Phase 1: Mental Health• Work Process:• Start: by Jan 30th, 2014• Time: 8 hours/day- 6 days/week• Patients:– ~10 new patients/month, ~30-40 active Pt.'s at a time.
• ROI: – Reducing readmissions– Freeing up in patient beds
Phase 2: Maplewood, MN• Work Process:• Start: by May, 2014• Time: 8 hours/day- 6 days/week• Patients: – Not eligible for other outreach programs, (home care, CHF
care, diabetes outreach)– ~20 active Pts./week
• ROI: – Increased compliance with care plans– Improved utilization patterns– Decreased geriatric fractures, frequency of lift assists
Phase 3: Hospital & Clinics• Work Plan• Start: by Sept, 2014• Time: 8 hours/day- 6 days/week• Patients: – Not eligible for other outreach programs, (home care, CHF
care, diabetes outreach)– ~80 active Pts./week
• ROI: – Increased compliance with care plans– Improved utilization patterns– Decreased geriatric fractures, frequency of lift assists
