Procedural Guidance This document has been accredited by the International Fire Service Accreditation Congress, (IFSAC) operated by Oklahoma State University and the National Board on Fire Service Professional Qualifications (Pro Board). It provides the authority and directive requirements to all DoD fire fighters, Air Force Readiness personnel, and all federal customers participating in the DoD Fire and Emergency Service’s Certification Program (F&ESCP). This procedural guide interfaces with Department of Defense Instruction (DoDI) 6055.06, Department of Defense Fire and Emergency Services Program and Department of Defense Manual (DoDM) 6055.06, The DoD Fire and Emergency Services Certification Program.

Summary of Changes

This “DoD F&ESCP Procedural Guide” dated August 2017, replaces the “DoD F&ESCP Procedural Guide, dated January 2017.

Changes from the previous version are identified with a vertical bar next to the paragraph containing the changed information. Specifically, this revision includes the following significant updates:

Corrects the course number and prerequisites for Telecommunicator I (10611W) and Telecommunicator II (16012W) (Chapter 2 and Attachment 5)

CHAPTER 1 Department of Defense Fire and Emergency Services Certification Program

1.1. Purpose. This chapter outlines the execution of the Department of Defense (DoD) Fire and Emergency Services Certification Program (F&ESCP) in accordance with DoD 6055.06M, DoD Fire and Emergency Services Certification Program. The purpose of the F&ESCP is to enhance the training program to improve firefighter performance reliability and strengthen the professionalism of all DoD Fire and Emergency Services (F&ES) personnel.

1.2. Responsibilities. In accordance with (IAW) DoD Instruction 6055.06, DoD Fire and Emergency Services (F&ES) Program, the Air Force Civil Engineer Center, Fire Emergency Services Division (AFCEC/CXF), hereafter referred to as the Administration Center, is designated as the lead agency responsible for implementation of the DoD F&ESCP.

1.3. DoD Certification Eligibility. IAW DoD 6055.06M, eligibility for DoD certification under the F&ESCP for the National Fire Protection Association (NFPA) 1000-series certifications is restricted to Military and DoD civilian (to include contracted and foreign nationals) Fire and Emergency Services personnel, who have successfully passed the written and performance evaluations. Eligibility for the NFPA 472, Hazardous Material courses is open to all U.S. government personnel.

1.4. Program Management. The Administration Center is dual accredited by the International Fire Service Accreditation Congress (IFSAC) and National Board on Fire Service Professional Qualifications (Pro Board), and as the accredited entity, issues IFSAC and Pro Board sealed DoD certificates for successfully completing certification requirements through the DoD venues identified in Chapter 2 IAW the policies and procedures contained within this document.

1.4.1. DoD certifications with an IFSAC and/or Pro Board seal and number do not expire. The DoD policy is once certified, always certified. There may or may not be training requirements to maintain proficiency in certifications based on the local authority having jurisdiction’s (AHJ’s) determination. Other certifications that have an expiration date will have the date printed on the certificate. Refer to the specific chapter within this document for more information specific to the certification.

1.6. No-Notice Quality Assurance Evaluations. In accordance with DoD 6055.06M, the Administration Center, members of the Fire and Emergency Services Working Group (F&ESWG) and representatives from IFSAC and Pro Board retain the right to visit and periodically inspect test sites to ensure compliance.

1.7. Help Desk and Support. For assistance with various agencies or programs identified within this document used to deliver the DoD F&ESCP, use the appropriate helpdesk as identified below:

CHAPTER 2 Course Enrollment and Training Venues

2.1. Purpose. This chapter identifies the various methods and venues for receiving DoD certifications. There are two main methods by which DoD employees may obtain DoD certifications under the F&ESCP. DoD employees can complete distance learning or in-residence courses at one of the DoD venues identified in this chapter. For DoD employees who received IFSAC and/or Pro-Board certification through non-DoD courses, DoD certification is not granted. Refer to Chapter 6 for more information on the F&ESCP reciprocity provision.

2.2. General Information. The predominant method of certification used for completing certification requirements is completing a Certification Course (also known as Career Development Course (CDC)) through formal enrollment with Air University (AU). The other means of obtaining certification is to attend and complete accredited in-residence courses at the DoD venues identified in paragraph 2.4.

2.3. Course Eligibility. Any federal employee, military, meeting the eligibility requirements in the AU Course Catalog may enroll in the F&ES certification course and complete the knowledge portion of the program following the AU guidelines. Note: enrollment in a course does not constitute a right to certification. Refer to paragraph 1.3 for certification eligibility.

2.3.1. There are no certification prerequisites for enrolling in courses through AU. However, students are limited to one (1) F&ES Certification Course enrollment at a time.

2.3.2. Eligibility requirements for DoD certification under the F&ESCP are identified in paragraph 1.3. DoD certification prerequisites are identified in Attachment 5.

2.3.3. The NFPA documents used to obtain accredited certification are typically updated every 5 years. In accordance with IFSAC by-laws and policy, certification cannot be issued for previous editions of accredited levels once the program updates to the current NFPA edition. Under current program policy, the DoD Administration Center cannot accept training/testing for certification purposes that is older than 5 years based on the typical updates performed.

2.4. Course Venues. All F&ES Certification Courses are available Total Force Virtual Learning Center (TF VLC). Students can access the TF VLC from any .mil, .gov, .edu, or .com domain. Refer to Attachment 1 for locations for all F&ES certification courses.

2.4.1. Students must be formally enrolled through AU prior to completing the course to receive credit for certification. Course completion certificates dated before the official enrollment date as listed with AU will not be accepted. See paragraph 2.6 for enrollment information.

2.4.3. All F&ES related courses were removed from the AFCEC VLC on 1 January 2016.

2.4.4. All FES Certification Courses on the GoLearn and AU derivatives of the Advanced Distributed Learning Service (ADLS) were removed 18 Mar 2016.

2.5. Course Mediums. The certification courses come in either a paper-based or web-based format dependent upon the certification title.

2.5.1. The use of downloadable (CD-ROM) based multi-media courses or courses on the Navy Marine Corps Intranet (NMCI) are no longer valid and do not meet the course requirements to obtain DoD certification. Course completion certificates from these venues will not be accepted.

2.5.2. Paper-based certification courses. The letter “N” is used in the course number to indicate a paper-based course. These course venues provide the student with a small web-based interface from which the Instructor Guide Sheets (IGS) and Performance Test Checklist (PTC) portable document format (*.pdf) are accessed and downloaded. Once the student accesses the course through the LMS and completes the Tutorial module, they will have access to the IGS and PTC modules which provide additional guidance on completing the paper-based course. (There is no course-specific content within the web-based interface). The final module of each course is the Certification Course Review Exercise (CCRE) for the student’s use in preparation for their End of Course (EOC) final exam. The CCRE is not the EOC. Refer to paragraph 2.7 for more information on course completion requirements. Courses requiring formal enrollment with AU under this format include:

2.5.3. The web-based certification courses are interactive courses completed in an online environment.

Once the student is enrolled in the course through AU, they can access the course and complete the computer-based courseware online. The web-based courses contain all instructional material and references necessary to complete the course and successfully pass the end of course exam. The Performance Test Checklists are located under the “Options” menu within the interactive courseware. The letter “W” is used in the course number to indicate a “Web-Based” course. The final module of each course is the CCRE for the student’s use in preparation for their EOC final exam. The CCRE is not the EOC. Refer to paragraph 2.7 for more information on course completion requirements. Courses requiring formal enrollment with AU under this format include:

2.5.3.1 Hazardous Materials Awareness is a web-based certification course that does not require formal enrollment.

2.5.3.2. The Hazardous Materials Operations- Mission Specific Competencies are web-based courses embedded within the HazMat Operations course. Completion of the individual MSCs do not require formal enrollment in the HazMat Operations course. Refer to Chapter 7 for more information on completing the HazMat Operations- MSC certification requirements.

