UNIVERSITY HEIGHTS BRANCH – REQUEST FOR PROPOSALS
FURNITURE AND LIBRARY SHELVING The Heights Library will receive sealed proposals for FURNITURE AND LIBRARY SHELVING at the University Heights Branch located at 13866 Cedar Road, University Heights, Ohio 44118. Proposals will be received in the offices of the Heights Library, 2345 Lee Road, Cleveland Heights, Ohio, 44118 until 2:00 PM (local time), May 8, 2017. Library representatives will review the proposals at that time. Proposals received after 2:00 PM may be returned unopened. Proposals must be marked to the attention of Ms. Nancy Levin, and must be clearly marked “UNIVERSITY HEIGHTS FURNITURE AND LIBRARY SHELVING PROPOSAL ENCLOSED”. Heights Library will not be responsible for any misplaced proposals. Companies may submit a proposal on one or multiple items. The library encourages companies to submit proposals on any items which they believe are equivalent to the items indicated as the basis-of design. Firms wishing to inspect the shelving in storage at Corrigan Moving Systems. 2931 Abbe Rd., Sheffield Village, Ohio will be given an opportunity to view the shelves during the week of May 1st. If you wish to view the shelves please contact Facilities Manager Tim Pasbrig, 216-932-3600, ext. 1270 and a time will be arranged. Proposals must be submitted on the proposal form included in the Documents. Proposals shall also include delivery and installation costs. The library is a tax exempt organization. Proposals shall include photographs, drawings, specifications and other information for each item submitted so that the library staff can evaluate whether the item submitted within the proposal is equivalent to the item listed as the basis-of-design. The library will be the sole arbiter to determine if an item submitted is equivalent to the item listed as the basis-of-design. Quotes will only be considered if the price of the items have been previously bid; i.e. State or Federal pricing, U.S. Communities, Sourcing Alliance, Ohio Schools Council, AEPA, OCEPC, etc.

004113.2 PREPARATION OF QUOTE FORMS
GENERAL
A. Proposals shall be submitted on Quote Forms included within this document. Proposer may submit a Proposal on one or both bid packages. Quote Forms shall be filled out completely, signed and dated. Failure to fill out the Quote Forms as required shall be cause for rejection.
B. All blank spaces on the Quote Forms must be filled in, including Addenda, if any issued, in order for the Proposal to be valid. Items not being quoted by a Proposer should be entered as “No Quote”. Owner reserves the right to accept or reject any Proposal with “No Quote” Items.
C. Library Shelving Alternate Quotes: Spaces for Alternate Bids should be filled in with the amount increased or decreased to the Base Quote.
D. Bidders shall submit along with their Quote Forms the Quote Summary Form. Only items being submitted by the Proposer need to be filled out, including Costs.
E. All bid amounts shall be given in both words and numerals. In the event of a discrepancy between words and numerals, the words shall govern.
F. Unit Prices: Space has been provided on the Quote Form Specification pages for the entry of unit prices for one unit of each item. Unit prices shall include the cost of all materials, freight, storage, delivery, installation and overhead and profit. The unit price will be applied should the Owner decide to order more or fewer units than are listed.
G. It is understood and agreed that work involved as indicated in the specifications of both Furniture and Library Shelving Proposal Packages, Alternates or Voluntary Substitutions accepted by the Library shall be completed in every particular and detail, and all work related to or involved with all items shall be executed in

accordance with the general description and inferable results indicated, whether such related or involved work is specifically mentioned within the description of the Item Number or not.
H. All Proposers are required to read the specifications and descriptions of the Base Quote Items, and to ascertain the extent of work that may be involved. Proposers for Library Shelving are required to also read the specifications and descriptions of the Library Shelving Alternates, and to ascertain the extent of work that may be involved.
I. Optional Recommendations: Provide additional recommendations for any of the product types: furniture or library shelving listed in the Proposal specifications. Recommendations will be assessed on ease of transaction (such as purchasing through STS or buying group) with associated cost savings, and improving quality such as additional accessories. Optional Recommendations shall be listed on the Optional Recommendations Form provided. Recommendations shall not be included in the Base or Alternate Quote Forms.
DEFINITIONS
A. “Furnish” means supply and deliver to the Project site, ready for unloading, unpacking, assembly, installation, and similar operations.
B. “Install” describes operations at the Project site including the actual unloading, unpacking, assembly, erection, placing, anchoring, applying, working to dimension, finishing, curing, protecting, cleaning, and similar operations.
C. “Provide” means to furnish and install, complete and ready for the intended use.
SUMMARY OF WORK
A. Provide furnishings as described and scheduled in the “Quote Forms”, “Furniture Specifications Proposal Package A” and “Library Shelving Specifications Proposal Package B”.
LIBRARY SHELVING BID PACKAGE B: SPECIAL REQUIREMENTS
A. The Library Shelving Proposal Package B consists of new library shelving; and existing shelving now in storage which shall be repainted, cut down to size where indicated in the specifications, transported and installed where indicated on the drawings. This package also includes new plastic laminate tops, tackable surfaces, end panels and other noted work where indicated in the specifications.
B. Existing library shelving now in storage shall be inventoried by the successful Library Shelving Vendor for both quantities and condition of shelving. Where shelving is to be found different in quantity from Quote Specifications; or shelving found to be damaged or with parts missing, the Vendor shall notify the Owner’s Representative and the Architect. The Owner’s Representative, Architect and Vendor shall determine the best method/means to achieve shelving count required. Should additional new shelving be required the Vendor shall submit a Proposal for the new additional shelving to the Owner.

