4 Keweenaw Chamber of Commerce Newsletter
The Portage Health Foundation, a non-profit charitable foundation serving Baraga,
Houghton, Keweenaw, and Ontonagon counties, is pleased to announce it is soliciting
targeted grant requests throughout the four-county area that focus on mental and be
havioral health programs.
The PHFoundation recognizes the importance of addressing mental and behavioral
health concerns and the need for more available care. According to the Copper Country
Mental Health Services’ (CCMHS) 2014 Community Needs Assessment Survey Report,
there is a substantial demand for more access to mental health services in the four counties. The report
identifies the following top-five concerns among survey respondents:
1.the need for more access to mental health services;
2.the need for more access to psychiatric services;
3.the need for more mental health services for children and youth;
4.the concern over increased substance/drug abuse; and
5.the need for more local inpatient/outpatient programs.
Additionally, the Western Upper Peninsula 2012 Regional Health Assessment Report highlights many of these
concerns. For instance, mental health and substance abuse issues are prevalent across the Western U.P. About
24% of local adults report a chronic or episodic depression diagnosis. From 2009-2011, alcohol was the most
common substance identified as a primary reason for hospital admission in publicly funded programs followed
by opiates (which includes prescription painkillers), and marijuana. Heavy long-term drinking can raise an
individual’s risk of liver disease and drug abuse can lead to serious health conditions. However, with additional
and/or expanded community resources to meet the gaps in available mental and behavioral health services,
individuals suffering from such issues will have more access to care and opportunities to heal. Evidence has
already shown that depression, for example, is a treatable mental health condition. Furthermore, with
expanded resources, service providers can also place additional effort on preventative measures.
“Mental and behavioral health needs are not isolated to our community,” said Kevin Store, Executive Director
of the Portage Health Foundation. “There exists real gaps in services locally and throughout the Upper
Peninsula. We hope this request for proposals will be met with innovative and collaborative proposals that will
address the needs of our citizens.”
The CCMHS report also indicates that to meet the mental health needs of the region, collaboration and
cooperation among community partners is required. To assist in meeting these needs and in creating
community partnerships, the PHFoundation is now accepting grant requests of all scope and size, or
applications that need partial funding, as part of this Request for Proposal (RFP) notification. Grants addressing
the following will be considered under this RFP:
Community health and preventative health initiatives that address areas of critical health risks, which include, but
are not limited to, behavioral and mental health and substance abuse and addiction.
Interested parties may download an application from the foundation’s website (www.phfgive.org); request a
grant application packet by contacting 906.523.5920; or by stopping by the PHFoundation offices located at:
PHFoundation
400 Quincy Street
Hancock, Michigan 49930
Grant requests will be accepted until Friday, September 4, 2015, and must be received at the PHFoundation
Offices no later than 3:30 p.m. EST. Late and incomplete applications will not be considered.

The Regional Chamber Alliance with other Western U.P. Chambers of Commerce are meeting for further collaboration in all six counties (Houghton/Keweenaw, Baraga, Ontonagon, Gogebic and Iron). We are working together to further advance the business community as a region. We are forming a steering committee from the six county Chambers to begin working collaboratively. Each Chamber is to identify one or two blight concerns on the highway in their area that affect us all. Something they could use more expertise from a larger group to address, that is too large to address on their own. Among other ideas like a regional business expo.
The Baraga Chamber of Commerce will be hosting the upcoming Thirsty Thursday at the Baraga Lakeside Inn on Thursday, August 20th. They are in the process of setting the agenda, one topic will be Cooperatives. Anyone may attend a Thirsty Thursday, if you are interested please RSVP and see the information on the Thirsty Thursday event in this newsletter.

Safe & Reliable Electricity
Customer Service
Proud Community Supporter
Our roots run deep in the Keweenaw, going back to 1884, just five
years after Edison’s incandescent bulb put electricity at the fingertips
of people. That year, Peninsula Electric Light and Power Company
was organized in the Keweenaw Peninsula. Because Peninsula
Electric Light and Power was our original predecessor, you might say
UPPCO grew up with the electric utility industry. From the Edison
bulb to high-tech electronics, our company has been providing energy
to the area and U.P. for more than 100 years.