2.5.4. Certification courses may have more than one version based on updates and changes to the applicable NFPA standard. These versions are differentiated from one another by an edit code following the course format in the course numbers. Ensure candidates take the correct version of the course and test in which they are enrolled. Refer to the following example for identifying course numbers and versions.

2.6. Course Enrollment. Formal course enrollment is only applicable to the courses identified in section

2.5. Courses not listed in section 2.5, do not require formal enrollment.

2.6.1. For USAF military, the course enrollment process is accomplished through the Military Personnel Data System (MILPDS).

2.6.2. For DoD civilians and other Service Component’s military, course enrollment is accomplished using the AU Form 23 located at the DoD F&ESCP website in the “Program News/Updates” section.

2.6.2.1. A fillable AU Form 23 is also included in this document. Open an electronic copy of this procedural guide.

2.6.2.1.2. Access the form in the navigation pane on the left side of the window by clicking the paperclip, and open the "AU Form 23 – Enrollment Worksheet" file.

2.6.2.1.3. The information required to send the form is located at the top of the form.

2.6.2.2. When emailing the form to AU, it must be encrypted to protect the personal identifiable information (PII). To send encrypted emails to the AU Registrar’s mailbox, individuals will need to copy the public certificates. To obtain the AU org box certificates:

2.6.2.2.1. Clear all cached email addresses previously used.

2.6.2.2.2. Open an electronic copy of this procedural guide found on the DoD F&ESCP website in the “Program News/Updates” section.

2.6.2.2.3. View the attached documents in the navigation pane on the left side of the window by clicking the paperclip, and open the "CFRR_STUDENT_SERVICES_AU.vcf" file

2.6.2.2.4. Click the left icon "Save & Close" to save the contact card in your email contacts

2.6.2.2.5. Locate and open the contact card in your contacts

2.6.2.2.6. Send an encrypted email to the AU Student Services using the "E-mail" icon located above the picture placeholder. Ensure your sign and encrypt the message prior to sending.

2.6.3. Allow up to 30 days for the AU Registrar to process the request which results in the employee being added into the Course Development and Student Administration/Registrar System (CDSAR) database located at https://www.my.af.mil/aurepmprod/auportal/. Refer to paragraph 5.6 for CDSAR access instructions.

2.6.4. Once the student is added to the CDSAR database, AU Support Services will forward the enrollment notification to the original requestor. If after 30 days, the registration is not visible in CDSAR, then contact the AU Registrar at DSN 493-8128 or commercial 334-953-8128, option #8.

2.6.5. Once the enrollment confirmation has been received, or validated in CDSAR, the student will log into the TF VLC to complete the course for which they are enrolled. Note: Paper-based and web-based courses are advertised as “online” but only the web-based courses require course work to be completed in the online environment.

2.6.6. Student Access to Courses on the TF VLC. Access into the TF VLC begins by registering for the site.

2.6.6.1. On the TF VLC homepage, click on the “Register Here” link to begin the registration process. Enter your official email address and click the “Request New Account” button. Note: The email used must be from a .mil, .gov, or .edu domain to validate student authorization for access to the courses.

2.6.6.2. The system will send a verification email to the email address provided. Click on the validation link within the email. Once clicked, the student will be returned to the TF VLC where they will input their PIN (6 to 10 digit number) and fill out their profile information.

2.6.6.3. After completing the profile information students will have full access to the website to include open access to all available courses.

WARNING: Students must be formally enrolled in the course with Air University prior to completing the course to receive credit for the coursework. Submitting course completion certificates dated prior

to the official enrollment date as listed in CDSAR will require the student to reset the course and complete the coursework to obtain a course completion certificate dated after the official enrollment

date. This does not apply to paper-based courses.

2.7. AU’s Certification Course Completion Requirements. AU will not record course completion and issue scorecards (commonly referred to Extension Course Institute (ECI) score cards) until the student passes the official EOC final exam at an approved Test Control Facility (TCF) and the scores are annotated in CDSAR.

Note: The Certification Course Review Exercise (CCRE) in the courses is NOT the EOC.

2.7.1. To request information on establishing a TCF or locating the nearest TCF, submit the request to the AU Registrar using the AU support desk information in paragraph 1.7.

To maintain IFSAC and Pro Board accreditation, Installation Fire Departments are not authorized to be an approved TCF.

2.7.2 Failure to complete any certification course taken through AU by either not passing the EOC final exam after two (2) attempts or being dis-enrolled for other reasons WILL result in an automatic six (6)

month penalty from the date of disenrollment where the candidate may not formally enroll in another FES certification course. All requests must have a written justification for the waiver signed by the Fire Chief/Flight Chief. AU will work the official waiver request through the Administration Center for approval/disapproval.

2.8. Schools and Academies. The Administration Center maintains IFSAC and Pro Board accreditation for all certification testing material and issues the DoD certifications upon a student’s successful completion of the certification test and performance test at the DoD Fire Academy, North Carolina Regional Training Site (includes OCONUS Silver Flag Training Sites), and the Army CBRN School.

2.8.1. The Louis F. Garland Fire Academy (LFGFA), also known as the DoD Fire Academy, located at Goodfellow AFB. TX, provides firefighting certification training using in-residence courses, distance learning (DL) courses and mobile traveling teams (MTT). The curriculum includes courses ranging from the basic apprentice course to the more advanced and technical courses. Contact the Fire Protection Training Manager with the 312 TRS/XPM at DSN 312-477-4840 or commercially at (325) 654-4840 for more information on requesting participation in one of the courses or requesting a MTT for one of the advanced courses identified in paragraph 2.8.1.2.

2.8.1.1. Students successfully completing the Apprentice Firefighter Course, course number X3ABR3E731 0A1A, graduate with the following DoD certifications:

2.8.1.2. Students who successfully complete the advanced courses will receive DoD certifications. The advanced courses include: (“*” course also provided by MTT and/or Distance Learning)

2.8.2. The North Carolina Regional Training Site (NC RTS) located in Stanly County, NC is accredited under the F&ESCP and provides National Guard firefighters certification training using in-residence courses only. The NC RTS also serves as the administrative hub for and curricula and testing material support to the Silver Flag training sites at Ramstein Air Base, Germany (USAFE) and Anderson AFB, Guam (PACAF). The levels available include: Rescue Technician, Rope Rescue I and II, and Confined Space Rescue I and II.

