Section 1: Introduction to the Ongoing Achievement Record
BSc (Hons) Nursing Programme: Adult Nursing The Ongoing Achievement Record (OAR) is a record of your achievement in practice assessment throughout your pre-registration nursing education and is part of your Skills and Practice modules. This record enables you to demonstrate that you have met the Nursing and Midwifery Council (NMC) progression points and Standards of Competence for Pre-Registration Nursing Education (NMC 2010). It is a mandatory requirement for you and your mentors to complete the OAR, the grade contributes to your overall degree. Mandatory Training The Programme requires all students to undertake mandatory training in a number of areas each year. This will usually be provided by Plymouth University at the start of each academic year. Your attendance will be recorded on your POW record. Placement Induction When commencing placement it is a requirement that you complete the appropriate Placement Provider induction within the stipulated timeframe. This is arranged by you placement host. Preparation for Placement Before commencement of each placement you are required to undertake preparatory work and explore the following:
Common patient conditions seen in the placement
Pathophysiology related to these conditions
Key elements of nursing care related to the care of patients in the placement
Assessment tools commonly used in the placement
Common medications and treatments used in the placement (refer to BNF online)

During your placement you will be supported and assessed by mentors. Who you will work under direct or indirect supervision for a minimum of 40% of your placement In your final year 3 placement you will be allocated to sign-off mentor and receive an additional protected hour a week to review your progress.
A mentor is a nurse who has completed specific preparation in assessing students and is responsible for ongoing supervision and assessment in practice settings. Other registered professionals who have been suitably prepared can supervise and contribute to the assessment of nursing students. A sign-off mentor is a mentor who has met additional criteria, is registered in the field of practice that the student intends to enter and makes a final judgement of competence and proficiency during the final placement of Year 3.
Practice Assessment
Assessment of practice is through continuous monitoring of progress. You should gradually move towards a point of practising at a level of proficiency, competence and accuracy under supervision. Formative and summative assessment by your mentor must be documented at set points in the programme. You will be assessed on the following NMC Domains in all placements:

The OAR must be submitted on the stated hand-in date; therefore you should ensure that all documentation is complete. All practice assessments must be signed by your mentor at the end of each practice placement. If this is not possible for any reason then they must be signed within 3 weeks following completion of the placement.
E-Portfolio Your e-portfolio will be assessed by your mentor therefore, you will be expected to share relevant evidence identified in the e-portfolio with them. You will be required to demonstrate that you are developing your skills and knowledge in relation to the assessed domain.

Guidelines for Practice Assessment
Formative assessment of practice - is an essential part of the assessment process that supports your development. The formative
assessment is an opportunity for you and your mentor to review progress, strengths and weaknesses, and to plan how to maintain
your strengths and improve your weaknesses. The formative assessment enables you to undertake a self-assessment and to review
the evidence gathered to support your achievements to date.
Action plan – if there are concerns raised during the formative assessment, the action plan should be completed to help you focus
on improving key elements of your practice.
Summative assessment timing – your summative assessment cannot be undertaken until you have gained at least 20 days or 150
hours experience on your placement. It is recommended that you should be summatively assessed towards the end of your
placement. Any concerns identified after summative assessment should be raised immediately with your personal tutor.
Evidence – to demonstrate achievement of a domain you must provide evidence to your mentor.
In total you are required to provide 3 pieces of evidence:

Non achievement - if you do not provide sufficient appropriate evidence or you have been assessed as not competent you will not pass the
module.
BSc(Hons) Nursing Programme - Assessment in Practice - Placement Referral
Normally students have 2 attempts at practice assessment
If a student has completed 150 hours they are eligible for assessment
All student are normally required to have the following:
Year 1 - Two summative assessments, Year 2 -Three summative assessments, Year 3- Two summative assessments
Any referred domains Re-assessment of all domains can be undertaken in the next placement*
(unless final placement of the year)

*Re-assessment will be classed as a second and final attempt unless supported with valid extenuating circumstances. Mentors will decide the grade however the

Expectations of Students in Placement
While you are in placement you are expected to;
Have undertaken the preparatory work before you start the placement
Work 40% of your time under direct or indirect supervision by a mentor
Work the shifts that the rest of the nursing team work i.e. 24 hour, 7 day if appropriate
Not normally work any more than 75 hours per fortnight (i.e. the equivalent of 37.5 hours per week).
Attend the hand over meetings at the beginning and end of the shifts
Not normally work more than two weekends in any four week period or more than 14 night shifts during the programme.
Where this is difficult, an alternative may be negotiated between student and mentor and personal tutor
Agree a development plan with your mentor within the first week of your placement
Agree the type of evidence that you intend to present to your mentor for each Domain
Identify and complete wider patient activity
Obtain completed written reports from a service user/patient /carer during each year of your nursing programme within some
clinical placements
Year One – at least one report
Year Two – at least two reports Year Three – at least two reports, one of which is undertaken in the final sign-off placement.
Agree a date for your formative assessment with your mentor

Show your mentor your OAR and any evidence that they request to see
Communicate any concerns or incidents to your PDT Lead/personal tutor as soon as possible

Be aware of the Expectations of Students in Placement (see previous page)
Ensure student are directly or indirectly supervised by a mentor for at least 40% of their time in placement
Ensure that the student is rostered to work the same shifts as the rest of the nursing team
Agree a development plan with your student within the first week of their placement
Agree the type of evidence that you expect the student to present for each Domain
Support the student in undertaking wider patient activity and if your student requires it, recommend an appropriate patient/service
user/carer who would be willing and able to provide your student with constructive written feedback related to;
appropriate communication skills
provision of caring and compassionate nursing care
demonstration of dignity and respect at all times
involvement of patient / service user / carer / advocate in decisions of care
provision of information about care
Agree a date for your student’s formative assessment with you

Grading of Practice The practice assessments within this programme are a component of the Skills and Practice modules for each year; therefore contributing to the overall grade.
To effectively achieve equal weighting of marks between theory and practice, practice will be graded and contribute to the overall degree classification for the student at the end of the programme.
Students are assessed in the all 4 domains in each placement area. The mentor will indicate the level of achievement the student
has met for each domain. The level of achievement awarded, from 0 to 100%, should reflect the skills, knowledge and values
pertinent to the stage of the student’s programme and the student should demonstrate these both in the practice area and with
the evidence collated in their e-portfolio. Only grades identified in the banding should be used.
A minimum grade of 40% must be gained for each domain for a pass to be achieved. The OAR contributes to the overall grade for the Skills and Practice modules in each year of the programme assessed by OSCE/written paper;. Referral in either element will result in an overall referral for the module, but the student will only be required to re-take the element on which they have been referred.
If the student is given a score of less than 40% (referral grade) for any domain then an Action Plan will be negotiated between the
student, mentor with the personal tutor/Placement Lead support and guidance on how to develop this domain further for resubmission. The student may work on any referred domains during their next practice placement any resubmitted domains will be capped at a grade of 40% for BSc (Hons) level. Under University Regulations the student is only allowed two summative attempts at practice.