C. Base Quote for Proposal Package B includes new shelving and existing shelving and all associated materials and work as indicated in the specifications.
D. Alternates for Proposal Package B include: 1. Alternate #1: Replacing Base Quote existing shelving items with new as
indicated in the specifications. Note that sizes of new shelving, end panels or other items may change from the Base Quote items.
2. Alternate #2: Replacing additional existing shelving items with new as indicated in the specifications. Note that sizes of new shelving, end panels or other items may change from Base Quote items.
3. It is the responsibility of the Proposer to read the specifications and descriptions of the Alternates and ascertain the extent of the work.
COST INCREASES
A. Any increases in the cost of materials, labor, tools, equipment, rentals, transportation or other items, which occur subsequent to the receipt of proposals, shall be borne by the successful Vendor whether such increases occur before or after award of contract.
ALTERNATES
A. Existing Library Shelving Alternates: Where alternate(s) pertaining to repainting are accepted by the Owner, the successful Vendor shall work with Owner on scheduling a pick‐up date at the storage facility where the existing shelving is located. Work to include electrostatic painting (quality outside source acceptable), assembly, installation, and similar operations. Transportation of Shelving: See Library Shelving Specification, Page 5, “Existing Shelving”.
B. Alternate prices shall include all costs, overhead and profit.
REQUEST FOR VOLUNTARY SUBSTITUTION
A. Requests for voluntary substitutions included in the Proposal must be submitted on the Voluntary Substitution Sheet which is a part of the Quote Form. Voluntary substitutions not on the Voluntary Substitution sheet shall be rejected. Proposer shall submit supporting data consisting of drawings, product data, test results, finish/fabric samples and other information necessary for evaluation. Owner reserves the right to request actual samples for review, especially where it pertains to quality, comfort, and use. Quality, design, sizes, and materials shall match base proposal items to the fullest extent possible. Owner reserves the right to reject any substitution not in their best interest.

SECTION 01 00 01 – PROJECT DESCRIPTION
010001.1 Heights Libraries is currently renovating and expanding the University Heights Branch located at 13866 Cedar Road, University Heights, Ohio. As a part of this Project, new furniture and library shelving are to be provided as defined in the proposal forms and Furniture Plans.
SECTION 01 00 02 – INSTRUCTIONS TO PROPOSERS
010002.1 PROPOSER REPRESENTATION
A. The Proposer, in submitting a Proposal/Quote Form represents that he/she has read and understood the Contract Documents.
B. The Proposer represents that prior to installation of products, he/she will visit and become familiar with project conditions under which the work is to be performed.
C. No allowance will be made to the successful Vendor by reason of any error or omission on his/her part, or due to the failure to comply with the representations of this Article.
D. Should Proposer, upon examination of Proposal Documents discover discrepancies, or questions of scope or intended quality, he/she shall immediately report in writing via email to Adam O’Brien, CBLH Design, at [email protected], no later than Monday, April 24, 2017. CBLH Design may respond by issuing an Addendum, if deemed necessary. No addenda will be issued later than Tuesday, May 2, 2017.
E. Neither Architect nor Owner shall be responsible for any oral interpretations. F. Proposer shall verify that he/she has received all Addenda issued, and shall
acknowledge receipt of Addenda in the Proposal. G. Investigation/References of Proposers: Owner shall retain the right to investigate or
require information from Proposers to determine the ability of the Proposer to fulfill proposal requirements before award of Contract(s).
H. Proposal Signature: Signing of the Quote Forms shall be construed as acceptance of all Work and noted provisions contained herein.
010002.2 FIELD VERIFICATION
A. Upon award of contract, and substantial completion of the interior environment, the successful Vendor shall field‐verify all related conditions and areas where furniture or shelving have been specified. Any conditions in question including windows, columns, hold dimensions, location of wall‐mounted items such as thermostats or other shall be brought to the attention of the Architect and Owner. 1. The successful library shelving Vendor shall submit shop drawings to the
Architect. Shop drawings shall include library shelving layout, end panel elevations and sections, and high pressure laminate tops, and shall identify any panels or other items limiting access to wall power/data outlets. Failure by the library shelving Vendor to submit shop drawings will result in liability on the Vendor for any discrepancies in the field.