2.8.2.1. National Guard Fire Departments: Contact the 145th CES/CEF Fire Chief at DSN 231-4206 or commercially at 704-391-4206 for more information on requesting participation in one of the in-residence courses.

2.8.2.2. OCONUS DoD Fire Departments: Contact the respective USAFE and PACAF Detachment Fire Chief’s office for class availability.

2.8.3. The U.S. Army Chemical, Biological, Radiological and Nuclear (CBRN) School is accredited under the F&ESCP. Students who successfully complete the Hazardous Materials courses with a test score of 80% or higher through this school will receive DoD certification for each specified levels. The Hazmat levels available include:

HAZMAT Awareness HAZMAT Operations Core HAZMAT Operations MSC – Personal Protective Equipment (PPE) HAZMAT Operations MSC – Mass Decontamination HAZMAT Technician HAZMAT Incident Commander

2.9. Other credentialing and/or F&ES related certifications are available through courses developed and/or maintained by the Administration Center. These courses do not require formal enrollment with AU and are detailed further in Chapters 7, 8, and 10.

CHAPTER 3 Certification Testing Program Policies

3.1. Purpose. This chapter outlines the policies and procedures for certification testing as it applies to certification tests located on the AFCEC VLC. This chapter does not apply to the Certification Course Review Exercise (CCRE) contained within the certifications courses on the TF VLC or the End of Course exams provided through AU.

3.2. Responsibilities. All program managers and test proctors are responsible for adhering to the guidelines contained within this guide. Each entity that manages certification testing must have at least one complete copy of this document locally available for use by all members of the organization.

3.3. General Information: Certification tests (exams) for courses that do not require official enrollment or testing with AU are located on the AFCEC VLC. Current tests on the AFCEC VLC include:

3.3.1. Access into the AFCEC VLC derivative begins with GoLearn (commonly known as ADLS) Learning Management System (LMS) located at https://golearn.adls.af.mil. From here students gain access to the AFCEC VLC derivative.

3.3.1.1. Access GoLearn and create an account following the on screen instructions. Login ID and Password are initially used, however, once established, access is Common Access Card (CAC) enabled. It is highly recommended Login ID and password is retained for future use. Students must access the LMS using a .mil computer.

3.3.1.2. To add the AFCEC VLC derivative, navigate to the ADLS gateway using the same steps in above and click on the AFCEC icon (not the “course listing” link) from the section labeled “You are NOT currently associated with Derivatives:”. This will prompt the student through the process of associating the AFCEC VLC derivative with their account. After successfully completing the association, the AFCEC icon will be shown under the “You are currently associated with Derivatives:” section of the ADLS Gateway and the student will be able to use the AFCEC shield to access the course list for FES certification courses.

3.4. Program Management: The certification testing program shall be maintained like other official testing programs. Refer to the respective service component regulations for additional requirements.

3.4.1. To request your agency’s access to the password protecting each test on the AFCEC VLC, a Program Manger Appointment Letter (Attachment 2) must be signed by the Installation Fire Chief or Flight Chief

and on file with the Administration Center. Completed appointment letters must be sent to the Administration Center via email at [email protected].

3.4.2. The program manager letter will remain current until changed by the local authority having jurisdiction unless revalidation is specifically requested by the Administration Center.

3.4.3. No more than five (5) managers are authorized: one (1) primary and four (4) alternates

3.4.4. A fillable version of the Program Manger Appointment letter is included in the electronic version of this document via the navigation pane on the left side of the window.

3.4.5. Clicking the paperclip and opening the "Attachment 2 Program Manger Appointment Letter.pdf" file provides access to the fillable document.

3.5. Program Managers/Alternate Program Managers. Program managers are responsible for proctoring tests, test administration, program security, and compliance with these instructions. Specifically, they are responsible to:

3.5.1. Monitor and proctor student testing to ensure students:

3.5.1.1. Do not have access to reference material during the test.

3.5.1.2. Do not receive any assistance during the test.

3.5.1.3. Are given enough time to take a test.

3.5.1.4. Do not copy or duplicate test material.

3.5.1.5. Do not take a certification test for any certification they are not officially attempting to obtain.

3.5.2. Report any test compromise to the Administration Center immediately upon discovery.

3.5.3. Send challenge questions to the Administration Center. (See paragraph 3.9.)

3.6. Testing Area. Testing must be conducted in a proctored environment by an individual identified in the current appointment letter on file with the DoD Administration Center. Violations of this policy may result in administrative action under the Service components official test compromise and security policies. Note: Access to the AFCEC VLC is limited to computers on a .mil domain.

3.6.1. The space and physical facilities selected for computer-based testing must be conducive to continuous surveillance by a test proctor during testing. Test proctors must ensure conditions are favorable for testing such as proper lighting, temperature, and freedom from distractions.

3.6.2. The program manager/proctor must be able to monitor all computers at one time while official testing is in progress. A centralized testing area is recommended for controlling the program.

3.7.1. After clicking the hyperlink for the appropriate test, the password screen will appear.

3.7.2. Program managers will input the password for the test on the “Input Password” screen. After inputting the password and clicking submit, a screen will indicate if the password was correct. If so, click "Continue" to begin the test for the student.

3.7.3. Students must read the information screen. In particular, this screen will display test challenge instructions. More information on test challenges is in paragraph 3.9.

3.8. Test Completion. Upon completion of the test, the student will be allowed to review the entire test. Information provided to the student differs based on the final score of the test.

3.8.1. If the student obtained a passing score of 80% or higher, information will be provided as to whether the question was answered correctly or not and the correct answer will be identified.

3.8.2. If the student fails to obtain a passing score of 80%, information will be provided on whether the student answered the question correctly or not. The correct answer will not be identified.

3.8.3. The test summary page (found in the student’s course transcripts) only provides a total score, not a breakdown of subjects. Students should use the test review process before they log out of the test to determine the areas requiring more study.

3.9. Challenging a Question. While taking a test, a student may find a question that he/she believes to be erroneous or invalid for some reason (i.e. typo, spelling mistake, missing graphic, ambiguous question, etc.). If a student has a concern about the accuracy or clarity of a question, they must challenge the question during the test session.

3.9.1. Test challenge instructions are provided on the opening screen of each test and include the following information:

3.9.1.1. The three (3) digit code located at the end of each question.

3.9.1.2. The entire question to include the answer selected.

3.9.1.3. A brief description of the issue with the question and/or answers. .

3.9.2. All test challenges must be sent via email to the DoD Administration Center at [email protected].

3.10. Program Errors: If a program error occurs, write down the information exactly as it appears on the computer screen or take a screenshot of the error and paste it in a Word document to email to the DoD Administration Center at [email protected].

3.11. Reference Material Use. Foreign national employees are allowed to use a host nation dictionary. No other items are allowed in the testing area to include, but not limited to: electronic devices (such as iPods, tablets, Smartphone’s, Smartwatches etc.), reference books, or other written materials.

3.12. Student’s Testing Responsibilities. Students should be prepared to sit through a test until the test is completed.

3.12.1. Students will not compromise any test by discussing or copying test questions. Violations of this policy may result in administrative action under the service components official test compromise and security policies.

