Benefits 

Eligibility 

Checkup

Using an on-line benefits calculation tool, trained staff screen seniors for hundreds of federal, state and local benefits programs, including; prescription drug coverage, weatherization, energy assistance, & financial assistance. Staff then works with seniors in applying for identified benefits.

Caregiving 

Resources

Informal/familial caregivers are provided a variety of assistance and supports to aid them in caring for their older loved ones. Services such as one-on-one counseling, family mediation, support groups, newsletters, information and referrals and education opportunities are available.

Care 

Coordination 

Services

Through a referral from I&A, trained and certified care coordinators provide a comprehensive needs and eligibility assessment and create appropriate individualized plans of care utilizing a variety of in-home services, allowing seniors to live as independently as possible in their homes and communities. Plans of care are monitored and updated as needed.

Chicago Fitness 

Plus

Award-winning fitness program that boosts endurance, strength, balance and flexibility. Senior centers are equipped with senior friendly exercise equipment and staffed by experienced, certified senior fitness professionals who lead group exercise classes and one-on-one training.

Chore/ 

Housekeeping: 

Heavy Duty & 

Short‐Term

Heavy Duty Chore is an one-time intensive cleaning for seniors whose living conditions pose a threat to their health and safety. Services include trash removal, window cleaning, moving heavy furniture and packing. Short-Term Chore assists seniors recuperating from a short-term illness; a temporary medical disability; and/or has been recently discharged from a medical setting. The temporary housekeeping assistance includes: light housekeeping tasks, laundry, and/or shopping.

Adult Protective 

Services Program

Trained and certified Adult Protective Services Caseworkers respond to reports of alleged physical, emotional and sexual abuse, neglect, confinement or financial exploitation of any person, age 18-59 living with a disability or older adults living in the community. The program provides assessment, investigation, intervention and follow-up services to victims. Investigators work with the eligible adult to develop a plan to alleviate the abusive situation and reduce the risk of further harm to the eligible person.

Foster 

Grandparent 

Program

Opportunity for seniors to make a difference in a child's life. This program also gives the senior volunteer a presence in the community, free annual physicals and the ability to supplement their monthly income.

DFSS Senior Services Programs and Services: At a Glance

Golden Diners 

Nutrition 

Program

Hot, nutritious lunches in a communal setting are provided at approximately 60 community locations each weekday throughout Chicago. In addition to meals, seniors can participate in a variety of activities, from health education and exercise classes to field trips.

Home Delivered 

Meals

Microwave-ready frozen or hot meals delivered to home-bound seniors who are unable to prepare meals for themselves and have no other means of acquiring nutritious meals. This service is available after an in-home assessment by a care coordinator.

Health & Wellness 

Program

Health promotion and disease prevention programming provided at all DFSS senior centers through monthly presentations, health screenings, and one-on-one consultations with wellness nurses, dietitians, pharmacists, and social workers.

Older Relatives 

Raising Children

For seniors who have primary caregiving responsibility of a child, from birth to age 18 or those caring for their severely-disabled adult children. Short-term one-on-one counseling and support groups are available. Emergency financial assistance can be accessed for one-time emergency rent payment, school uniform and equipment, medication, and other basics.

Ombudsman 

Program

Ombudsmen protect, defend and advocate for residents in long term care facilities such as nursing homes and assisted living facilities. Utilizing staff and volunteers, they inform residents and their families of their rights, resolve complaints, provide information on residents needs/concerns to families, facility staff and the department, and advocate for quality individualized care for the residents.

Respite Care

Allows family caregivers caring for a senior, time for themselves. Respite provides professional trained staff to care for a frail senior, who has a familial caregiver, on a temporary basis. This care can be provided in home or in a nursing facility. Respite can be used in an emergency, such as the illness of a caregiver or for planned time away; such as a vacation, special event, appointment or just some time for themselves.

Senior Companion 

Program

Opportunity for seniors to make a difference in the lives of lonely, frail and homebound seniors by reading, engaging in conversation, preparing light meals and more. This program also gives the senior volunteer a presence in the community, free annual physicals and the ability to supplement their monthly income.

