to control the water level in Lake Jackson. Water level is maintained to not exceed 56.0 ft. Structure Technical Details:Type: Corrugated Aluminum Bituminous Coating pipe with upstream control Number of Barrels: 5 Size of Barrels: 7 ft Length of Barrels: 46.5 ft Flow Line: 45 ft Bypass Water Level: 59 ft Control: Discharge is controlled by manually operated flashboards Board Size: 3" x 6" x 108" Inspection Summary: This structure was inspected by the surface team and the underwater team on April 27, 2010 with findings generalized in Table 1.1.

Deficiencies The general condition of Structure G111 was rated C-3 with the following C-3 and C-4 rated deficiencies: C-3:
CIVIL: Vehicle gate hardware needs replacing. Upstream and downstream riprap needs replacement. Guardrail timber spacing needs replacement, some missing. Fencing on access bridge needs repair. Need signs: No Boating, Gates Open Without Warning/No Swimming, No Parking, Structure Identification. Level and gate position indicators missing.
MECHANICAL: Drill operated stems should have drill stand provided for operation, areas of corrosion and debris build-up in stem threads, areas of corrosion on fasteners and operator input shaft, Gate 6 missing 1 of 3 cover fasteners, Gate 6, 1 of 4 anchor bolts loose. No lock washers or double nuts on any anchor bolts. Dissimilar metal reaction occurring between carbon steel operator housing and aluminum machinery frames.

2.1 Previous Inspections
The structure was last inspected on March 13, 2007 by Malcom Pirnie, Inc. During the previous inspection, the structure was reported to be in overall C-2 - C-3 condition, as follows:

An approximately a 4’ x 6’ x 1’ void currently exists, however after reshaping of the barrel, it is expected that a smaller area will need to be filled.

3.1 Current Inspection Complete inspection findings, by structure element, are presented on the checklist in Appendix A. Each finding is assigned a rating ranging from C-1 to C-5 (Table 3.1).

3.2 Project Organization
Structure inspections were performed by separate inspection teams accompanied by a District Field Station staff member. The underwater inspection team performed inspections separately from the surface team so that the structure could be locked out/tagged out. The above-water inspection team was led by HDR Engineering, Inc. The underwater team was led by Infrastructure Engineers, Inc. Key project personnel, their organization, and their responsibilities are listed in Table 3.2.

downstream arch end section has failed (5’), due tosettlement and separation from downstream end of CMP. There is settlement and shifting up to 3”. This is due to the erosion of grout bags supporting end arch section.
2. Foundation of grout bags has settled. These issues are due to the erosion and settling over time.
These are C-4 items since these repairs are capital repairs and are needed for the proper operation of the structure.
1. Dewater structure using coffer dam (Section 02401 of SFWMD Specifications) and bypass via the adjacent spillway.
2. Barrel end arch section only needs repositioning and stabilization with grout bags; assume 10 to 20 grout bags. Note, more may be needed.

CV3 Vehicle gate at the structure has damaged and corroded hardware.
Remove and dispose of damaged and corroded hinges (2) and install (2) heavy duty galvanized gate hinges; per SFWMD Detail Sheet C2. There are upstream gates used for vehicle control, this gate is seldom used and is in the open position under normal operating conditions.
CV4 Upstream slope requires more rip-rap, erosion protection. The area affected is approximately 100 ft long by 15 ft along the bank slope.
Remove the existing remaining rip rap. Install geotextile fabric and rip rap for 100 ft by 15 ft area beginning at the south side of the timber catwalk and extending south along bank, in accordance with Section 02370 of SFWMD Specifications and Detail C10 of SFWMD Standard Details.