3.12.2. If a student fails a certification test, the student will not be given a retest until the next day after the student has had time to review the course material.

3.13. Updates and Revisions. The DoD Administration Center will provide all updates to the certification tests which will require no action on the part of the program managers.

CHAPTER 4 Performance Testing

4.1. Performance Testing: Every certification course has two main components: knowledge and performance testing. The knowledge portion of the process is measured using the certification tests discussed in Chapter 2 and completion of the official EOC exams. The performance testing must be accomplished IAW DoD Manual 6055.06-M; DoD Fire and Emergency Services Certification System.

4.1.1. Students must successfully pass the required knowledge test(s) before the official performance tests can be administered.

4.1.2. Performance tests are administered at the local level.

4.1.3. Performance tests follow specific guidelines, checklists, and procedures and are designed to measure a student's competence at completing specific tasks that are measured against predetermined standards.

4.1.4. Specific performance tests procedures and evaluator requirements are outlined in the Performance Test Checklists included within the course. They are found under the “Options” menu within the interactive courseware. Some performance tests may have additional requirements discussed later in this document under the specific certification levels.

4.2. Performance Test 10-Day Notice. Every performance test and performance test re-evaluation conducted for an IFSAC/Pro-Board certification requires a 10-day advance notice to be made to the Administration Center. The performance test reference number must be included in the cover letter or sent in as an attachment with the certification request. Failure to include this information may require re-accomplishment of the required performance tests.

4.2.1. The 10-day notification must be submitted using the Performance Test Notification application located on the DoD F&ESCP website.

4.2.2. To generate a new performance test number, complete all fields under “Performance Test Notification” screen and click on the “Submit Test Information”

4.2.2. Performance test notifications will not be accepted via telephone.

4.3. In accordance with DoDM 6055.06, performance test evaluators cannot be the same individuals who provided the candidate’s training and must be certified at least one level above the level being evaluated with a minimum of one-year on-the-job experience in the specific level being evaluated.

4.3.1. For Firefighter I/ II, Airport Firefighter, Marine Firefighter, all Driver-Operators, Wildland Fire Fighter I/II and Confined Space Rescue, one level higher is Fire Officer I.

4.3.2 For Hazardous Materials Operations and all Mission Specific Competencies, one level higher is Hazardous Materials Technician.

4.3.3. For Fire and Life Safety Educator I, one level higher is Fire Instructor II and Fire Inspector II.

4.3.4. For Fire and Life Safety Educator II, one level higher is Fire Instructor II and Fire Inspector III.

4.3.5. For Plans Examiner I, one level higher is Fire Inspector III.

4.3.6. For Incident Safety Officer, one level higher is Fire Officer II.

4.3.7. For Health and Safety Officer, one level higher is Fire Officer III.

4.3.8. For Public Telecommunicator I and II, HazMat Technician, and HazMat Incident Commander, evaluators must be certified for the respective level with at least one year on-the-job experience in the level being evaluated.

4.3.9. The Hazmat Officer Part 1 performance test evaluations must be proctored by the Assistant Chief for Training, the Assistant Chief of HazMat and Safety, the Deputy Fire Chief or the Fire Chief.

4.3.10. For HazMat Safety Officer, one level higher is HazMat Technician and Fire Officer III.

4.3.11. The Fire Inspector III, Fire Instructor III, Fire Officer III, Fire Officer IV, and HazMat Officer (Part 2) performance test are evaluated by the Administration Center after the candidate completes their required projects locally. Refer to the Performance Test Checklists for items which are sent to the Administration Center for evaluation.

4.4. Exceptions to Evaluator Certification Requirements. New courses and/or certifications may require exceptions to the evaluator’s certifications or the one-year on-the-job experience in the specific level being evaluated. The following exceptions are in effect through the dates listed.

4.4.1. HazMat Safety Officer. 1 October 2016 – 31 September 2017, certification in HazMat Safety Officer is not required for the evaluator. 1 October 2017 – 31 September 2018, evaluators must be certified in the specific level, however, the one-year on-the-job experience in the specific level being evaluated is not required for the evaluator. 1 October 2018, there are no exceptions to the requirement under paragraph 4.3.

4.4.2. Marine Fire Fighter. 1 January 2017 – 31 December 2017, evaluators must be certified in the specific level, however, the one-year on-the-job experience in the specific level being evaluated is not required for the evaluator. 1 January 2018, there are no exceptions to the requirement under paragraph 4.3.

4.4.3. Fire and Life Safety Educator I/II. 1 May 2017 – 30 April 2018, evaluators must be certified in the specific level, however, the one-year on-the-job experience in the specific level being evaluated is not required for the evaluator. 1 May 2018, there are no exceptions to the requirement under paragraph 4.3.

CHAPTER 5 Certification Package Submissions

5.1. Purpose. The purpose of this chapter is to provide instructions on how to submit a package to the DoD Administration Center and use of the Mail Tracking System application located on the DoD F&ESCP website.

5.2. Mail Tracking Number. The Mail Tracking System allows customers to obtain a unique identification number for use in submitting packages to the DoD Administration Center. Prior to the package being sent to the DoD Administration Center, a mail tracking number must be assigned to the package. This number is also required on the cover letter accompanying the package. The process starts with the end user.

5.2.1 To generate a new mail tracking number for a package, complete all fields under “Request Tracking Number” and click on the “Submit Request Information”

5.2.2. A package for a single certification level that contains more than one name (i.e. 10 people receiving Fire Instructor I) should be submitted under that (Fire Instructor I) certification level.

5.2.3. The “Multiple” level must be used if the package contains certification requests for more than one certification level (i.e. HazMat Awareness, Inspector I and Fire Officer I) regardless of the number of individuals identified.

5.2.4. It is imperative you annotate the Mail Tracking ID number assigned. It is required on the cover letter submitted. Additionally, you can print a copy of the information for your records using the print function within your browser.

5.3. Assistance. For problems or assistance using this application, send an email message to the DoD Administration Center at [email protected].

5.4. Certification Package Submission Requirements and Format. After the package has been entered into the Mail Tracking System, a complete package should be submitted to the DoD Administration Center via mail, fax, email (preferred) or file transfer protocols (FTP) using the information in paragraph 1.5.

5.4.1. To create an electronic package for email, scan all documents necessary into one (1) *.pdf file per cover letter and send the file to the email address identified in paragraph 1.5. NO OTHER FILE FORMATS WILL BE ACCEPTED.

5.4.2. Email Packages. Certification packages containing Personal Identifiable Information (PII) sent to [email protected] must be encrypted IAW DoD policy. To send encrypted emails to the AFCEC/CXF DOD Certification mailbox, individuals will need to copy the public encryption certificates. To obtain the certificates (if not already done):

5.4.2.1. Clear all cached email addresses previously used.

5.4.2.2. Open an electronic copy of this procedural guide.

5.4.2.3. View the attached documents in the navigation pane on the left side of the window by clicking the paperclip, and open the “DOD CERTIFICIATION_AFCEC CXF.vcf" file.

