A required one-time, non-refundable fee will be assessed before computer check- out. An installment arrangement can be made if necessary. Check-in Date: May 2020 (or upon date of withdrawal from school) The aforementioned items being lent to the borrower are in good working order. It is the borrower’s responsibility to care for the equipment and ensure that it is retained in a safe environment. This equipment is, and at all times remains, the property of the Catoosa Public School district, of Catoosa, OK. It is herewith lent to the borrower for educational purposes for the academic school year. The borrower may not deface or destroy this property in any way. Inappropriate use of the machine may result in the borrower’s loss of rights to the machine. The equipment will be returned when requested by Catoosa Public Schools at the end of the academic school year, or upon the date of withdrawal from Catoosa Public Schools. The district property may be used by the borrower only for non-commercial purposes, in accordance with the district’s policies, the Catoosa Public School Student Handbook, as well as local, state, and federal statutes. The borrower may not install or use any software other than what is owned or approved by the district, and made available to the borrower in accordance with this agreement. The borrower agrees not to make any unauthorized use of modifications to said property. The district is not responsible for any computer or electronic viruses that may be transferred to or from the borrower’s data storage devices. The borrower agrees to use his or her best eorts to assure that the district property is not damaged or rendered inoperable by any such electronic virus while in the borrower’s possession. Catoosa Public Schools maintains the right to obtain reimbursement from, or on behalf of borrowers for any intentional damage to, loss of, or failure to return district property. The borrower acknowledges and agrees that the use of the district property is a privilege. By the borrower’s agreement to the terms hereof, the borrower acknowledges

If the borrower damages, loses, or destroys the charger or charger cord, the borrower will be responsible for the replacement or repair of the device. If possible, the borrower should charge his/her laptop at home and not bring the charger to school, so that it doesn’t get lost or stolen.

POLICIES AND RULES REGARDING COMPUTER USE

PARENTAL RESPONSIBILITIES

Your son/daughter has been issued a new MacBook Air computer to improve and personalize his/her education this year. It is essential that the following policies and rules be followed to ensure that your son/daughter receives the maximum benefit from the use of the computer, and that the computer is used in a safe, efficient, and ethical manner.

• I understand that my son/daughter will not be allowed to take the laptop home untilall holds are cleared against his/her name, or arrangements for payment have beenmade.

• I understand that there is a non-refundable user fee I must pay for my child to use apersonal laptop. If this fee is paid upfront, I will receive a 10% discount, or I can payeach semester in two payments.

• I understand this fee does not cover the charger or charger cord. If this, or anycomponent of this, is damaged, lost, or destroyed, it is the sole responsibility of mychild to replace it.

• I will supervise my son’s/daughter’s use of the laptop at home.

• As a family, we will discuss appropriate places to use the laptop to ensure its safetyfrom dropping and damage.

• I will discuss our family’s values and expectations regarding the use of the Internetand email at home, and I will supervise my son’s/daughter’s use of the computer toaccess the Internet and email accounts.

• I will not attempt to repair the laptop, nor will I attempt to clean it with anything otherthan a soft, dry cloth.

• I will immediately report to the school any problem with my son’s/daughter’s laptop,and I understand that if my son/daughter damages the computer through gross

negligence, there will be a $50.00 fine, and/or my child may face consequences up to and including the loss of computer use, privileges, or suspension.

• I will not attempt to load or delete any software to/from the laptop.

• I will make sure my son/daughter recharges the laptop battery nightly.

• I understand that if my son/daughter comes to school without his/her computer, Imay be called to bring it to the school.

• If I am unable to bring the computer to the school, my son/daughter may be requiredto complete an alternate assignment; if the laptop is being used in class, my son/daughter will still be responsible for completing the missing computer assignment.

• I understand that only a portion of my child’s instruction and assignments will becompleted using technology. There will be traditional instruction and assignmentsrequired of my child.

• I agree to make sure the laptop is returned to the school when requested or upon myson’s/daughter’s withdrawal from school.

STUDENT RESPONSIBILITIES

Your laptop is an important learning instrument and is primarily for educational purposes. In order to take your laptop home each day, you must be willing to accept the following responsibilities:

• All holds must be cleared or payment arrangements made before I will be allowed totake the laptop home.

