Care4life Outreach & Enrollment Toolkit

OverviewThis toolkit has been developed to provide a brief overview of the care4life product and resources. It has been designed to aid in your organization’s outreach and enrollment efforts.

care4life: The ProductAn innovative text-based diabetes education and management tool, care4life, has the ability to allow users to set blood glucose test reminders; medication and appointment reminders; weight and exercise goals; and receive healthy eating and living tips. Based on the American Diabetes Association and National Diabetes Education Program self-care guidelines, care4life is a 6 month program to encourage users to take control and gain the skills to manage their diabetes. Offering reminders and support to your patients in a free, convenient, on-the-go package; care4life is an innovative self-management system.

How to use this Toolkit This toolkit has been designed as a guide for patient engagement, outreach, and enrollment efforts. The care4life program offers flexibility in arranging and planning how you will offer care4life to your patients, including many different outreach options. Each organization has the opportunity to choose the best outreach option for their patient population; therefore this toolkit will offer an overview of the outreach options, the enrollment process and provide additional resources to aid in patient enrollment.

care4life PartnershipYour organization has partnered with MPCA to offer this innovative, easy access, diabetes management tool to your patients. The next step is to engage patients and assist them in the enrollment efforts. At the end of the toolkit you will find an Appendix that includes several resources including additional information on the product and the enrollment process.

Voxiva: The DeveloperOver 83% of Americans own mobile phones, and over 73% of phone owners send text messages, providing an opportunity to conveniently reach populations with helpful health information. Pioneer in the mobile health industry, Voxiva combines trusted health content and interactive engagement to improve health at lower costs. Utilizing multiple channels of engagement, Voxiva m-health products reach users through text messages, mobile websites, personal portals, and management dashboards.

MPCA: Program SupportMichigan Primary Care Association has partnered with Voxiva to offer the innovative diabetes self-management product, care4life, to member Health Centers and their patients for free. Along with making this tool available, MPCA is excited to provide on-site outreach and enrollment support, including staff training, to participating organizations.

Outreach The care4life program offers a number of outreach options that allow your organization to reach its patients in the most appropriate manner. These options range from simply providing patients with the free care4life informational trifold to utilizing social media outlets and organization newsletters to spread the word about this new tool!

The following list provides an overview of all of the possible care4life outreach options. These options can be used on their own or in conjunction with another to build the most appropriate outreach plan for your organization.

In partnering with MPCA to offer the care4life mobile diabetes self-management tool to your patients, you will automatically receive the FREE informational trifold to begin engaging patients. You will find this trifold, following the care4life outreach options.

□ Social mediao Post to Facebook, Twitter, and other social media outlets about the availability of care4life at

your organization

□ Health Center newslettero In your next newsletter provide a description of care4life and a contact at your organization for

patients to contact if interested in the program

□ Utilize staff – word of moutho Have staff participate in an enrollment/outreach web presentation to arm them with information

about the program, how to share with client and how to help clients enroll o Great for diabetes education and support group leaders, AmeriCorps members, as well as for

one-on-one patient provider contact

□ Print outreach materialso Use the print outreach materials provided by MPCA and Voxiva in your waiting areas, patient

education folders, etc.

□ Targeted direct mail campaign o Let MPCA do the work for you! Provide us with patient contact information over a secure file

share and we will send out 2 letters and an informational stuffer, targeted to your diabetic patients. These letters will include information about care4life at your organization, what the program is, and how to sign-up

o The letters will look as if they are coming from your organization, including your organization’s logo, ensuring that patients are familiar with the sender

Engaging PatientsThe Outreach and Enrollment presentation attached in Appendix A provides some additional information on engaging patients and would be a good resource for staff to review.

Depending on the outreach method your organization has chosen, engaging patients will take different forms. There are opportunities for your staff members to take an active role in engaging patients or for staff members to be more active as a resource or source of information.

Some options include: Care Managers introducing the care4life tool to their patients Sending out targeted direct mail introducing the care4life program and allowing patients to

follow up with their care provider Patient Navigators contact patients and introduce/enroll patients in care4life

Keep in mind that not all type-2 diabetic patients will be prepared to use a tool such as care4life.This tool has been designed for those 18 years and older, male or female, but is not recommended for pregnant women. Additionally, to receive the full benefits of this tool the patient would need regular access to a personal mobile phone with the ability to send and receive text messages. On the following page you will see a Patient Readiness Screener that can help identify an ideal care4life user.

Once the ideal patient has been identified, you can help them understand the care4life tool by discussing the features, such as medication reminders, healthy eating and exercise tips, and blood glucose test reminders. Following the Patient Readiness Screener you can find a list of real care4life text messages to aid you sharing this tool with your patients.

Glucose monitoring

Goal trackingcare4life. Time to check your BEFORE meal glucose. Reply with your BEFORE meal glucose reading (e.g. 125).

care4life. 7am med reminder: Sometimes you might feel overwhelmed. Remember to take it one day at a time. Focus on what you can do today.

care4life. Did you meet your goal of exercising at least 3 days last week? Reply with the number of days you exercised last week (e.g. Reply 3).

care4life. With clean hands, use the lancet device to poke the fleshy side of the fingertip, slightly off center from the middle of the tip (hurts less).

care4life. Time for your weekly medication check-in. Reply 1 if you took all your meds last week, 2 if you took most, 3 if you took some, or 4 if you took none.

Resources: There are many resources available to aid in your care4life outreach and enrollment efforts. Some of these resources include text shortcut cards that can be given to patients in order to help them use care4life more efficiently, or appointment reminder cards that can be included in patient mailings. Please refer to the contact list at the end of this toolkit for the appropriate person to contact in reference to any questions you may have.

Q. Does care4life cost money?

A. No. There is no cost to enroll in care4life.

Q. Will I get charged for text messages?

A. If you have an unlimited text message plan that includes sending and receiving text messages, you will not be charged any additional fee for using care4life. If you do not have an unlimited text message plan, you will be charged standard message and data rates by your carrier.

Q. What if I have pay-as-you-go or a pre-paid cell phone?

A. If you have an unlimited text message plan that includes sending and receiving text messages, you will not be charged any additional fee for using care4life.

Q. Is this service available to every mobile phone subscriber in the United States?

A. The majority of mobile carriers provide this service. It is available to over 96% of people with cell phones. You can find the full list of participating carriers here.

Q. How long is the care4life program?

A. Care4life is a 6-month program.

Q. What happens at the end of 6 months? Will I be charged any money?

A. Care4life is intended to be a 6-month program. At the end of 6 months, you will not be charged any money. You can continue to set medication, glucose tracking and appointment reminders. You will not receive any new education content. You will no longer be able to use any part of the program after 1 year.

Q. Is my personal information safe?

A. Information collected from you is used to enroll you in the care4life service so you can receive care4life messages. Your information is not sold or shared with any third parties. If your care4life subscription is being sponsored by an organization, the organization may have access to your data. Q. What happens if I'm sent a message but I'm in area where I don't have any reception?

A. Once you return to an area with reception, the message will be delivered to your mobile phone. Carriers will generally try for up to 48 hours to deliver a message to a phone.
