Abstract

Pheochromocytoma is a rare tumor of chromaffintissues most commonly arising from the adrenalmedulla. We retrospectively reviewed the records of34 patients with pheochromocytoma who underwentsurgical treatment between 1971 and 2006. Fourteenpatients (41.2%) were females and twenty (58.8%)were males. The most frequent symptoms wereHypertension (97%) and palpitation (38.2%). Sevenpatients had an extra-adrenal tumor and in onepatients the tumor occurred in the urinary bladder.Nine patients (26.5%) had persistent hypertension, 11patients (32.4%) had paroxysmal hypertension and 13patients (38.2%) had persistent with paroxysms. The24-h urinary total metanephrines and vanillylmandelicacid (VMA) were the most sensitive biochemical testsfor the diagnosis of pheochromocytoma. One of ourpatients was found to be associated with hereditarypheochromocytoma syndrome. All hypertensivepatients were preoperat ively treated withphenoxybenzamine and propranolol. All underwentexplorative laparotomy and adrenelectomy. Allpatients were followed up with 24-h urinary VMA levelsand CT scan of abdomen regularly.

Introduction

Pheochromocytomas are most commonly tumours ofadrena l medu l la ry o r ig in and i s a ra recatecholamine-producing tumor of neuroectodermalorigin arising from chromaffin cells. Incidence isaround 1 to 2 per million population. Poll coined theterm pheochromocytoma in 1905 when he describedthe dusky (Pheo) color (Chromo) of the cut surface ofthe tumor when exposed to dichromate (1,2) .Pheochromocytoma by definition produces andsecretes catecholamines. Similar tumours that do notsecrete active substances of any kind are called nonfunctioning paragangliomas. The hallmark clinicalmanifestation of pheochromocytoma is hypertensionaccompanied with various signs and symptoms inexcess of catecholamines or other bioactives u b s t a n c e s . T h e e a r l y d i a g n o s i s o fpheochromocytoma is important not only because itoffers the possibility of curing hypertension but also

because unrecognised pheochromocytoma is apotentially lethal condition. The aim of this article is toanalyse the clinical finding, diagnostical values of thelaboratory tests and possibilities of morphologicallocalizing techniques in a series of 34 patients withsurgically proven pheochromocytoma.

Pheochromocytoma an overwhelming 90% of allcases arise from adrenal medulla where the biggestcollection of chromaffin cells is found. Extra-adrenalpheochromocytoma (also called paragangliomas) isusually encountered intra-abdominally along thesympathetic chains or from the organs of Zuckerkandl(3). Intra-thoracic pheochromocytoma (<1%) is alsorelated to the sympathetic chain. Other extra-adrenalsites are intrapericardial (4,5) inter-atrial septum(6), prostate(7) and urinary bladder.

The first clinical description of pheochromocytoma wasby F. Frankel in 1886 when a young female patientwith a history of episodic attacks of headaches,palpitations and anxiety died suddenly. Post-mortemexamination showed bilateral adrenal medulla tumors.The first surgical excision of the pheochromocytomawas reported in 1927 by Roux who described removalof a suprarenal tumor in a patient with a two-yearhistory of episodic vertigo and nausea(8,9).

Familial pheochromocytoma in association withmedullary carcinoma of the thyroid and parathyroidgland hyperplasia or adenoma have been designatedas multiple endocrine neoplasia syndrome type II A(MEN IIA or Sipple's syndrome) .MEN type IIB is thecoexistence of pheochromocytoma and medullarythyro id carc inoma wi th gast ro in tes t ina lganglioneuromatosis (benign mucosal neuromas inlips, tongue, buccal cavity), benign mucosal neuromasin eye lids conjuctiva and cornea, and marfanoidfeatures. Pheochromocytoma is also recorded as afirst manifestation of von-Hippel-Lindau disease, anautosomal dominant disorder characterized by thedevelopment of hemangioblastomas in the cerebellum,spinal cord and retina, renal cell carcinoma and cysts,pancreatic cysts and pheochromocytoma. There isalso an increased prevalence of pheochromocytomaamong patients with von-Recklinghausen's disease(10).

Pheochromocytomas account for 0.1% of all patientswith hypertension and can present with a highlyvariable clinical picture. These two characteristics

(rarity and variability) render these tumors very difficultto diagnose so that many of them are discoveredincidentally during radiological examinations(especially of the abdomen) or at autopsy.

Materials and Methods

This is a retrospective study of all patients diagnosedto have pheochromocytoma from 1971 to 2007.Medical records were used for collecting datasincluding demographic data, clinical symptoms,familial disease, pre- or post-operative urinaryconcentrations of VMA, localization procedures,preoperative pharmacological treatment, surgicalfindings,operative and histopathological reports andfollow up status were recorded and analyzed..Inpatient records were used for collecting datas of theimmediate postoperative period (until discharge or 40days – whichever is later) and the outpatient recordsfor additional information including follow up. Themedian period of follow up is two years.

