July 1, 2016Implementation funding will be disbursed

June 2-18, 2016Applications Reviewed

• For those sites concerned about a gap in funding, apply by the 1st Review Cycle submission date

• Note: this is not a rolling application

Required Materials

Include a narrative description that describes the following:

• Strategy, goals, and tactics to address the social determinants of health

• Interim benchmarks, key milestones and overall timeline of implementation efforts

• Anticipated barriers to implementation and how you intend to respond

Key questions include:

• What data has already been used to inform your strategies?

• How will data be used and shared in new ways to identify key needs and opportunities, as well as to track progress, measure outcomes, and inform continuous improvement?

• What data-sharing agreements are currently in place or in development?

• Sustainability of the 1:1 Match

• Strength and likelihood of success of the Implementation Plan

• Achievement of Planning goal outcomes

• Alignment with BUILD pillars

• Engagement of the primary partners throughout the planning year