Senior Medicare 

Patrol

The SMP (Senior Medicare Patrol) Program educates consumers about health care fraud, waste, and abuse. Program staff and volunteers help people with Medicare and Medicaid to: educate, detect, and report possible Medicare and Medicaid fraud, waste, and abuse.

Employment 

Training 

Employment training and job placement for seniors (at least 55 years old) for re-entry into the job market.

ITINs with the middle digits 78 or 79 (i.e. 9XX-78-XXXX) will expire on January 1, 2017.

ITINs that have not been used on a tax return in the last three years (2013, 2014, or 2015) will expire on January 1, 2017.

Keep your cash. You earned it!

The City of Chicago offers free, confidential tax help at locations across Chicago from late Januarythrough mid-April. Federal and Illinois tax return preparation and e-file are offered free of charge by the City’s two partner nonprofits, the Center for Economic Progress and Ladder Up. They annually prepare thousands of returns.

If you are a family earning up to $55,000 or an individual earning up to $30,000, you may beeligible to take advantage of this free service and also find out if you qualify for the Earned Income TaxCredit (EITC), Child Tax Credit (CTC), and/or student financial aid for college! Trained volunteertax preparers can assist you in completing Federal and state income tax returns for the 2016 taxyear and, in some cases, for prior years.

Plan Ahead. If you qualify for the Earned Income Tax Credit and/or the Additional Child Tax Credit, a new law that takes effect in 2017 means that the IRS cannot issue your refund before February 15. No one can speed up your refund, and the City’s tax help is free.

Durable MedicalEquipment (DME)

Make sure that Medicare will cover it!

Walkers, wheelchairs, hospitalbeds, and back/knee braces aresome examples of DME. DME ismainly used in the home.

What is DME? If you need DME, you and yourdoctor can make that decision.They know your health needs.

Talk to yourdoctor first!

Mrs. Smith received a call from a mannamed Jim who said he worked forMedicare. Jim asked Mrs. Smith if she hadany pain, and she told him that sometimesher back was sore. Jim said Mrs. Smithcould receive a "free" back brace becauseMedicare will pay for it. Jim sent Mrs.Smith a back brace and used herMedicare number to bill Medicare for avery expensive back brace. The backbrace did not fit Mrs. Smith, so she neverused it.

One year later Mrs. Smith injured her backafter a fall, and her doctor wrote aprescription for a back brace for her.

However, Medicare would not cover thecost of the back brace, because Medicarealready paid for a back brace for Mrs.Smith (the "free" one Jim sent her). Mrs.Smith lost her benefit.

It is important to always work with yourdoctor for your health care needs,especially when it comes to DME. Don'ttake the chance of losing your benefits byaccepting "free" equipment that is not rightfor you or something you may not need.

This is a common example of fraud...

Be careful with your Medicare number,so this does not happen to you!

Protect Your DME Benefits

A sales person who calls youon the phone CANNOT:

Diagnose or assess your health care needs

Prescribe durable medical equipment (DME) for you

Fit you for DME over the phone

That is the job of your doctor. Your doctor knows you and your health history.Don't let a sales person talk you into something you do not need.

Would you take medicine from a person other than your health care provider? Ofcourse not! The same holds true for DME. Only your doctor can order DME for you.

It is impossible for someone calling on the phone to make sure equipment will fityou, and that is the job of a DME supplier. A good DME supplier will work with youuntil the equipment fits you just right and works properly for you.

Medicare fraud by not giving out your personal information tosomeone you do not know.

Medicare fraud by reading your Medicare Summary Notice tomake sure the claims for services and supplies are what youactually received.

