Top causes of data center downtime

1. Human Error - Allowing unqualified persons into the data center

2. Lack of a SOP, MOP, or emergency procedures

3. Lack of predictive measures, including testing and maintenance

4. Failure to maintain or replace UPS and UPS batteries

5. Lack of redundancy resulting in a single point of failure including lack of a back-up generator

6. Primary cooling failure

Could this have been prevented or remedied faster?•Breaker coordination study–When new electrical distribution systems are added to the fold, a re-coordination of the entire system should be done

•MOP & mock drills–more thorough testing methodologies probably could have caught the issue sooner

Amazon Web Services Service DisruptionVirginia, June 2012

One of the main breakers on a back-up power distribution circuit was incorrectly configured to open at too low a power threshold and opened when the load transferred to this circuit.

The army of Cisco and Microsoft guys on staff couldn’t troubleshoot why the power went out.

Power Restored After Outage at San Diego Airport

The Associated Press

Saturday, May 25, 2013 | SAN DIEGO

Power has been restored to a San Diego Airport terminal, after an outage caused minor flight delays and forced security workers to hand-screen luggage and passengers.

According to San Diego Airport Authority spokeswoman Cheryl Brown:

A high-voltage transformer blew on the terminal's roof Saturday evening. The outage shut down computers, jet ways and the baggage delivery system.

Designing and Operating Data Centers for 100% Availability

RagingWire knew that utility outages were just the tip of the availability iceberg. Equipment failures, insufficient maintenance practices, human error, and improper equipment testing and commissioning all lurk beneath the surface.