Discussion

The present study showed wide variability insymptoms as compared to other studies (11,12). Thepresence of a paroxysmal event which, althoughaspecific, has always been considered as a hallmark,has been reported in 11 out of 34 patients in our study.In contrast to other studies (13), the most frequentsymptoms were abdominal pain and hypertensionwhile frequency of other symptoms was rather low.Therefore, in view of the very low sensitivity of anysymptom or of any association of them, the clinicalsuspicion is often extremely difficult. This difficulty canexplain the long mean time lag between initialsymptoms and diagnosis and why, quite often,pheochromocytoma is discovered as an incidentaladrenal mass or at autopsy. Therefore, the mostfrequent reason for suspecting pheochromocytoma ishypertension, especially if paroxysmal or resistant,especially if accompanied by an adrenal mass.

One of our patients were associated with the familialform of pheochromocytoma. Though one of ourpatients were incidentally discovered, data fromprevious studies confirm that pheochromocytomashave to be taken into account in the differentialdiagnosis of adrenal incidentalomas and that theabsence of hypertension does not rule out thepresence of a pheochromocytoma (14),

The biochemical method used for the diagnosis ofpheochromocytoma in our study was 24-h urinaryVMA levels. This finding is in agreement with the datareported in the literature,(15,16,17) but use of moresensitive assays such as urinary and plasma freemetanephrines should be considered for bettersensitivity and specificity. Results indicate similarlyhigh sensitivity of plasma total or free or urinarym e t a n e p h r i n e s f o r t h e d i a g n o s i s o fpheochromocytoma. All three measurements provideclearly better clues to detect this tumor compared withthe classical determination of plasma catecholamines(18) To date, none of these tests appear to be able todetect pheochromocytoma before the others at anearlier time of tumor development.(19) A normal valueof any of these does not exclude the presence of apheochromocytoma. Recent studies indicate plasmafree metanephrines as the most sensitive diagnosticindex. (20)

Localization of the tumor relies mainly on CT scan ofabdomen(21). Nevertheless, due to the frequency ofincidental adrenal masses, MIBG scintigraphy shouldalso be performed before surgery. In the present study,CT scan of abdomen was used in all cases forlocalization with 100% sensitivity while MIBG scanwas used in one patient who had negative VMA andUCA. In our study, 7 out of 34 tumors wereextra-adrenal, which is in line with most recent studiesin which ectopic tumors comprise 10 to 29% of theadult pheochromocytomas. The 10% rule is no longerapplied (22) the higher rate may well reflect increaseddisease awareness and improved tumor localizingfacility since the last century. None of Our tumorsshowed evidence of malignancy as judged by localinfiltration or presence of metastasis.

As compared to other studies(23) preoperativepreparation formed an important part of managementin our study. All hypertensive patients receivedphenoxybenzamine and propranolol before theoperation. Arterial line placement and preoperativecorrection of intravascular volume was done in allpatients.

The effects of surgery on BP were documented in ourstudy by the disappearance of hypertension in about60% of patients. BP did not change significantly aftersurgery in 40% of our patients with hypertensionindicating, as suggested by others, the presence ofother causes of hypertension or nonreversiblecatecholamine-induced structural changes in thecardiovascular system or delay in diagnosis(24)

Demographic data

This study includes 34 patients of surgically proven pheochromocytoma. We retrospectivelyreviewed the records of 34 patients with pheochromocytoma who underwent surgicaltreatment between 1971 and 2006. Fourteen patients (41.2%) were females and twenty(58.8%) were males.[Figure-1]

Clinical presentation

The most frequent symptoms were Hypertension (97%) and palpitation (38.2%). Thevarious patterns of hypertension among the patients in our study are shown in the table.

Biochemical and pathological evaluation

The 24-h urinary total metanephrines and vanillylmandelic acid (VMA) were used todiagnosis pheochromocytoma. Among the 34 patients, 32 patients ( 94%) were positive and02 patients (6%) showed negative results for urinary VMA. A lab value of VMA more than8mg/24 hours urine sample is considered as positive. A lab value of UCA of morethan 150ug/24 urine sample is considered as positive test. In this study, 30 patients(88.2%) were positive for UCA and 04 patients (11.8%).were negative for UCA test. In onepatient both VMA and UCA were negative and MIBG scan was done to confirm the diagnosisof pheochromocytoma.

Localization procedure

In our study CT scan is used as a predominant tool to localize the tumour. Eight of thepatient had tumor on the left side and nineteen of them had the tumor on the right side andseve in the extra adrenal sites. The distribution of the extradrenal pheochromocytoma in ourstudy is shown[Figure-2]

Preoperative preparation

All hypertensive patients received phenoxybenzamine and propranolol before the operation.Arterial line placement and preoperative correction of intravascular volume was done in allpatients.

Intraoperative eventsAll our patients underwent open laparotomy. Midline incision with lateral extension was usedin all cases. Adrenal vein was ligated first and minimal handling of tumor was done in allcases.[Figure-4]