5.4.2.4. Click the left icon "Save & Close" to save the contact card in your email contacts.

5.4.2.5. Locate and open the contact card in your contacts.

5.4.2.6. Send an encrypted email to the DoD Administration Center using the "E-mail" icon located above the picture placeholder. Ensure you sign and encrypt the message prior to sending.

5.4.3. File Transfer Protocol (FTP) Packages. If the document file size is too large to be sent via email (more than 8 MB) or encryption cannot be utilized, use the U.S. Army’s Safe Access File Exchange (SAFE) application located at https://safe.amrdec.army.mil.

5.4.4. Faxed Packages. Faxes are still accepted, however, the internet fax site is processed bi-weekly. When faxing, submit packages as a single fax to the fax number identified in paragraph 1.5.

5.5. Certification Package Requirements.

5.5.1. Cover letter. An example of the required cover letter and other common documents necessary for inclusion in the certification package submissions is provided in Attachment 3 and 4. Ensure you edit the cover letter to meet the certification level requested.

5.5.1.1. Under the program’s IFSAC and Pro Board accreditation, the signature authority for the cover letter is the installation Fire Chief (Flight Chief) or higher and cannot be delegated.

5.5.1.2. If the Fire Chief is not available for signature for extended absences of 30 days or more, the Deputy Fire Chief may sign as the “Acting Fire Chief.”

5.5.1.3. For organizations without a Deputy Fire Chief, the signature authority is the Fire Chief’s (Flight Chief’s) supervisor. No other signatures will be accepted.

5.5.2. Course Completion Certificate. For all web-based courses, a course completion certificate must be included in package submissions. To maintain accreditation the Course Completion Certificate must be dated after the official enrollment date listed in CDSAR. To capture this information:

5.5.2.1. While logged into the GoLearn LMS, click on “My Transcripts”. Au transcripts are still available for course work completed prior to the removal of the certification courses on 1 March 2016.

5.5.2.2. Locate the course and click on “Certificate” link in the “Title” column.

5.5.2.3. Once the window opens, use the browser’s print function to print the page to a document which can be used as part of the package submission such as Adobe PDF.

5.5.2.4. On the Total Force VLC, certificates are available by clicking on “Get Certificate” under the students transcripts.

5.5.3. Depending on the certification, additional documentation may be required. Refer to Attachment 5 for a complete list of documents to be included in the package submissions and Chapters 7, 8, and 10 for more specific information.

5.6. Access to CDSAR. To obtain the CDSAR print out of the test results, training manager may request access to CDSAR located on the AU Portal which resides on the Air Force Portal. Training managers may also access student history and current enrollments using CDSAR.

5.6.1. Air Force Portal Access. For those who DO NOT have access to the Air Force Portal, users will need to register for an AF Portal account. Registering for the AF Portal is a two-part process. In the first part, you will self-register for an AF Portal ID. Your ID and temporary password will be emailed to you. In the second part, you will login to the AF Portal with your ID and password and complete your AF Portal Profile.

5.6.1.1. Visit www.my.af.mil.

5.6.1.2. Click on the "Register for a new AF Portal Account w/ your CAC" link.

5.6.1.3. Click OK on the warning box that appears.

5.6.1.4. Complete all required fields as indicated in bold with an asterisk (*).

5.6.1.5. For Non-Air Force personnel, select "Other" for the last 4 fields: Service Type, Org Unit Type, Top-Level Org, and Base/Location).

5.6.1.6. Click the 'Next' button.

5.6.1.7. Verify that the information provided is correct and click 'Submit'. To make a change, click the 'Back' button, make the corrections, and click 'Next'.

5.6.1.8. Your ID and temporary password will be emailed to you. You must use this ID/Password to complete the registration and complete your profile.

5.6.1.9. In the email you receive, you may be asked to submit a signed DD Form 2875, System Authorization Access Request (SAAR) to complete the registration process. To view the DD Form 2875, open the navigation pane on the left side of the window by clicking the paperclip, and then open the "DD Form 2875 - SAAR" file.

5.6.1.10. Once your AF Portal Profile is complete, refer to the instructions in 5.6.2 for access to the CDSAR application on the AU Portal.

5.6.2. CDSAR Access on the AU Portal. For those who already have an AF Portal account:

5.6.2.1. Visit https://www.my.af.mil/aurepmprod/auportal.

5.6.2.2. Click on the "Log In" button on the AF Portal page.

5.6.2.3. Select a certificate and click on the "OK" button.

5.6.2.4. Click on the "Click here to login or create a new account" button.

5.6.2.5. Click on the "Log In" button.

5.6.2.6. Hover your mouse over the "AU Systems" link on the left side of the screen and click on the "CDSAR" link to the right of it.

5.6.3. Follow the directions and information in the CDSAR Single-Sign-On (SSO) User Guide attached to this document. To view the CDSAR SSO User Guide, open the navigation pane on the left side of the window by clicking the paperclip, and then open the "CDSAR SSO User Guide" file.

5.6.3. Should you have further issues with application, submit a help desk ticket to the Field Assistance Service using the "Contact the Help Desk" link on the AF Portal Login page for assistance.

5.7. Certification Package Processing. The Administration Center will verify certification packages submitted meet the requirements listed in this guide (or as amended through future policy letters).

5.7.1. Certification packages, which meet all the requirements, will be processed and the individual’s certificates will available on the DoD F&ESCP website within 7-10 business days.

5.7.2. Certification packages which do not meet the requirements will be returned for corrective action.

5.7.3. Digitally signed documents are accepted.

5.8. Website Updates. DoD certificates are available for viewing and/or printing on the DoD F&ESCP website after the next system update. The website is normally updated nightly, Monday through Friday. As with any communications system, technical issues may delay the update; however, once communications are restored, the update will occur. The date of the last update is provided on the web page for your reference.

CHAPTER 6 Reciprocity

6.1. Reciprocity Provision. IAW DoD 6055.06M, the DoD recognizes and accepts certifications accredited by any IFSAC or Pro-Board entity as being equivalent to the DoD certification for comparable titled levels. Reciprocity is not authorized and will not be accepted for certification levels which the DoD is not accredited.

6.2. Under this reciprocity provision, eligible employees, as identified in paragraph 1.3, may progress within the F&ESCP using external (non-DoD) IFSAC and/or Pro Board accredited certifications to meet the prerequisites as identified in Attachment 5 without retesting or recertifying the lower level certifications.

6.3. The Administration Center is only authorized to issue a DoD certificate with IFSAC seal for successfully completing certification courses acquired from DoD venues as identified in Chapter 2. DoD certificates will not be issued for external IFSAC and/or Pro Board certification; however, information pertaining to external accredited certifications, required by DoD 6055.06-M, may be maintained on the DoD F&ESCP website.

6.3.2. The ability to view/print a DoD certificate using the external IFSAC and/or Pro Board certificate information is not available. Individuals must maintain the original IFSAC and/or Pro Board certificate from the issuing entity.

6.4. To request the addition of external IFSAC and/or Pro Board accredited certifications to the DoD Look-up System, submit the following information to the Administration Center.