2. The successful furniture Vendor shall submit shop drawings to the Architect. Shop drawings to include furniture layout, furniture interface with floor/wall power outlets and all technical drawings required showing complete electrical/data port type, location, interface, cords and power that is to be provided under Contract.
3. Electrical power/communication outlets for furniture with power requirements is supplied via floor and wall outlets. It is the responsibility of the successful Vendor to field‐verify the location of all outlets. Vendor shall notify Architect of any problems that may result in the compromise of the design. Architect will coordinate any required modifications with Owner and Construction Manager.
010002.3 NOTICE TO PROCEED
A. The successful Vendor for Furniture and/or Library Shelving will be issued a Purchase Order by the Owner to proceed with purchase and installation. The Successful Vendor(s) shall review Furniture and Library Shelving orders with Architect and Owner prior to placement of order.
SECTION 01 10 00 – SUMMARY OF THE WORK
011000.1 Refer to Furniture Plans and Furniture Specifications dated 04/06/2017.
011000.2 WORK INCLUDES:
A. Providing, unloading, verifying quantities, determining condition of, installation and assembly of the specified furniture and library shelving listed in the Quote Forms.
B. Cleaning and removal of all scope‐related packaging/debris from the site. C. Compliance with Owner’s requirements for submitting proposals.
011000.3 Work noted Not in Contract (NIC) will be furnished and installed by Owner and consists of the following:

011000.4 WORK SEQUENCE
A. Construction is scheduled to be substantially complete to enable the installation of furniture and library shelving on or about August 21, 2017.
B. Any updates to the construction schedule or additional information related to schedule coordination will be provided to the successful Vendor(s). Vendor shall coordinate the delivery and installation of furniture and library shelving with Owner’s Representative.
C. Vendor shall notify Owner’s Representative three (3) weeks prior to installations.

D. Any safety requirements that may be necessary if furniture and library shelving are delivered and installed prior to the completion of construction shall be coordinated with Construction Manager.
SECTION 01 31 00 – PROJECT COORDINATION
013100.1 Installation: Vendor shall coordinate Work related to other contractor activities through Owner’s Representative to ensure orderly and efficient installation of the Work.
013100.2 Delivery: Vendor shall coordinate locations for unloading, work, elevator usage, and parking with Owner’s Representative. It is the responsibility of the Vendor to provide appropriate wall and floor protection to ensure no damage occurs to previously installed Work by Others.
SECTION 01 31 01 – DELIVERIES, STORAGE, AND HANDLING
013101.1 DELIVERIES
A. Deliver new furniture and library shelving to the site in manufacturer’s original packing complete with labels and instructions for handling, storing, unpacking, protecting, and installing. Items that can be preassembled prior to delivery shall be packed and protected for transport and delivery along with instructions where provided by the manufacturer.
B. The successful Vendor of the library shelving package shall arrange with Owner’s Representative the delivery from its current off‐site storage location of any existing library shelving that is intended to be reused.
C. Inspect new and existing furniture and library shelving upon delivery to ensure items are undamaged and properly protected.
013101.2 STORAGE
A. There shall be no storage of goods at the library prior to installation. B. During installation, any storage required shall be arranged by the successful
Vendor(s) with the Owner’s Representative.
SECTION 01 31 02 – INSTALLATION
013102.1 EXAMINATION
A. Examine Substrate and conditions under which the Work is to be performed. B. Examine goods immediately upon delivery and again prior to installation. Reject
damaged or defective items. C. Do not proceed with the Work until unsatisfactory conditions have been corrected.