WAIVERS: Every trip/event participant must register by completing and signing a Senior Services Emergency Contact/Waiver. RESERVATIONS: Reservations are not guaranteed until the ticket is paid for. Please do your best to register as early as possible, since many of our trips and events fill up quickly. REFUNDS: In order to make programs available to as many people as possible, we do our best to keep costs low. We base the cost per person by dividing the cost of the outing by the number of people we expect on the bus. We therefore regret we are unable to give refunds or discounts. However, if we have a waiting list and are able to sell your seat on or before the day of the trip, we will be happy to refund your money. Otherwise, we suggest you give your ticket to a friend as an “early” birthday or holiday present. Naturally we will refund your money if an event is canceled or significantly altered by Senior Services. WHEELCHAIRS: If you use a wheelchair and would like to participate, please contact us at (312) 746-4798 to discuss arrangements. TELEPHONE NUMBERS: When you register, make sure that you provide us with your phone number. Sometimes, in order to fill buses, we need to combine two locations. This can result in a change in the date of your program. If we have your phone number, we can contact you to work out these details. PROMPTNESS: Most of our events and trips are on tight time schedules, and it is our policy to leave promptly at the stated departure time. This makes us unable to wait for latecomers. We regret that no refunds can be given to tardy participants who miss a bus. NAME TAGS: We require participants to wear name tags as identifiers as part of our safety procedures. ABOUT OUR TRIPS AND EVENTS CONT… GROUP ISSUES: Group leaders must obtain reservation numbers to register for trips. Payment must be made by the Money Due Date given at time of registration. Unless otherwise designated, Group leaders are given one complimentary ticket when leading 19 or more participants. When two groups share a bus, the first group boarding the bus should fill in the driver’s side of the bus, leaving the passenger side for group two. Front seats by the driver are reserved for the group’s leader or Family & Support Services staff. Group leaders are responsible for taking attendance to make sure everyone is accounted for and for bringing emergency forms for each participant. Group leaders must immediately let staff know of any accidents or incidents that occurs during the trip. (We salute the energy and dedication of our group leaders! We couldn’t host these programs without them). ACCOMMODATIONS FOR PERSONS WITH HEARING IMPAIRMENTS: If you need a sign language interpreter for any of our lectures, programs or classes, please let us know as soon as possible so we can make the necessary arrangements. You can let us know by either calling your center on your TTY, writing a letter, stopping in or asking a friend to contact us. We welcome your participation. PARKING: The five Regional Senior Centers have adjacent free parking. Regrettably, Renaissance Court has NO free parking, but plenty of public transportation options. PREPARATION: Since some of our outings require walking, climbing or varying amounts of stamina, participants are responsible for learning about programs and trips before signing up for them, paying close attention to all trip details and any trip advisories or cautions. Proper footwear and clothing are the participant’s responsibility. Diabetics may want to consider bringing their own snacks should they have concerns about an event’s meal service time. WELLNESS PROGRAM JANUARY - MARCH 2017

The most important component of the Wellness Program is YOU! Healthy

living helps to prevent chronic disease and certain disabilities. It can

ensure that today’s older persons as well as future generations not only

live longer, but also live better. It’s never too late to adopt healthy lifestyle

habits. The Wellness Program can help you start a personal wellness

program or provide you the support you need to maintain your wellness

goals.

- The Wellness Program at Central West, Southeast (Atlas) and the

Southwest Senior Centers is provided by the Bowman Center/Rush

University Medical Center.

- The Wellness Program at the Northeast (Levy) Senior Center,

Northwest (Copernicus) Senior Center, and Renaissance Court is

provided by White Crane Wellness Center.

At each senior center, the latest information on healthy living and disease

prevention is provided through monthly talks with the wellness nurse,

social worker, dietitian, and pharmacist.

Take a look at the wellness schedule at the center nearest you and meet

the wellness team. It is the first step to a healthier lifestyle.

WELLNESS TEAM OF PROFESSIONALS

WELLNESS NURSE

The Wellness Nurse is available weekly at each center. The Wellness Nurse is knowledgeable about health issues and provides education, guidance and insight. The Wellness Nurse provides blood pressure and other health screenings, support groups, and monthly health talks to help you manage your health.