6.4.1. A completed letter (Attachment 6) signed by the fire chief/flight chief containing

6.4.1.1. Individual’s full name and social security number.

6.4.1.2. Certification information identified in paragraph 6.3.1.1 - 6.3.1.2 above.

6.4.1.3. A copy of the original IFSAC and/or Pro Board certificates for each level being requested (to include all prerequisites not already in the DoD database system). All certificates must have a legible IFSAC and/or Pro Board seal and number on them. Training certificates or certificates issued by the accredited entity under their reciprocity provision (secondary certificates) will not be accepted.

6.4.1.4. Follow the guidance in Chapter 5 and Attachment 5 for package specific documentation requirements.

6.4.1.5. For Air Force only: IAW AFI 32-2001, receiving certifications from non-DoD entities requires approval from the Detachment Fire Chief. Reciprocity request packages must contain the Detachment Fire Chief’s approval letter to be processed. Reciprocity request submitted without the approval letter will be returned to the Detachment Chief with no further action taken by the Administration Center.

6.4.2. There is no reciprocity for non-IFSAC or non-Pro Board certifications. The Administration Center will validate certificates submitted with the issuing organization for authenticity.

CHAPTER 7 Hazardous Materials Certifications

7.1. Purpose. This chapter outlines procedures for obtaining Hazardous Materials Awareness and those Mission Specific Competencies not included in HazMat Operations. The certification process described in Chapter 2, 3, 4, and 5 must be followed to meet the formal enrollment course completion and certification requirements for HazMat Operations Core with Personal Protective Equipment (PPE), HazMat Technician, HazMat Incident Commander, and HazMat Officer Courses.

7.2. Program Management. Deviations from the procedures as presented in this chapter will result in candidate’s application for certification to be delayed until this guidance is met.

7.2.1. The HazMat T-t-T’s are no longer authorized to conduct the academic portion of the HazMat courses; however, they should be utilized as performance test evaluators following the student’s completion of any of the web-based courses.

7.3. Hazardous Material Awareness. HazMat Awareness does not require formal enrollment through AU. This course is located on the TF VLC. Refer to Chapter 2 for access procedures.

7.3.1. To receive the DoD certification, the final exam must be witnessed (proctored) by the student’s supervisor or training manager. The proctor is responsible for monitoring the student testing to ensure students:

7.3.1.1. Do not have access to unauthorized reference material during the test. (Note: The Emergency Response Guidebook (ERG) is provided electronically within the test for questions requiring use of the reference.)

7.3.1.2. Do not receive any assistance during the test.

7.3.1.3. Are given enough time to take a test.

7.3.1.4. Do not copy or duplicate test material.

7.3.1.5. Use the “logout” function of the program to exit the course. Clicking the “x” located in the top-right corner of the screen will close the program but will not allow for the system to capture course completion.

7.3.2. The proctor will submit the student’s course completion certificate along with the signed cover letter (Attachment 3) to the DoD Administration Center using the information in paragraph 1.5 for requesting the DoD certification.

7.3.3. The DoD Certification certificate for HazMat Awareness will only be awarded upon the candidate’s initial completion of the web-based course. Subsequent course completions, such as those used to meet the annual refresher will not result in a new DoD Certification certificate being issued. The course completion certificate within the LMS should be used to provide verification of annual training completion if necessary.

7.4. HazMat Operations - Mission Specific Competencies (MSCs). DoD employees who are already HazMat Operations certified and want (or need) to specialize in one or more of the HazMat Operations MSCs can access and complete the applicable MSC using the TF VLC without formal enrollment in the

course through AU. These levels are not required to be completed if the candidate is continuing to the HazMat Technician level.

NOTE: For certification to Firefighter I, the MSCs PPE and PC are not required if the candidate is certified to Hazmat Technician prior to completing Firefighter I.

7.4.1. DoD HazMat Awareness and Operations certifications are prerequisites for all MSC certifications. The Personal Protective Equipment (PPE) MSC has been incorporated into the HazMat Operations Core with PPE course (47202W) and is not available as a stand-alone MSC. The PPE MSC is a required prerequisite for the other seven (7) MSCs.

7.4.2. DoD employees may complete any of the additional MSCs based on the tasks and duties they are expected to perform. Course material for the MSC’s is available as stand-alone modules included in the HazMat Operations Core with PPE course and include:

7.4.3.2. Pass the applicable MSC final exam on the AFCEC VLC. This test must be proctored in accordance with the guidance in Chapter 3 and completed prior to the candidate completing the required performance tests.

7.4.3.3. Pass the applicable performance evaluations IAW the procedures outlined in Chapter 4. Evaluators for the MSC performance test must be certified to the HazMat Technician level.

CHAPTER 8 Emergency Medical Responder

8.1. Purpose. This chapter explains the policies and procedures for completing the DoD’s Emergency Medical Responder (EMR) certification and how to obtain the prerequisite Cardio Pulmonary Resuscitation (CPR). The DoD EMR course is not mandatory, however, firefighters must maintain EMR (or higher) and CPR to meet their Firefighter and Fire Officer certifications.

8.2. Program Management. The guidance in this chapter only applies when the DoD EMR course is utilized to meet the requirements of NFPA 1001 and 1710. Anyone failing to comply with the guidance as it pertains to these specific courses will not receive the DoD certificate.

8.3. General Information. The Emergency Medical Responder course is a web-based training program that provides users with the knowledge and skills necessary to successfully perform initial medical care as the first person arriving on the scene. The EMR course meets the latest Department of Transportation (DoT) National Standard Curriculum.

8.4. Cardio Pulmonary Resuscitation (CPR). Due to the significant changes in 2015 CPR/AED guidelines, the DoD F&ESCP’s Health Care Provider CPR Course, May 2011 is no longer valid and cannot be used to meet CPR/AED certification.

8.4.1. To fulfill the CPR/AED requirements for firefighters, installations should look to their local AHA for the Health Care Provider CPR or American Red Cross for the CPR/AED for the Professional Rescuer.

8.4.2. For those installations that do not have a local AHA or ARC, the Uniformed Services University of the Health Sciences’ (USUHS) Military Training Network (MTN), located at https://www.usuhs.edu/mtn, provides free of charge a CPR/AED course which meet AHA guidelines.

8.4.3 The MTN also provides instructor course requirements and materials for local use.

8.5. Emergency Medical Responder (EMR). This course is the minimum level of emergency medical training necessary to meet NFPA 1001 and 1710 and has been approved through a memorandum of Understanding with the USAF’s Surgeon General’s office.

8.5.1. Individuals must be current and remain current in CPR.

8.5.2. Note: The requirement for this certification is superseded by an Emergency Medical Technician – Basic Life Support (EMT-BLS) or higher certification.

8.5.3. The “Emergency Medical Responder Course” is located on the TF VLC. Refer to Chapter 2 for access procedures.

8.5.4. The EMR course is considered complete when the student completes the courseware and successfully passes each training module, passes 100-question Emergency Medical Responder Assessment, and completes the performance test evaluation.