• I understand that the user fee does not cover the charger or the charger cord. If this,or any component of this, is damaged, lost, or destroyed, it is my responsibility toreplace or repair it.

• When using the laptop at home, at school, or anywhere else I may take it, I will followthese policies and rules, as well as the rules contained in the Catoosa Public Schoolhandbook, and I will abide by all local, state, and federal laws.

• I will treat my laptop, and everyone else’s laptop, with care by carrying it in its bag,and not dropping it, getting it wet, leaving it outdoors, using it with food or drinknearby, or using it in horseplay, etc.

• I will not loan my laptop to anyone, not even my friends or siblings; I am responsiblefor my laptop, and it will stay in my possession at all times.

• I will not attempt to load or delete any software onto the laptop.

• I will not remove programs or files from the laptop.

• I will honor my family’s values when using the laptop.

• I will use my laptop in safe locations as agreed to by my parents.

• I will not give personal information when using the laptop.

• I will not use the laptop to spread rumors or create conflict with other students.

• I will bring the laptop to school fully charged and in its bag every day. If it isn’t in thelaptop bag, it will be taken away until the bag is brought to school.

• If I forget my laptop, I will be allowed to call home and ask someone to bring it toschool.

• If I do not bring my laptop to school, I may be required to complete an alternateassignment if the laptop is being used in class. I will still be responsible forcompleting the missed computer assignment.

• I agree that email (or any other computer communication) should be used only forappropriate, legitimate, and responsible communication.

• I will keep all accounts and passwords assigned to me secure, and I will not sharethese with any other students.

• I will not attempt to repair the laptop, nor will I attempt to clean the laptop unless I’munder the direct supervision of a teacher and have been issued a Clorox wipe.

• I will return the laptop, charger, and bag when requested or upon my withdrawal fromCatoosa Public Schools.

• I understand that if I damage the laptop or allow any damage to the computer, therewill be a $50.00 fine, and/or I will face consequences up to and including the loss ofcomputer use, privileges, or suspension.

• If I intentionally damage another student’s laptop, I may have to pay a $50.00 fineand face administrative punishment.

• I will return the laptop in good condition when requested or upon my withdrawal fromCatoosa Public Schools; if damages are noticed at this time, I will have to pay arepair fee for these damages.

• I will report to the school any problem with my laptop immediately.

• I understand that if I violate any of these rules, I may lose the privilege to use thecomputer at home or even at school.

Consequences for MacBook Air Damage

Catoosa Public School’s IT Department may determine whether damage is accidental or due to neglect.

Accidental Damage

• 1st Insurance Claim: Students will not be fined. Laptop will be repaired at no cost.• 2nd Insurance Claim: The student forfeits the right to take their laptop home, but the

school will repair the laptop.• Examples of Accidental Damage: Another student accidentally knocks the laptop

off of desk; someone trips over the charger cord and pulls the laptop off of a desk.

Damage Due to Negligence

• 1st Insurance Claim: The student will have to pay a $50.00 fine, but he/she keepsthe right to take their laptop home.

• 2nd Insurance Claim: The student will have to pay a $50.00 fine and will forfeit theright to take the laptop home. If a student damages the laptop more than two times,he/she may forfeit their right to get a personal laptop, and he/she will have to use acomputer in the library.

Computer Use Fee

• 6th grade: $60.00 for the academic school year ($20.00 for the 1st semester and$40.00 for the 2nd semester)

• 7th-12th grade: $80.00 for the academic school year (may be paid in two paymentsof $40.00)

• Some families may qualify for a 10% discount off of their total fee if they qualify forone of the following:

1. Free/Reduced Lunches2. Two or more students in grades 6th through 12th3. The entire fee is paid by August 31st

I understand that the school district has taken reasonable precautions to ensure that access to controversial material is limited to the extent possible, but I realize that it is not possible to guarantee that my child will never encounter objectionable material. I hereby release the school district from liability in the event that my child acquires inappropriate material through use of the district’s technology resources, including the Internet.
