It gives me a great pleasure to welcome you all to this year's Annual Meeting, celebrating not only our last

year's accomplishments, but also 43 years' worth of Optho Remedies Pvt. Ltd achievements.

Such occasions & meetings builds more strong connections among us, after all-that's what it's all about for

us:. successful community inclusion. I want to start by thanking you for joining us today; your support and

presence here, is indicative of the steps we've made in the right direction.

This event is a key step towards a collective vision of integration and target achievement strategies. As it is a

deep belief that cooperation and integration will greatly strengthen our roots and bring benefits to all .

Thus, this meeting today embodies this belief.

March is the month dedicated in ophthalmic fraternity

for creating awareness and adding fuel to the fight

against Glaucoma. Optho remedies tries to contribute

the maximum to 'World Glaucoma Patient Association'

(WGPA). Optho remedies has been conducting regular

glaucoma awareness activities throughout the year on

6th of every month to symbolize the beginning of 'World

Glaucoma Week'. Glaucoma Awareness Walk &

Glaucoma OPD campaign have been done through out

the week, thousands of Glaucoma Patients were screened

in different parts of the country. We will be organizing

awareness camps round the year on 8th & 20th of every

month at leading hospitals & institutes, to mark the

beginning of “World Glaucoma Week 2017” and Optho

thThe 74 . AIOC, 2016 was held at Kolkota from 25th-28th.Feb., 2016.The conference was organised by

All India Ophthalmic Society.The conference took place at Science City & Milan Mela ground . More

than 150 companies participated including surgical companies and publication houses. The event

witnessed the presence of more than six thousand delegates, including trade people.

Honourable Mr. Somnath Chatterjee, former speaker of parliament was the chief guest of the

conference. Prof. Priya Abraham of Christian Medical college and Hospital, Vellore, India and Prof.

Arunaloke Chakrabarti PGIMER, Chandigarh were the key speakers of the conference. The conference touched and focused on all

spheres like Eye Surgeries, Ophthalmology and Eye Health.

Highlight of the event: Medical session are common phenomenon to every conference, as management lesion has touched all

discipline of study so as medical science too.

Mr. Tourbo of Germany conducted a session on skill management for the doctors at AIOS, Kolkota has tied up with IIM, Kolkota to

conduct a session on skill management.

Optho was present at Trade Hall No.-2 ,Semi Pavilion Stall No. A, elegant stall right at the entrance to greet & welcome all

the delegates. All eminent doctor's left their footprint at our stall. Budding ophthalmologists enjoyed the casino game at

our counter. We unveiled our entire ophthalmic products range by means of product collage, displayed at the stall and we

SOC (Stabilized Oxychloro Complex)

SOC is biodegradable polymer and it is an oxidative-

type preservative.

SOC dissipates by converting into components normally

found in tears, such as sodium ions, chloride ions, oxy-

gen, and water. This unique quality makes SOC an

unusually gentle preservative that is ideal for chronic

use. For this reason, products containing SOC may have

an improved tolerability profile, reduced toxicity,

and, ultimately, better patient compliance.

SOC consists of an equilibrium mixture of oxychloro

species: 99.5% chlorite (ClO 0.5% chlorate (ClO), and

trace amounts of chlorine dioxide (ClO which have

bactericidal, fungicidal, and virucidal activity.

Thus SOC has preservative action in bottle and product

Products of the Spring/Summer SeasonFalling leaves, flowers, blossoming all around and bright sunlight is the announcement of the arrival of the most beautiful spring season. It is the most pleasant and enjoyable season , the whole of the environment is filled with pollen grains lingering everywhere making the life of inhabitants miserable . Pollen grains are the main cause of ocular allergy. Thus, the beginning of the spring season in beginning of the ocular allergy. Optho is marketing various formulations to cover the allergic sign & symptoms. Odin eye drop is one of the formulation which cures the ocular allergy miraculously.Odin Eye drops :This is not a new product for all of you have been promoting it successfully since long . But now odin has arrived in the new look. As the concentration of the main effective ingredient olopatadine concentration has been raised to 0.2% as compared to 0.1% . The enhancement of our odin makes more efficacious and more convenient to use.Odin eye drop is clear ophthalmic solution to control the seasonal and perennial allergic conjunctivitis, and intense itching caused due to change of season (like spring ) and various allergens.Odin contains olopatadine in higher concentration of 0.2%.. Olopatadine is a white coloured powder, which is freely soluble in water to provide a clear and shining solution. Odin eye drop is especially designed to cure allergic conjunctivitis, associated with intense ocular itch and tearing from the eye. Which may caused due to change season or allergen such as pollen mould or animal danger . These are foreign body which is also known as allergens.The ocular allergic conjunctivitis is caused when allergen or antigen such as pollen grain mold binds with antibody on the surface of mast cells to induce degranulation which releases a content of granules or mixture of components (hiscamine, cycokine etc.) Thus, histamines releases in eyes and further causes the vascular response which is manifested in the form of redness, itching ,watery discharge & mucus discharge.Dual Activity of Odin :- The drug which counteracts in more than one component ,always play a better role ,Like wise Olopatadine , on the one hand blocks H1 activity and on the other hand it inhibits the release of histamines, tryptase PGE2 from human conjunctiva mast cells. The dual action of ODIN 0.2% makes it a unique anti allergic conjunctivitis drug.