SOCIAL WORKER

The Social Worker is available one half day several times each month by appointment for private, one-on-one consultations. See the Wellness Social Worker for advice and information on resources for those times when you are faced with life’s challenging issues. The Wellness Social Worker also leads a discussion group on a variety of topics focusing on contemporary issues of aging.

DIETITIAN

The Dietitian is available monthly to assist you in achieving your nutritional goals with one-on-one consultations, nutritional health talks, and discussion groups. The Wellness Dietitian shares insights about dietary needs, strategies to make positive lifestyle changes as well as the relationship between nutrition and your health.

PHARMACIST

A Pharmacist is available at each center one half-day each month. In addition to informative presentations and question and answer sessions on medications and health-related issues, the pharmacist is available by appointment for private, one-on-one consultations to address your concerns. Make an appointment for a “brown-bag” medication review (bring all of your medications into the pharmacist for review) to receive help with timing your medications or bring in your lab results for discussion. NURSE PRESENTATIONS Pain and its Problems: Concerns about Substance Abuse For some it can be a struggle balancing the need for chronic pain relief with the desire to prevent substance addiction or relapse. Living with chronic pain can disrupt sleep patterns, cause profound stress, lead to depression, and affect work and family responsibilities. The nurse will discuss the importance of being cautious with treatment and of monitoring intense pain, particularly with use of opioid medications such as Vicodin and Oxycontin. We will also discuss pain management alternatives, which include non-addicting medications and medication-free mind-body therapies.

Learn how to eat a nutrient-dense diet with plenty of whole grains, fruits, vegetables, and dietary fats to maintain optimal nutritional health no matter your health issue.

PHARMACIST PRESENTATIONS Pain Management – What You Need to Know Pain is one of the most common symptoms that occur with aging and chronic disease. A majority of seniors live with pain on a daily basis and often seek many different treatment strategies to achieve relief. Since many older adults often use medications, the goal of this session is to review the benefits and risks of the various drug therapies used to manage pain. An emphasis will be placed on the recent national initiatives designed to control and decrease the use of powerful “opioid” pain medications and what you can do to maintain your independence and a healthier quality of life.

Bitter cold temperatures are more than an inconvenience; they can pose a serious threat to health and safety. The City of Chicago has many services available to help residents weather the winter months.

to extreme cold weather.• Report inadequate heat in a residential building.• Learn about programs that assist with home heating costs.• Connect to shelter and supportive services.

Protect Yourself and Family:• Wear several layers of loose, warm clothing and keep your head, hands and feet

covered when outdoors.• Avoid alcoholic beverages.• Recognize the symptoms of hypothermia: stiff muscles, shivering, puffy or

swollen face, cold skin, slow breathing and mental confusion.• Never use an extension cord with a space heater.• Ovens should not be used to heat homes.• Landlords must heat residential buildings to at least 68 degrees during the day

and 66 degrees overnight. If you are unable to resolve a heating problem with your landlord, call 3-1-1.

Don’t Take a Chance With a Heart Attack: Know the Facts and Act Fast

More than 1 million people in the United States have heart attacks each year. Many of them don’t act quickly enough

to make it to the hospital on time for help. When a heart attack happens, delay in treatment can be deadly. Learn the warning symptoms of a heart attack, and know the single most important thing you can do to save a life: call 9–1–1 immediately for emer-gency medical care.

What Is a Heart Attack and Who Is At Risk?A heart attack happens when blood flow to the heart suddenly becomes blocked and the heart can’t get oxygen. If not treated quickly, the heart muscle fails to pump and begins to die.

Heart attacks most often occur as a result of coronary heart disease (CHD), also called coronary artery disease. Certain risk factors make it more likely that you will develop CHD and

have a heart attack. Major risk factors for a heart attack that you can control include smoking, overweight and obesity, high blood pressure, high cholesterol, diabetes, an unhealthy diet, and lack of physical activity. Risk factors that you can’t control include a family history of early heart disease and increasing age. Talk to your health care provider to find out whether you are at risk for a heart attack.

Of the people who die from heart attacks, about half die within an hour of their first symptoms and before they reach the hospital.