A. Provide and install all items as shown on the drawings and as specified. B. Comply with manufacturer’s installation instructions and recommendations. C. Provide connection devices, hardware, and accessories required for complete
installation. D. Components are to be Installed and locked securely into place at heights and
dimensions indicated. Where mounting heights are not indicated, consult Architect for direction.
013102.3 ADJUSTING
A. Remove and replace new components that are scratched, chipped, or otherwise defective. Provide new matching units, installed as specified and without evidence of replacement.
B. For reused existing library shelving: Items with obvious scratches, dents or other damage should be brought to the attention of the Owner’s Representative and Architect for direction on proceeding with the item(s). Where replacement with new is deemed necessary, Owner will request the Vendor to submit a quote in writing for the replacement of those item(s)matching type, size, color and finish.
C. Level and align as necessary to ensure level work surfaces, shelving, and other items. Properly align adjacent surfaces for a smooth transition.
013102.4 CLEANING
A. Remove packing material and debris from project site at the end of each working day.
B. Clean furniture and shelving of soil marks, dust, fingerprints, temporary labels/tags, and loose thread from fabric items.
C. Cover and protect any items that are subject to damage from staining, abrasions, scratches or other conditions that may adversely affect appearance or use until furniture and library shelving installation is complete.
SECTION 01 31 03 ‐ GENERAL SPECIFICATION NOTES

Refer to the enclosed Contract Documents for floor plans and furniture locations. On the floor plans all furniture and library shelving items are tagged with an item number that refers back to the itemized bid form. In the event of discrepancy, quantity on the Itemized bid form takes precedence over drawings.

Grommet and tabletop power/data outlet locations are noted in the bid form specifications. Provide wire management for cables/wires running horizontally on the underside of work surfaces and tables.

SECTION 01 77 00 – PROJECT CLOSEOUT
017700.1 Punch List Inspection: Request a punch list inspection upon completion of:
A. Submittal of warranties and maintenance manuals: Warranties shall include all manufacturers’ standard and special warranties for each type of furniture and library shelving noting life time, limitations, and exclusions. Maintenance manuals shall provide information on proper upkeep.
B. Cleaning instructions: Include cleaning instructions and special requirements for all materials. Upholstery/fabric materials shall have a cut sheet noting furniture item(s) on which they are installed, manufacturer, pattern, color, content, and finish (such as ink‐resistant, mildew‐resistant, etc.), and recommended cleaning method.
C. Delivery of spare parts or materials to Owner. D. 100 percent completion of the Work. If 100 percent cannot be shown include a list
of incomplete items, value of incomplete items, and when the Work will be completed.
E. Complete final cleanup requirements.
017700.5 WARRANTIES
A. Vendor shall guarantee all work performed and all materials and equipment furnished against defect in material and workmanship for a period of (1) year from the Date of Final Acceptance. Whenever within the guarantee period the Vendor is notified by Owner that an item has proved ineffective or does not meet specification requirements, the Vendor shall immediately replace, repair, or otherwise correct the defect without cost to the Owner, and the guarantee shall be extended another year from the date of repair/replacement completion.

LIBRARY SHELVING Specifications
SCOPE: This specification covers delivery and installation of new Steel Library Shelving of the bracket type and existing shelving. Heights, depths and accessories shall be as indicated on the plans and/or Schedule of Equipment. NEW MATERIALS: Only the finest quality of materials and workmanship will be considered. Sheet steel is to be cold rolled, Class 1 steel. Gauges are U.S. standard. All shelving shall be carefully installed and leveled to the floor and/or walls at the most inconspicuous locations by factory trained specialists. TYPE OF BOOKSTACK: New Steel Bookstacks shall be cantilever, with each unit having a welded frame assembly as manufactured by MJ Industries Inc., of Georgetown, Mass. Welded frames every other unit or starter and adder combinations will not be considered. Commercial or case‐type shelving will not be acceptable. The modular construction shall be such that all components of a section may be removed without affecting the adjacent unit. This will allow any section or range to be divided or rearranged without the necessity of purchasing additional components. Sway bracing which prevents insertion of oversized material (past the center line) on any base or adjustable shelf is not acceptable. STANDARD COMPONENTS ARE AS FOLLOWS:
A) WELDED FRAME: Upright columns of welded frame shall be formed of not less than #16 gauge steel into channel shape with no less than ½” stiffening flanges measuring 2” in the web and 1 1/4 “ at front and rear. Uprights are to be slotted with a series of 3/16” x 5/8” slots spaced 1” on centers. Every sixth slot shall be shaped differently to ease visual leveling of shelves. Bottom spreader of welded frame shall be formed of # 16 gauge steel channel shape measuring at least 1” x 1 ¾” in cross section and be equipped with two adjustable plastic covered levelers to provide protection to floor surface and to prevent the stack units from “walking”. The outer end will receive pre‐drilled weld nuts to receive floor levelers. Bottom spreader to be welded to uprights with open portion facing upward. Top spreader of welded frame will consist of #16 gauge tubular steel shape measuring at least 1” x 2 ½” in cross section. Top spreader to be welded to uprights at concealed locations.
B) CLOSED BASE SHELF: Base shelves shall be one‐piece construction formed of not
less than #18 gauge steel designed to fit around the welded frame uprights and

