Account set-up

In early July you’ll receive your 12 digit code.

Set up your registration account and access the application form.

Tell us basic information - including your provider type and your contact details.

Disable any extra codes you don’t need by entering them in the form.

Batch windows

Select a 28-day batch window in which you’ll submit your application.

Think about what changes that may be happening and pick a window that best suits you .

Provider type

This is the legal entity which is responsible for the primary medical services you supply.

It is the overall provider of services that must register with CQC, not the individual premises where care is provided.

You can register as a partnership, an individual or an organisation.

CRB and GMC numbers

For registration only the provider and the registered manager will need a CRB number.

If you have a GMC number this can be used instead.

Those without a GMC number will need to have a CQC- countersigned enhanced CRB.

Private work

Include all the services the provider is responsible for in your registration form, this includes any private work.

In some cases this may be undertaken by a separate legal entity.

If you do have a separate legal entity this will have to register with CQC in its own right.

Filling in and submitting

As part of registration set-up you’ll be asked to create an account.

You can set up access for a second person to help fill in the form.

Either account holder can fill in any part of the application.

You can change or delete user accounts for your application (e.g. if someone leaves) at any time.

You’ll need to make one person responsible for signing and submitting the form.

Only the responsible person can do the final review, sign the declaration and submit.

It should be one of the partners on behalf of the partnership or a director on behalf of an organisation.

In a partnership we would expect all partners to look at the application form and accept its contents before it’s signed off and submitted.

You can make changes to any part of your registration application up until the point you submit it to CQC.

Complete the registration application as you currently are - don’t pre-empt changes.

You’ll be able to make changes through your online account from early 2013.