Duration of Action : Most of the anti allergic preparation require frequent dosing and thus are inconvenient to use. ODIN 0.2% drop is unique wipes out itch immediately after the instillation of first drop and effective for 24 hrs, thus it once daily. Thus its most convenient for the use by patient.Dosage & Administration: 2 drop in affected eye one daily.ODIN is comfortable :- Odin provides minimum ocular discomfort after application.

ODIN VS KETOROLACBoth the drugs are for similar kind of use i.e . allergic conjunctivitis and ocular itch. Odin has faster onset of action as compared to Ketorolac (KT) KT requires frequent dosing every 2 hourly.KT (ketorolac) irritant nature, where ODIN has no ocular discomfort, thus great patient acceptance.

Tocin - F is anti bacterial & anti inflammatory preparation as it contains Tobramycin 0.3% w/v & Fluoromethalone 0.1% w/v in the form of microfine aqueous suspension.

TOCIN-F Eye dropsTobramycin sulphate:1. It is a white coloured powder which is freely soluble in water and gives a clear solution.2. It is broad spectrum antibiotic active against gram positive & gram negative microorganism. But unlike other antimicrobial its spectrum for gram-negative organism is very broad as compared to any other anti microbial. Due to its unique spectrum it is the most preferred antibiotic .3. S i n c e To b r a m y c i n i s n e v e r recommended as Oral or systemically hence resistance has never been reported.4. Tobramycin has minimal protein binding , a such it penetrates deeper in the corneal layer ensuring deeper penetration.5. Tobramycin is safest and well tolerated antibiotic for children and elderly patients.6. It belongs to aminoglycoside group of antimicrobial.7. Tobramycin is choice for treating the infection in pre & post operat ive state , corneal u lcer & ophthalmia neonatrum. The infection which occurs in eye of new born kids.Fluoromethalone:1. This is a white coloured powder and is insoluble in water to provide a suspension .2. It belongs to corticosteroid family .But it is considered to very safe drug as it does not raise the IOP . Reason for Safety: Fluoromethalone is very poorly absorbed by the cornea. Whatever quantity is absorbed by the cornea is very rapidly eliminated . Therefore this can be used for longer period of time without any steroidal side effect.3. Being a corticosteroid fluoromethalone is anti-inflammatory and anti allergic. Especially in chronic conjunctivitis which persist for longer period , fluoromethalone can be used to eradicate the same very safely.4. Offcoure fluoromethalone being a corticosteroid is strictly contraindicated in Glaucoma & corneal ulcer .

Both the molecules in TOCIN–F eye drop are safe for the use of prolonged therapy . The formulation safe & well tolerated in (babies/kids)* .Therefore whenever the patients coming with allergic conjunctivitis and secondary infection, or in chronic allergic conjunctivitis persisting for long time .TOCIN – F is a drug of choice .It can also be prescribed, when the patient are not responding to other antimicrobial drugs and have developed resistance.

* It is suggested to promote Tocin-F among paediatric doctor's too.

Causes of Red Eye :

Conjunctivitis is the most common cause of red eye and is one

of the leading indications for antibiotics. Causes of

conjunctivitis may be infectious (e.g., viral, bacterial,

chlamydial) or noninfectious (e.g., allergies, irritants). Most

cases of viral and bacterial conjunctivitis are self-limiting.

Other common causes of red eye include:

1. Blepharitis

2. Corneal abrasion.

3. Foreign body.

4. Subconjunctival hemorrhage.

5. Keratitis.

6. Iritis.

7. Glaucoma.

8. Chemical burn.

9. Scleritis.

A thorough patient history and eye examination may provide

clues to the etiology of red eye. The history should include

questions about unilateral or bilateral eye involvement,

duration of symptoms, type and amount of discharge, visual

changes, severity of pain, photophobia, previous treatments,

presence of allergies or systemic disease, and the use of

contact lenses. The eye examination should include the

eyelids, lacrimal sac, pupil size and reaction to light, corneal

involvement, and the pattern and location of hyperemia.

Preauricular lymph node involvement and visual acuity must

also be assessed. Common causes of red eye and their clinical

presentations are summarized under:

How the corneal epithelial wounds develop and which mechanisms are involved in the resurfacing of the cells??