Ann Steigler’s Story

One evening I felt pressure in my chest and tingling down both arms. I just didn’t think I could be having a heart attack. I didn’t expect it to happen to me. The next morning, I still had a strange sensation in my chest. I called the doctor and was sent to the hospital. Consid-ering how many first heart attacks are fatal, I was lucky to be alive the following morning to seek medical help.

Know the Symptoms of a Heart AttackMany people aren’t sure what’s wrong when they are having symptoms of a heart attack. Some of the most common warning symptoms of a heart attack for men and women are:

Chest pain or discomfort. Most heart attacks involve discomfort in the center or left side of the chest. The discomfort usually lasts for more than a few minutes or goes away and comes back. It can feel like pressure, squeezing, fullness, or pain. It also can feel like heartburn or indigestion.

Upper body discomfort. You may feel pain or discomfort in one or both arms, the back, shoulders, neck, jaw, or upper part of the stomach (above the belly button).

Shortness of breath. This may be your only symptom, or it may occur before or along with chest pain or discomfort. It can occur when you are resting or doing a little bit of physical activity.

Heart Attacks Don’t Always Cause Common SymptomsNot all heart attacks begin with the sudden, crushing chest pain often seen on TV or in the movies, or other common symptoms such as chest discomfort. The symptoms of a heart attack can vary from person to person. Some people can have few symptoms and are sur-prised to learn they’ve had a heart attack. If you’ve already had a heart attack, your symp-toms may not be the same for another one. It is important for you to know the most common symptoms of a heart attack and also remember these facts:

Heart attacks can start slowly and cause only mild pain or discomfort. Symptoms can be mild or more intense and sudden. Symptoms also may come and go over several hours.

People who have high blood sugar (diabetes) may have no symptoms or very mild ones.

The most common symptom, in both women and men, is chest pain or discomfort. But women also are somewhat more likely to have shortness of breath, nausea and vomiting, unusual tiredness (sometimes for days), and pain in the back, shoulders, and jaw.

Quick Action Can Save Your Life: Call 9–1–1Any time you think you might be having heart at-tack symptoms or a heart attack, don’t ignore it or feel embarrassed to call for help. Call 9–1–1 for emergency medical care, even if you are not sure whether you’re having a heart attack. Here’s why:

Acting fast can save your life.

An ambulance is the best and safest way to get to the hospital. Emergency medical services (EMS) personnel can check how you are doing and start life-saving medicines and other treatments right away. People who arrive by ambulance often receive faster treatment at the hospital.

The 9–1–1 operator or EMS technician can give you advice. You might be told to crush or chew an aspirin if you’re not allergic, un-less there is a medical reason for you not to take one. Aspirin taken during a heart attack can limit the damage to your heart and save your life.

Every minute matters. Never delay calling 9–1–1 to take aspirin or do anything else you think might help.

Your heart may stop beating during a heart attack. EMS technicians have the training and equipment needed to treat you if this happens.

If you are unable to reach 9–1–1, have some-one else drive you to the hospital right away. Don’t drive yourself to the hospital. You can cause a car accident if your symptoms worsen.

When given quickly, medicines and other treatments can stop a heart attack.

Action Steps To Take Now—Before a Heart Attack HappensWork out an emergency heart attack action plan with your spouse or someone else close to you so you will know what to do if a heart attack occurs. This is especially important if you are at high risk for a heart attack or have already had one. Steps you can take now include:

During a routine office visit, talk to your doc-tor or health care provider about your risk for a heart attack. Find out now whether you can take aspirin if you have heart attack symptoms in the future.

Write down important information to share with EMS and hospital staff in case of a medical emergency. (See box, What To Write Down and Keep in a Handy Place.)

Put your written information in a handy place and tell someone else where it is. Share it with family, coworkers, and neighbors, and always keep it with you.

Consider taking part in a research study (clinical trial) if you’ve had or are at risk for a heart attack. Research supported by the National Heart, Lung, and Blood Institute (NHLBI) has uncovered some of the causes of heart diseases and conditions, as well as ways to prevent or treat them.