Note: The Emergency Medical Responder web-based training course can be completed individually or by using a group-teaching format. If the group format is used, only one copy of the student record is required for all of the students who complete the course using this method. The course completion details submitted for certification must be in the Instructor’s name.

8.6. EMR Test Administration. The student must print and provide a copy of their web-based course student record to the program manager. The program manager will confirm that the student(s) completed all of the required training modules and then schedule the student to take the official final exam using the Emergency Medical Responder Assessment located on the AFCEC VLC (Refer to Chapter 3).

The certification course review exercise (CCRE) at the end of the EMR course is NOT the final test.

8.6.1. With the exception of a dictionary, absolutely no study materials are allowed in the test room/area during testing.

8.6.2. Test Failures. If a student fails the test, the student should restudy before taking the test a second time. Refer to Chapter 3. If the student fails the test twice, the student must completely re-accomplish the multimedia training modules that require further study.

8.7. EMR Performance Test Administration. After passing the required final exam, each candidate must complete the required hands-on performance tests to meet the intent of the DoT National Standard Curriculum standards.

8.7.1. EMR is not IFSAC or Pro Board accredited. There is no requirement to provide the DoD Administration Center with the 10 -day notice of the performance testing

8.7.2. The “Performance Test Checklists” can be downloaded from the “Options” drop down menu within the course.

8.7.3. Performance testing must be conducted using the guidance in Chapter 4 and accomplished IAW the DoT National Standard Curriculum.

8.7.4. Anyone currently certified at the Emergency Medical Responder (or higher) and Fire Officer I may be used to evaluate the required performance tests. Exception : The AF Surgeon General has directed the minimum evaluator requirement for Air Force personnel testing to the EMR level is a certified (national or state) Emergency Medical Technician – Basic Life Support (EMT-BLS) or higher.

8.8. Certificate Package Requirements and Format. Complete packages should be submitted to the Administration Center via the information in paragraph 1.5. Follow the guidance in Chapter 5 and Attachment 5 for package documentation requirements. Examples of other required documents are provided in Attachment 7 and 8. Packages that do not follow this guidance will be returned for correction. The Administration Center will issue the DoD certificates for EMR which are only valid for two years from the issue date.

8.9. Re-certification Requirements: There is no “challenge” process for re-certification nor is there a process to accumulate CEUs applicable to recertification. To obtain up-to-date certificates after the expiration date, the student must re-accomplish the EMR courses as outlined in this chapter.

9.1. Purpose: This chapter provides information in regards to the policies and procedures for DoD installations requesting to utilize or host National Fire Academy (NFA) courses.

9.2. Update: The DoD Administration Center and the NFA are in the process of developing the means for the NFA to provide non-IFSAC and non-Pro Board accredited courses of instruction for those installations desiring NFA courses. There are no updates available for this edition of the procedural guide.

CHAPTER 10 Incident Command System (ICS) 300 and 400

10.1. Purpose. This chapter explains DoD policies and procedures for becoming certified in ICS 300 /400 to satisfy DoD credentialing requirements required by Homeland Security Presidential Directive (HSPD)-5 and numerous memorandums from the Secretary of Defense and the Secretary of Air Force. It also provides guidance to Air Force ICS 300/400 Train-the-Trainer instructors who teach these courses at Air Force installations.

10.2. General Information. These two courses are required as in-residence courses by the National Incident Management System (NIMS) Integration Center (NIC) because of the cross functional interaction and group dynamic scenario based exercise requirements. To meet the in-residence requirement, there are only two means of completing the in-residence requirement.

10.2.1. The DoD Fire Academy provides in-residence ICS 300/400 training. Contact the DoD Fire Academy at DSN 477-4864 or commercially at (325) 654-4864 to obtain information on attending the in-residence course. Fire Emergency Services and Emergency Management personnel graduating from the ICS 300/400 course graduate with T-t-T certifications.

10.2.2. The ICS 300/400 courses are based on group dynamics and taught using the PowerPoint slides that have been included on the Air Force Incident Command System SharePoint located at https://cs3.eis.af.mil/sites/21340/OO-MS-CE-16/default.aspx. . An ICS 300/400 T-t-T instructor should be able to complete the required training for ICS 300 within 16-24 hours and ICS 400 within 12-16 hours using the provided material.

10.3. Program Management. To maintain accountability and eliminate potential violations, the guidance contained in this chapter is mandatory. Anyone failing to comply with these instructions will not become credentialed in ICS 300/400.

10.3.1. To effectively and accurately teach the required course material, certified Air Force ICS 300/400 T-t-T instructors must use the support material provided on the ICS SharePoint.

10.3.2. Instructors are also encouraged to use the ICS 300/400 jeopardy game as an innovative way to conduct a course review exercise prior to having their students take the final exam for each course.

10.4. Course Prerequisites. This course does not require formal enrollment. The student must complete the required prerequisite courses before attending one of the ICS 300/400 in-residence training courses.

10.4.1. The prerequisites for ICS 300 are ICS 100, ICS 200, ICS 700 and ICS 800.

10.4.2. The prerequisites for ICS 400 are ICS 100, ICS 200, ICS 300, ICS 700 and ICS 800.

10.4.3. Air Force Personnel Only. The Air Force Emergency Management Program Course (AFEMPC) satisfies the ICS 100 and 700 prerequisites and the Air Force Emergency Response Operations: First and Emergency Responders Course (AERO FERC) satisfies the ICS 200 and 800 prerequisites as approved by the NIC. AFEMPC and AERO FERC are available on the AFCEC VLC. Refer to Chapter 3 for access procedures. Individuals who completed previous version of the AERO courses are not required to complete the newer courses.

10.5. Test Administration. All ICS 300/400 tests must be proctored by an ICS 300/400 T-t-T graduate. After completing the required training course, a final exam must be scheduled and proctored. The minimum passing score is 70%.

10.6. Performance Testing. There are no additional performance test requirements for the ICS 300/400 courses beyond the student participation in the group activities conducted at the end of each course module.

10.7. Test Failures. If a student fails a test, instructors should provide the student an overview of what questions they missed with a review of the course material and have them take the alternate version of the applicable course exam. It is imperative that anyone failing a test be given sufficient study time before taking the test a second time.

10.8. Record Keeping. It is the responsibility of the instructor(s) to maintain the student prerequisites, student attendance records and course test results. Instructors should keep one copy of the "student record" on file locally. It is recommended that the candidate keep a copy of the submitted package until the certificate has been issued and posted on the DoD F&ESCP web site.

10.9. Certificate Package Requirements and Format. Complete packages should be submitted to the Administration Center via mail, fax or email (preferred) contact information in paragraph 1.5. Follow the guidance in Chapter 5 and Attachment 5 for package’s specific documentation requirements. Note: An example of the cover letter for ICS 300/400 is provided in Attachment 9. Packages that do not follow this guidance will be returned for correction.

10.10. Reciprocity. The Administration Center is not authorized to issue certificates for non-DoD ICS 300/400 training courses. Follow the guidance provided in Chapter 6 for adding non-DoD 300/400 certificate information to the DoD database.

10.10.1. The DoD employee must provide proof of the applicable prerequisites level(s) as identified in Attachment 5 when submitting for addition to the DoD database.

