My first Health Resources Services Administration (HRSA) training grant explored ways to train and retain family physicians in rural and underserved areas, culminating in piloting, implementing, and accrediting three new “1+2” programs with the second and third year of training outside the large metropolitan teaching hospital in community settings. By augmenting urban, tertiary care training with community-based, rural experiences, the retention of family medicine resident graduates increased from 25% to 50-75%, with half of the graduates practicing in federally designated health professional shortage areas, medically underserved areas, or medically underserved populations (HPSA/MUA/P). Retention success was duplicated with New Mexico’s first dental residency training program, funded in part by the Kellogg Community Voices initiative. I served as Principal Investigator (PI) for that $4.7 million, seven-year initiative. My current work explores sustained financing of community-based primary care education, leveraging Public Health Service Act, Social Security Act (Titles XVIII-Medicare and XIX-Medicaid), and Affordable Care Act provisions to support rural interprofessional innovative education and training models. In 2007-08 I received immersion training in applied health policy as a Robert Wood Johnson Health Policy Fellow working for U.S. Senator Jeff Bingaman. I researched and drafted health workforce provisions later included in Title V of the Affordable Care Act. After returning from Washington, I applied health policy skills as president of the New Mexico Medical Society, drafting medical homes legislation for the state’s Medicaid program, and health workforce data collection legislation requiring licensed health professionals to complete standard data sets at the time of licensing and renewal. These bills were later passed and signed into law by the governor. During my time working for Governor Martinez as the Director of the New Mexico Office of Health Care Reform, I helped prepare the state to implement Affordable Care Act (ACA) provisions. I established New Mexico’s state health insurance exchange / marketplace by writing, submitting, and receiving funding from the Centers for Medicare and Medicaid Services (CMS) for $34.3 million - engaging advocates, institutions, insurers, providers, consumers and stakeholders in the process. New Mexico’s state Marketplace was later anchored by authorizing legislation creating the New Mexico Health Insurance Exchange, signed into law by the governor. As principal investigator of state, federal, private foundation, contract and cooperative agreement funding in excess of $50 million in my academic career, I work to improve health insurance coverage and access to quality health care, emphasizing community-based service-learning models in rural areas. Health Care Reform – Governor Susana Martinez appointed me Director, New Mexico Office of Health Care Reform in August of 2011. The priorities of the Office were:

a.   New Mexico Health Insurance Exchange (NMHIX) – to provide consumer choices for high quality care and to streamline regulatory and administrative processes. I wrote then submitted the level one establishment proposal on 9/29/11, and received CMS notice of award for $34,279,483 on 11/29/11. In 2013, legislation was passed then signed by the governor formally creating NMHIX.

b.   Medicaid Modernization – the principles were to: 1) Incorporate services tailored to meet the needs of NM’s vulnerable populations, offering full

spectrum benefits from newborns to the elderly. 2) Increase personal responsibility – create incentives to rewarding healthy behaviors, institute

sliding scale co-pays for overused services (ex. emergency department). 3) Institute pay-for-performance incentives to reward health care outcomes rather than only paying

for quantity of services provided (ex. patient centered medical home, health home).

Medical Home, Health Home) that measurably promote coordination and integration of care, incentivize personal responsibility, reduce fraud and abuse, align payment rewards with improved health outcomes, control cost growth and assure patient satisfaction.

Health Workforce - Community-Based Education - Service Learning Models – The U.S. health professions pipeline begins and often ends in more prosperous and adequately staffed urban areas, leaving underserved inner city and rural areas and populations without access to quality care. By creating service-learning models in areas of need, in community health centers and public hospital sponsored clinics, access is improved, and more health professionals are trained and settle into practices where they are most needed. Working as health staff for Senator Jeff Bingaman as a Robert Wood Johnson (RWJ) Health Policy Fellow, I researched and drafted provisions for a comprehensive health workforce bill introduced in the 111th Congress - S.790 “The Health Access and Health Professions Supply Act of 2009.” Provisions were included in H.R. 3590 the “Patient Protection and Affordable Health Care Act” signed into law in March 2010. The provisions I worked on during my time with Senator Bingaman that were later included in the ACA were:

a)   National Health Workforce Commission (ACA Title V, Sec. 5101). b)   Teaching Health Centers – THC’s (Title V, Sec. 5508), Status: $230 million appropriated for 2011 to 2015.

THC’s were reauthorized in the 2015 Medicare Access and CHIP Reauthorization Act (MACRA). c)   Sec. 5102 State Health Care Workforce Development Grants and Sec. 5103 Health Care Workforce

Since my first grant as an assistant professor in 1990, and continuously through 2016, I have served as principal investigator of state, federal, contract, and private foundation funding in excess of $50 million. My first two grants were Health Resources Services Administration (HRSA) Public Health Service Act (PHSA) Title VII, Section 747 primary care training grants from 1990-96 that decentralized family medicine residency training to medically underserved, community-based sites. Three new Accreditation Council for Graduate Medical Education (ACGME) accredited sites were created in Santa Fe, Las Cruces, and Roswell, NM. Overall, only 25% of the University of New Mexico (UNM) School of Medicine resident graduates remained in NM to practice. The decentralized family medicine programs retained 50 to 75% of graduates in NM, most practicing in rural and underserved areas. These pioneers served as the models for the H.R. 3590 “Patient Protection and Affordable Care Act” Title V, Sec. 5508 Teaching Health Centers provision that I researched and drafted as a staff member for Senator Bingaman (NM). New Mexico was 49th of 50 states in per capita dentists. I helped recruit dental faculty in partnership with the UNM Department of Surgery. Using the practice relief model developed through the UNM Locum Tenens Program (primary care practice relief program that I served as director of for 12 years) and Specialty Extension Services Program (that I founded and directed for nine years), and seed funding from the Kellogg Foundation (Health Commons) grant I was PI on, we created a dental residency program in 2004. Almost two-thirds of the dental residency graduates remained in NM to practice, and half were underrepresented minorities. In 2008-09, I served on an Institute of Medicine planning committee for an oral health workforce meeting. Over my academic career, I used data to inform policies at the state and federal levels to retain health professionals in rural areas. I testified at the state legislature about New Mexico’s health workforce shortages, and recommended programs to address them, resulting in legislation and a permanent appropriation to create the UNM Locum Tenens (LT) Program. I directed the UNM Locum Tenens Program starting in 1995, founded UNM’s Specialty Extension Services (SES) Program in 1998, and served as director of both programs through 2007. The LT/SES Programs provided > 700 days of practice relief coverage per month in 2007. Over the years I directed these programs, over 250 resident graduates were placed into NM practices where they had provided LT/SES coverage. Of the 500 resident and other providers that participated in the LT/SES Programs, half continued to practice in NM.