This tax tip is updated to clarify that self-insured employers that are not applicable large employers, those with fewer than 50 full-time or full-time equivalent employees in the preceding calendar year, should file Form 1095-B. Self-insured employers that are applicable large employers should report health coverage information on Form 1095-C. This tip does not apply to information reporting on health care coverage of individuals who are not employees or entitled to coverage because of a relationship to an employee.
All providers of health coverage, including employers that provide self-insured coverage, must file annual returnswith the IRS reporting information about the coverage and about each covered individual. Insurance Companies must report on coverage under employer plans that are insured.
Employers should report this information on Forms 1094-B and 1095-B or on Forms 1094-C and 1095-C, depending on whether the employer is an applicable large employer for purposes of the employer shared responsibility provisions. An applicable large employer is generally defined as an employer that employed an average of at least 50 full-time employees – including full-time equivalent employees – in the preceding calendar year. `
As coverage providers, employers providing self-insured coverage that are not applicable large employers must:• Report the coverage on a Form 1095-B, Health Coverage, filed with the IRS, accompanied by a Form 1094-B
transmittal. While filers of more than 250 Forms 1095-B must e-file, the IRS allows and encourages entities with fewer than 250 forms to e-file.
• Furnish a copy of the 1095-B to a “responsible individual,” the person who should be the statement recipient. For employer coverage the statement recipient generally is the employee. Providers, including self-insured employers, may electronically furnish the Form 1095-B if the recipient consents.
If an employer providing self-insured coverage is an applicable large employer, it generally reports information regarding coverage of an individual on Form 1095-C instead of Form 1095-B. Form 1095-C combines reporting for two provisions of the Affordable Care Act for these employers. However, when reporting coverage of an individual who was not a full-time employee for any month of the year, an applicable large employer may choose to use Form 1095-B.
The information reporting requirements are first effective for coverage provided in 2015. Thus, health coverage providers will file information returns with the IRS in 2016, and will furnish statements to individuals in 2016, to report coverage information in calendar year 2015.
The information that a provider must report to the IRS includes the following:• The name, address, and employer identification number of the provider.• The statement recipient’s name, address, and taxpayer identification number, or date of birth if a TIN is not
available. If the statement recipient is not enrolled in the coverage, providers may, but are not required to, report the TIN.
• The name and TIN, or date of birth if a TIN is not available, of each individual covered under the policy or program and the months for which the individual was enrolled in coverage and entitled to receive benefits.
For more information, see Questions and Answers on Information Reporting by Health Coverage Providers on IRS.gov/aca. Employers who provide self-insured coverage should review Publication 5215, Responsibilities for Health Coverage Providers. Applicable large employers should review Publication 5196, Reporting Requirements for Applicable Large Employers.
Employers Providing Self-Insured Health
Coverage Must Report on Information Returns
Tickets on sale NOW for The Really Big Show, August 1
Last year, ‘The Really Big Show’ sold out! But Program Director Todd Brassard is hopeful that earlier ticket sales this year will increase the chances for die-hard fans to purchase seats in advance.
The show is Saturday, August 1 at Michigan Technological University’s state-of-the-art Rozsa Center, opening at 5:30pm for VIP ticket holders and 7pm for general admission.
Last year more than 1,100 people packed the Rosza Center to listen to young musicians rock the house. Heading into the program’s sixth year, the show is expected to sell out again.
With auditions almost over and only a few spots left, PJ encourages talented musicians to send their audition videos now. Specific performances and campers won’t be announced until July. However, the same amazing team of world-renowned instructors will again guide the young musicians through this well-rounded, life-changing experience, and help them develop their music talents. The show is guaranteed to blow fans’ minds away again this year!

About PJ Olsson’s Rock CampPJ Olsson’s Rock Camp is an intensive two-week musical program for area youth, ages 11 to 20. Campers are rapidly developed from independent musicians into tight-knit bands through the exciting real-world process of self-exploration, talent development, musical collaboration, relentless rehearsal, and live performance. “In an age of video games, smart phones, and social media, kids are more likely to text than talk and Facebook than face-to-face,” says Creative Director, PJ Olsson. “Rock Camp provides a unique opportunity for kids to get together creatively in a positive, safe, and cultivating environment.”
