Missing plugs or incorrect plugs were used. No easily visible marking system in place to identify which lines required plugs. Procedure for checking plugs at start-up not being consistently followed by operators or maintenance personnel. Operators were not aware that specific types of plugs must be used depending on the size of line. Not plugging lines can create a fugitive emissions

Contaminated with various levels of hazardous materials – from routine oils and lubricants to organic acid that can cause injury with contact