10.10.2. Certificates must be issued by an entity that has been approved to deliver ICS 300/400 training by the National Emergency Responder Credentialing System.

Purpose

This document provides background information, instructions and screen shots for Single Sign

On (CAC) access to CDSAR. SSO access is required to ensure CDSAR meets AF network

security requirements to maintain its Certification and Accreditation status to connect to the AF

network.

Roles and Approval Authorities

CDSAR users gain access to Batches, Querys, Reports, Outputs and other functions/features

based on roles assigned. Below is the Approval Authority Structure which is based on the roles

of CDSAR users. Users will access CDSAR based on their assigned role(s). Request will be

approved by the next level of the approval authority structure.

AU Portal Login/Create Account

1. Do you have an AU Portal account?

a. If Yes, go to the Existing AU Portal User section.

b. If No, continue with step 2.

2. To access the AU Portal page, enter the following URL into your browser

https://auportal.maxwell.af.mil/auportal/welcome.AirUniversity.

3. To create an AU Portal account click, “Click here to login or create a new account”. This takes

you to the AU Portal login screen shown on next page.

4. Click the radio button for the right criteria which should be the default of ‘Common

Access Card.’

5. Next, click the ‘Create New Account’ button.

6. Enter your demographic information in the fields provided. The bold fields require you

to insert information.

7. Click the ‘Next’ button to complete AU Portatl account creation. Proceed to the

“Existing AU Portal User” instructions.

The welcome screen for CDSAR provides confirmation of CDSAR account creation for a new

user. To get role assignments approved and assigned by the Approval Authority applicable to

your functional area, complete the form with your supervisor’s name and email address.

4. Enter supervisors first and last name and email address.

5. Select the role(s) necessary to complete your duties. For a list of roles reference the

CDSAR User Roles table below.

Supervisor Approval

The supervisor is responsible for continuing the account/role approval process. This should

take place within a 24 hour window.

A Role Approval confirmation screen will appear. This screen confirms that the request of the

user has been approved by the supervisor and requires approval from the Final Approval

Authority (FAA).

Final Approval Authority (FAA)

The final approval authority will receive an email with instructions to provide access to CDSAR.

Follow the instruction found in the email. The instructions include a link to sign into CDSAR.

Follow the remaining instructions to enter the new user into CDSAR.

User Search screen generates a list of users based on search criteria ‘First Name’, ‘Last Name’

and ‘Account Locked’.

The FAA can sort the results list by clicking on First Name, Last Name or Email address headers

at the top of the Results list.

1. Locate the requesters name or email address.

a. If the name does not appear, enter it into the User Search

b. Be sure to type the name of the requester as it is seen in the email

2. Once the name of the requester is found, click the ‘Edit’ button

After clicking on the ‘Edit’ button, the User Edit screen will appear.

Initially the user edit screen will show the account locked and no roles selected. To unlock the

account, user roles must be selected.

3. Select the user role(s) indicated in the Approval email

a. Once a role(s) is selected, this action deselects the ‘Account Locked’ checkbox which

will grant the user access to assigned role and related functions/features.

4. Click the ‘Save’ button to apply the roles to the user

The following page shows selection of the role requested by the user.

A. PART I: The following information is provided by the user whenestablishing or modifying their USER ID.

(1) Name. The last name, first name, and middle initial of the user.

(2) Organization. The user's current organization (i.e. DISA, SDI, DoDand government agency or commercial firm).

(3) Office Symbol/Department. The office symbol within the currentorganization (i.e. SDI).

(4) Telephone Number/DSN. The Defense Switching Network (DSN)phone number of the user. If DSN is unavailable, indicate commercialnumber.

(5)Official E-mail Address. The user's official e-mail address.

(6) Job Title/Grade/Rank. The civilian job title (Example: SystemsAnalyst, GS-14, Pay Clerk, GS-5)/military rank (COL, United StatesArmy, CMSgt, USAF) or "CONT" if user is a contractor.

(7) Official Mailing Address. The user's official mailing address.

(8) Citizenship (US, Foreign National, or Other).

(9) Designation of Person (Military, Civilian, Contractor).

(10) IA Training and Awareness Certification Requirements. User mustindicate if he/she has completed the Annual Information AwarenessTraining and the date.

(11) User's Signature. User must sign the DD Form 2875 with theunderstanding that they are responsible and accountable for theirpassword and access to the system(s).

(12) Date. The date that the user signs the form.

B. PART II: The information below requires the endorsement from theuser's Supervisor or the Government Sponsor.

(13). Justification for Access. A brief statement is required to justifyestablishment of an initial USER ID. Provide appropriate information ifthe USER ID or access to the current USER ID is modified.

(14) Type of Access Required: Place an "X" in the appropriate box.(Authorized - Individual with normal access. Privileged - Those withprivilege to amend or change system configuration, parameters, orsettings.)

(15) User Requires Access To: Place an "X" in the appropriate box. Specify category.

(16) Verification of Need to Know. To verify that the user requiresaccess as requested.

(16a) Expiration Date for Access. The user must specify expirationdate if less than 1 year.

(21) Signature of Information Owner/OPR. Signature of the functionalappointee responsible for approving access to the system beingrequested.

(21a) Phone Number. Functional appointee telephone number.

(21b) Date. The date the functional appointee signs the DD Form2875.

(22) Signature of Information Assurance Officer (IAO) or Appointee.Signature of the IAO or Appointee of the office responsible forapproving access to the system being requested.

(23) Organization/Department. IAO's organization and department.

(24) Phone Number. IAO's telephone number.

(25) Date. The date IAO signs the DD Form 2875.

(27) Optional Information. This item is intended to add additionalinformation, as required.

C. PART III: Certification of Background Investigation or Clearance.

(28) Type of Investigation. The user's last type of backgroundinvestigation (i.e., NAC, NACI, or SSBI).

(28a) Date of Investigation. Date of last investigation.

(28b) Clearance Level. The user's current security clearance level(Secret or Top Secret).

(28c) IT Level Designation. The user's IT designation (Level I, Level II,or Level III).

(29) Verified By. The Security Manager or representative prints his/hername to indicate that the above clearance and investigationinformation has been verified.

(30) Security Manager Telephone Number. The telephone number ofthe Security Manager or his/her representative.

(31) Security Manager Signature. The Security Manager or his/herrepresentative indicates that the above clearance and investigationinformation has been verified.

(32) Date. The date that the form was signed by the Security Manageror his/her representative.

D. PART IV: This information is site specific and can be customizedby either the DoD, functional activity, or the customer with approval ofthe DoD. This information will specifically identify the access requiredby the user.

E. DISPOSITION OF FORM:

TRANSMISSION: Form may be electronically transmitted, faxed, ormailed. Adding a password to this form makes it a minimum of "FOROFFICIAL USE ONLY" and must be protected as such.

FILING: Original SAAR, with original signatures in Parts I, II, and III,must be maintained on file for one year after termination of user'saccount. File may be maintained by the DoD or by the Customer'sIAO. Recommend file be maintained by IAO adding the user to thesystem.
