Supporting people with moderate to severe dementia
in a community setting

Acute Care
Aim: To improve the care and experience of people with dementia and their families/carers across acute settings by July 2016,
through testing and evaluating a range of approaches in relation to 4 specific care actions

Improvement focus on four of the care actions: 1- Leadership 2- Workforce 8- Families, Friends and Carers 9- Stress and Distress

Improving Safety People with dementia &their carers feel safe • Knowledgeable & skilled
staff • Evidence of personalised
care planning • Reduction in
inappropriate antipsychotic medications

Improving Experience People with dementia, their carers & staff feel supported • ‘What matters to me’ is