Damage to the corneal epithelium caused by chemical, physical or microbial insults results in swelling of the stroma, activation of stromal fibroblasts and infiltration of various inflammatory cells, leading to the loss of corneal transparency and integrity. Therefore, it is important to repair corneal epithelial damage as rapidly as possible. Most corneal epithelial wounds are indeed repaired promptly. However, under certain clinical conditions, resurfacing of the corneal epithelium is delayed. Knowledge of the mechanisms that underlie the healing of corneal epithelial wounds is crucial for the development of new treatments. Resurfacing of corneal epithelial defects occurs in three phases: 1) Immediately after the wound or injury of epithelial cells, the neighbouring intact epithelial cells begin to migrate over the affected area until they cover it with a cell monolayer.

2) This is followed by the proliferation of the migrated epithelial cells, resulting in the restoration of the normal epithelial thickness.

3) A few weeks after the wounding occurred, the epithelial cells begin to differentiate, the surface of the healed region becomes smooth and a well-layered structure is restored.

The three phases of corneal epithelial wound healing are thus characterized by migration, cell proliferation, and cell differentiation, respectively. Two different types of epithelial movement during the covering process of the exposed area of the cornea have been observed as shown in figure 1: epithelial abrasion (upper panel) is followed by an initial sheet-like movement of individual epithelial cells (middle panel) and a subsequent landslide-like movement of the remaining epithelium enmasse (lower panel).

The matrix metalloproteinases (MMPs) are a family of

proteolytic enzymes whose function is to maintain and

remodel the tissue architecture. They have been implicated in a

wide range of processes and diseases and their substrates

comprise a variety of extracellular matrix components,

secreted cytokines and cell surface molecules. MMPs including

MMP-9 (gelatinase B), -2 (gelatinase A), and -1 (collagenase)

have been reported to be involved in the pathogenesis of 2ulceration and pterygia of the cornea.

Role of CD44 in regeneration of corneal cells with the help

of sodium hyaluronate:

CD44: The CD44 antigen, a type 1 transmembrane

glycoprotein, is the main receptor for hyaluronan. It is present

on cell membranes of almost all human cells. A specific

function of CD44 is the capability to bind and internalize HA;

however, it can interact with other ligands, such as fibronectin,

collagens, osteopontin, and matrix metalloproteinases

(MMPs). It has proven to work as a docking agent for MMP9 and 5as a growth factor reservoir. In a wound environment, CD44 is

responsible for cellular internalization of HA degradation

products. The other role of CD44 signalling in the wound

environment is to induce fibroblast migration from

surrounding tissue into the wounded area. It is noteworthy

that neither CD44 nor HA alone can induce cell migration and

promote wound healing; an interaction between HA and CD44 5,6is essential to activate this process. . the ability of hyaluronan

particles to bind with the CD44 receptor depends on its

molecular size.

CD44 (receptor) are involved in cell- cell interaction and that

they provide tensile strength for healing epithelial cells,

whereas the others may serve as an adhesion molecule that is

required in cell-matrix interactions for effective cell migration,

including the lateral movement observed during wound

closure and the upward movement seen during epithelial cell

hyper proliferation.

Purpose: Sodium hyaluronate (hyaluronic acid) is known to promote corneal epithelial wound healing in vivo and in vitro, in animal experiments. Sodium hyaluronate is the ligand for CD44, a cell surface adhesion molecule which has been found on normal human corneal epithelial cells. The purpose of this study was to investigate the effect of sodium hyaluronate on human corneal epithelial cell migration, proliferation, and CD44 receptor expression.

Aragona et al, the effects of longer term treatment with 0.15%Sodium Hyaluronate (preservative-free) compared with 0.9% Sodium Chloride (Saline), on the ocular surface of patients with dry eye syndrome was investigated. After 3 month treatment in patent with medium to severer dry eye, sodium hyaluronate improved the impression cytology score with a statistically significant difference compared to the baseline significant difference compared to the baseline value (p=0.024 v baseline). At the same time the difference compared to 3 months treatment with 0.9% Sodium Chloride was also statistically significant (p=0036 v Sodium Chloride 0.9%). After 3 months treatment with 0.15% Sodium Hyaluronate the main cytological further of impressions made against the temporal interppalpebral bulbar conjunctiva were the presences of smaller cells with both polygonal and round shape and the presence of goblet cells. This was compared to the 0.9% Sodium Chloride treated group where cytological features were presence of polygonal cells with keratinised, basophilic cytophlasm, markedly reduced nucleus/cytoplasm ratio and an absence of globlet cells. It was concluded that 0.15% Sodium Hyaluronate may effectively improve ocular surface damage associated with dry eye

6syndrome .