CV5 Downstream slope requires more rip-rap, (some washout visible). The area affected is approximately 125 ft long by 30 ft along the slope, beginning at the west tip of the downstream berm; as shown in photo.
Insert coffer dam downstream of berm and dewater area, per Section 02401 of Specifications. Replace dirt that has washed out in the 125 ft by 30 ft area along whole slope (native soil), approximately 1,000 cu. yds, and compact soil in accordance with Section 02200 of SFWMD Specifications. Install geotextile fabric and rip rap for 125 ft by 30 ft area in accordance with Section 02370 of SFWMD Specifications and Detail C10 of SFWMD Standard Details.
CV9 Missing timber spacer on downstream guardrail (west rail, 9th spacer counting from the south). Only 1 bolt per spacer, resulting in spacer rotation (all guardrail).
Replace missing timber offset on downstream guardrail (West rail, 9th spacer counting from the south) with Southern Pine Lumber (no. 1 grade) timber block offset, per FDOT Specification Section 536. Replace missing bolts on all spacers with galvanized steel bolts (ASTM A153) as noted in Section 02451 of SFWMD Specifications.
CV10 1. Far downstream timber fence post is tilting and has sagging barbed wire.
2. Chain link fence not attached to posts at upstream south corner next to platform. Fencing does not prevent access or falling behind structure.
1. Reset tilting timber fence corner stabilize with concrete footing (per C4 of SFWMD Detail). Remove hanging barb wire, site is used as bridge and barb wire is not needed.
2. Reattach existing chain link fence with U-shaped galvanized steel wire to provide tension of fence fabric, per Section 02821 of SFWMD Specifications.
CV100 Missing sign. (“No Boating”) Provide sign as shown in Detail C5 of SFWMD Standard Details. Sign to be aluminum (0.125”) with timber sign posts and stainless steel mounting hardware. Lettering to be as shown on Detail C5 (6” Tall).
CV102 Gates Open Without Warning/No Swimming sign is missing.
Provide sign as shown in Detail C5 of SFWMD Standard Details; with the modification that the message read “Gates Open Without Warning”. Sign to be aluminum (0.125”) with timber sign posts and stainless steel mounting hardware. Lettering to be as shown on Detail C5 (6” Tall).
CV103 No parking sign is missing. Provide sign, as noted in Section 02821 of Specifications [Non-metallic, 0.125” thick white polyethylene warning signs mounted to wooden post already existing onsite]. Standard No Parking Sign, size: 12x18 inch.

CV105 No structure identification. Install structure identification sign using SFWMD standard ID post sign or galvanized U-channel post 6' long with a 6"x4" 0.04 gauge metal plate with a white background (Engineer grade reflective material) and black lettering.
CV150 Water Level indicator on structure is missing. Furnish and install water level indicator on structure; similar to Staff Gauge per Section 02781 of SFWMD specifications. Porcelain enamel iron gauge, strips graduated in tenths of a foot. Attach to structure with stainless steel fastenings.
CV152 Gate position indicators are missing. Replace ten stem covers with clear plastic stem covers that have closed, open, 1/4, 1/2 and 3/4 position indicator decals.
ME2 Drill operated stems; Drills do not have drill stands provided for operation.
Operating drills should be provided with support stands and torque limiting couplings for the safety of the operating staff.
ME6 Areas of corrosion and debris buildup in stem threads, areas of surface corrosion on fasteners and operator input shaft (approx 50% area), gate 6 missing 1 of 3 cover fasteners, gate 6, 1 of 4 anchor bolts loose. No lock washers or double nuts on any anchor bolts. Dissimilar metal reaction occurring between carbon steel operator housing and aluminum machinery frames.
Clean non-bearing machinery surfaces according to the requirements of SSPC SP2 and paint using methods described in Section 09900 of SFWMD specifications (2010). Replace missing cover fastener with self tapping galvanized hex head screw, and tighten anchor bolt on Gate 6. Install galvanized lock washers on all mounting fasteners. Install isolating neoprene rubber 70A durometer hardness gaskets between the steel operating housings and the aluminum machinery supports.
ME50 Gate Operation should be performed with a torque power tool on a stand. Debris in stem threads causes gate operation to slow.
Stem rods should be kept clean of debris and lubricated on a frequent basis.
US58 Vegetation debris keeping Gate 4 from fully seating.
Open gate 4. SFWMD dive team to clean out debris at Gate 4.