snap into the base brackets without the need for fasteners. Front height of base shelf shall be 3” and sides shall have stiffening flanges.
C) BASE BRACKET: Base brackets shall be one‐piece and shall fit snugly around the welded frame uprights. Brackets shall be formed of #16 gauge steel and shall have a flange which will rest on the floor. Bracket shall provide a positive leveling capacity. Top and front edge of bracket shall be flanged outward ¼” and profile of base bracket shall be sloped 15 degrees to match the shelf end brackets. Base bracket shall have an impression with a hole in it for attaching adjoining base bracket fasteners contained within the impression.
D) ADJUSTABLE SHELF: All shelves shall be capable of supporting book loads of 50
pounds per square foot without deflection in excess of 3/16”. Nominal shelf sizes formed of #18 gauge steel. Actual dimension of shelf shall be 1” less than the nominal dimension. Front and rear edges of shelf shall be box formed ¾” high, capable of receiving wire book support and snap‐on label holders. Sides of shelf to be flanged downward to allow locking into end bracket grips. Each shelf shall measure at least 35 3/8” clear between end brackets.
E) SHELF END BRACKET: Shelf end brackets shall be formed of #16 gauge steel with a
15 degree sloped front edge. Top, bottom and front edge are to be flanged outward ¼”. Rear edge shall have two crimped hooks at top and a positioning tab at the bottom for engaging into frame slots Two grips are to be provided for engaging side flanges of shelves. Brackets shall extend at least 6” and have an impression to prevent brackets from overlapping.
ACCESSORIES
1) HINGED PERIODICAL SHELF: To consist of sloping display shelves hinged to
adjustable shelf and base shelf brackets. Display shelves shall have 14” actual height, be hinged to allow a clear storage height of 8” and stand without holding when in an open position. Lower edge of display shelf shall have flange and turned up lip to provide a 1 3/8” clearance behind lip.
2) STEEL CANOPY TOPS: To be formed of # 18 gauge steel with a 1 ½” front edge
and supported by #12 gauge brackets which engage in the slots of the frame uprights. Sizes shall extend the full width and depth of base shelf.
3) STEEL END PANELS: Shall consist of one‐piece of # 18 gauge steel formed into
flush profile with 1 ½” square edge and exposed return flanges of not less than 3”. Tops and bottoms shall be closed with spot welded closure flanges. Center of double faced panels shall be equipped with full height channels for attaching to frame uprights and to eliminate oil canning.

4) STEEL BACKS: Shall be # 18 gauge one‐piece construction which completely fills
the space between the uprights from the top channel to the base shelf. Upright fillers shall be provided for attachment.
5) BOOK SUPPORTS: To be one of the following types:
(a) Findable book supports. Cork shall be provided on the base.
6) SHELF BACKSTOPS: Shall be of #20 gauge steel 3” high with stiffening flanges and
hooks to engage into frame uprights. Backstops are to be independent of shelves and/or brackets allowing for adjustment of shelves without moving backstops.
7) HPL COUNTER TOPS: Where specified, top shall be 1‐1/4” T overall with 1/16” T
general purpose grade plastic laminate (1) face and (2) long edges. Where multi‐piece tops are required, joints shall be splined and tite joint fastened. Laminate Color: WilsonArt “Fusion Maple” 7909‐60.
8) PLASTIC LAMINATE END PANELS: Where specified, panels shall extend full height
and full width of shelving and be finished both faces and all edges with 1/16”T general purpose grade plastic laminate or “slatwall”. Refer to Architectural Design Drawings for additional details. Color: WilsonArt “Fusion Maple” 7909‐60
9) WOOD TOP BRACKET: Bracket is made from # 14 gauge steel with a flange at the top with three holes for fastening tops. Bracket measures 7” in length. One pair of brackets per section face is used for attaching tops.
10) CORNER FILLER: Shall consist of two side pieces and a top cap. All shall be # 18 gauge steel. The side pieces shall be formed with return bends and have a receiver clip spot welded to them to allow the sides to slip together without any exposed hardware. The top cap shall close the space at the top so as to prevent objects from falling in the corner.