The academic practice relief model, and the decentralized training of residents in rural areas and community-based venues, received state, national, and international recognition for improving the geographic distribution of health services to areas of need. I worked with New Mexico’s Health Policy Commission, Department of Health, and the Medical, Dental and Nursing Boards to create data tools to inform public policy and improve access to care. A physician workforce survey was developed, administered, analyzed and the results disseminated in 2001 and again in 2006. The survey was incorporated into the physician electronic licensing and re-licensing in 2010. To assure data could be generated for nursing, dental, physicians and other licensed clinical providers through state licensing boards, I worked with state legislators to draft SB 14, the “Health Care Workforce Data Collection, Analysis and Policy Act of 2011” (Senator Dede Feldman). Since it was enacted, crucial licensed health workforce data has been generated - particularly important as the federal designation and scoring of Health Professions Shortage Areas (HPSA’s), Medically Underserved Areas/Populations (MUA/P) are updated. Thirty-eight federal programs, including the National Health Service Corps scholarship and loan repayment programs, use HPSA/MUA/P scoring to allocate funds. Licensing data overestimate direct patient care full time equivalents. Accurate, timely data help assure rural and medically underserved urban areas have an adequate health work force. Health Commons – Community-Based, Patient-Centered Medical Homes - New Mexico and Arizona are rural states with the majority of their counties federally designated as HPSA/MUA/P. Intractable health problems, such as health disparities between economic and ethnic groups, poor access to care, chronic diseases, and the uninsured have social determinants. The Health Commons conceptual model was developed by the UNM Department of Family & Community Medicine with safety net collaborators. The Health Commons linked medical, dental, behavioral, care management and social services in an enhanced, community-based, patient-centered primary care home. I was principal investigator (PI) for the W.K. Kellogg Foundation’s Community Voices Healthcare for the Underserved initiative, an award totaling $4.7 million from 1998-2007. The interventions reconfigured the health system to respond to individual, provider and community needs. For example, the UNM Care Plan created a primary care home for 15,000 of Bernalillo County’s uninsured. The UNM Care findings were published in Academic Medicine in 2000, and descriptions of the Health Commons model were published in the American Journal of Public Health in 2002 and the Annals of Family Medicine in 2006. These models framed the insurance and delivery system reforms necessary to create public-private partnerships to provide health insurance to 45,000 of the state’s working poor through the State Coverage Insurance. SCI was a Medicaid expansion allowed by a Centers for Medicare & Medicaid Services (CMS) Health Insurance Flexibility & Accountability Initiative (HIFA) waiver. These experiences proved invaluable in creating New Mexico’s state based health insurance exchange / marketplace (NMHIX), catalyzed by the $34.3 million CMS establishment grant that I wrote and submitted in 2012. NM’s legislature formally passed a bill creating NMHIX, signed into law by the governor in 2013. By March of 2016, nearly 55,000 had selected plans on New Mexico’s Health Insurance Marketplace. New community based health commons model clinic sites were funded, constructed, and opened in three NM counties (Bernalillo, Hidalgo, Sandoval). The Health Commons model was used in frontier areas of the state such as Lordsburg, which had not had a physician in over a decade, and never had a dentist. Now Hidalgo Medical Services employs 85 individuals, and is the largest employer in Hidalgo County. The model helps small communities catalyze economic development, improves health outcomes, quality & access to care and are ideal sites to train health professionals including students and residents to learn team-based, integrated and coordinated care. In 2009, during my NM Medical Society term as president, I helped research and draft House Bill 710, a Medical Homes bill (Representative Danice Picraux), passed by the state legislature. After the governor signed the bill into law in 2009, I worked on helping practices create medical homes programs for the state’s 550,000 enrollees in Medicaid, Children’s Health Insurance Program (CHIP), and State Coverage Insurance (SCI, a public-private partnership covering 45,000 of the state’s working poor) programs. I served on the Lovelace Clinic Foundation (LCF) Research Board. LCF was the state’s Health Information Exchange (HIE), that helped hospitals, physicians and providers throughout the state adopt and “meaningfully use” Electronic Health Records. EHR’s are essential tools for practices to implement Patient Centered Medical Homes and Accountable Care Organization models. In the four years I have been back at the University of Arizona, I have expanded my work in applied health policy – informing policy related to increasing health insurance coverage, reducing the uninsured, narrowing health disparities, and assuring a well-trained and distributed health workforce to meet the health needs of Arizonans. These principles are reflected in my current PI grant portfolio, and the Arizona Center for Rural Health’s mission to improve the health and wellness of Arizona’s rural and vulnerable populations.