Methods: Human corneal epithelial cell cultures were established from 32 donor corneoscleral rims and maintained separately in three different culture conditions: (1) culture medium only, (2) sodium hyaluronate enriched (0.6 mg/ml) medium, and (3) hydroxypropylmethylcellulose enriched (2.5 mg/ml) medium. The total area of migrating epithelial cell sheets in each case was measured by planimetry on days 4, 8, 12, and 16. Cytospin preparations of cells cultured in the different culture conditions were examined immune-histochemically for proliferation and CD44 receptor expression using antibodies directed against Ki67 and CD44 respectively.

Results: Cells cultured in the presence of sodium hyaluronateshowed significantly increased migration at days 12 and 16 (Friedmen test: p� =� 0.0012, day 16; p� =� <0.001, day 12) compared with cells cultured in the other media. There was no difference in cell proliferation (Ki67) or CD44 expression on cells cultured in the different culture conditions.

Conclusions: Sodium hyaluronate promotes migration but not proliferation or CD44 expression on human corneal epithelial cells in vitro. The beneficial effect of sodium hyaluronate in corneal wound healing is likely to be related to rapid migration of cells leading to rapid wound closure. This may be facilitated by the adhesion between CD44 on the cells and hyaluronic acid, which coats the surface of the denuded cornea.

This characteristic of Sodium Hyaluronate is described as pseudoplastic behaviour. The pseudo plastic properties of Sodium Hyaluronate enable it to be a sufficiently viscous and static substance when at rest and to transform to a more fluid and mobile substance when a stress force is applied. This behaviour ensures a high lachrymal viscosity when the eye lids are open which contributes to enhanced tear film stability and a reduction in tear film evaporation. This should also equate to a reduced requirement on the frequency of instillation of Sodium Hyaluronate for the treatment of dry eye. In addition during increased blinking and subsequent increased shear stress on the tear film, the progressive reduction in viscosity of sodium hyaluronate helps to reduce any friction of movement between the eye lids and the ocular surface.

Fluorophotometric measurement of the precorneal resisdence time of topically applied hyaluronic acid.

AbstractPurpose: This study was performed to separately assess the aqueous flow applied with hyaluronic acid, and the behavior of hyaluronic acid itself on the ocular surface.

Methods : Two different fluorescent dyes, fluorescein sodium dissolved in 0.1% fluorescein conjugated with hyaluronic acid (F-HA) dissolved in saline, were used. A volum of 20 ul of tested solution was applied to teh eye of 10 healthy volunteers. Fluorescein sodium dissolved in saline served as a control. The fluorescent intensity of the precorneal tear film was measured at eh central cornea every minute for 10 min. The turnover rate was calculated using the equation that plots fluorescent intensity against time the equation that plots and expressed as % min.

Case Study Viscoelastic propertiesEsto,ates of the ocular surface residence times of 0.3% Hydroxpropylmethylcellulose (Hypromellose), 1.4% Plolyvinyl Alcohol and 0.2% Sodium Hyalyronate were made using quantitative gamma scinitigraphy in six patients with keratoconjunctivitis sicca. Sodium Hyaluronate hacl a mean half life on the ocular surface of 321 seconds, which was significantly longer than Hydroxproplymenthylcellulose 44 sconds; p=0.012 and Polyvinyl Alcohol 39 seconds; p=0.013. Given its prolonged residence time, favourable rheology, and emerging clinical support, Sodium Hyaluronate may

3come to pay a greater part in the treatment of dry eyes .

Conclusion : Our results indicate that the bulk aqueous flow applied with HA and the turnover of HA itself are different. HA molecules may adhere to the ocular surface by surface-chemical and/or the ocular surface may explain the mechanism in which hyaluronic acid has been show to enhance tear film stability for a few hours.

Results: Turnover rates of topically applied 0.1% F-HA, 0.1% HA and saline were 8.1 (SD 3.6)% min, 21.6 (2.8)%, min 31.0 (3.7)%min, respectively. AbstractSolutions of hydroxypropylmethylcellulose (HPMC) and polyvinyl alcohol (PVA) are widely used as artificial tears. However, their usefulness is limited by the short duration of their effect. Dilute sodium hyaluronate (SH) solutions exhibit non-Newtonian rheology with high viscosities at low shear rates, which would be expected to enhance their ocular surface residence time. Using quantitative gamma scintigraphy, estimates of the ocular surface residence times of 0.3% HPMC, 1.4% PVA, and 0.2% SH were made in six patients with keratoconjunctivitis sicca. The sodium hyaluronate solution had a mean half-life on the ocular surface of 321 s, significantly longer than hydroxypropylmethylcellulose (44 s; p = 0.012) and polyvinyl alcohol (39 s; p = 0.013).
