    Hand writing was not readable. Most of registers were without title page which

    including name of village, cluster no. & name ofPHC, sub center.

    Pages in registers were not in page no. wise. There were many tick mark in registers so very

    difficult to verify last mark and count it.

    Many torn pages in registers.

    Some names are repeated & some are check out.
