After filling out all required fields and entering Lucas and Ohio (OH) as shown, select continue

Please select “no” for each question, then select continue

Please select other, then continue

Please select organization, then continue

Please select Social or civic, type social into the box, then continue

Please choose “receive letter online.” If you do not have adobe reader installed please install by clicking the blue Adobe Reader link, then continue
