DEPARTMENT CHAIR - 2015-2016 IN REVIEW

Our community involvement has substantially increased with the hiring of additional physicians and advance practice
providers (APPs) to augment the care provided in the HCMC network of neighborhood clinics. We now have a total of 74
MD and APP providers working within the department at 6 clinics scattered throughout metro Hennepin County.
Within the Family Medicine Residency Program, we said goodbye our PCBH psychologist James Anderson, PhD, and
hello to Marchion Hinton, PhD. All of our Core Teaching Faculty have completed the U of MN Faculty Development
Fellowship and several Faculty members and residents presented at the National STFM meeting held in Minneapolis in
May. Chandra Cherukuri will be starting as a teaching faculty member in early August. Chandra is a 2014 graduate of the
HCMC FM Residency program and has spent the last two years in geriatric and sleep fellowships. Our residency program
recruitment has been successful yet again and a full complement of residents started their training this past June.
At Whittier Clinic, the APT program has completed its move to Whittier from East Lake Clinic to be closer to their
patient base and more actively involve residents in the unique care model it provides. We continue to expand the services
we provide to our patients. Cardiology, General Surgery, OB-GYN, Chiropractic, Acupuncture, Podiatry, Psychology,
Pediatrics, Lactation Education and Hand Therapy all have a presence at Whittier. We model ourselves as a place where
our patients can obtain as many of their health care services as possible. This is only possible through the generous
support of the HCMC departments that send specialty care providers to practice here.
I would like to thank all that have made the past year an exciting one, as we say goodbye to old friends and hello to new
ones. We look forward to the coming year, as we offer outstanding services to our patients, a rewarding education to our
residents and interesting challenges to our providers.

LETTER FROM THE PROGRAM DIRECTOR
I am pleased to share with you the Annual Report of the Department of Family Medicine at Hennepin County Medical
Center. The report is a summary of our activities and accomplishments from July 1, 2015 to June 30,2016. We are
pleased to continue to build on our reputation of excellent clinical training, community involvement and advocacy, quality
and safety training and developing leaders for Family Medicine.
Our residents have the privilege of providing comprehensive care for the most vulnerable of our patients at Whittier
Clinic, a certified Minnesota Health Care Home. There they participate in creating a culture of health for all patients in
their care. They participate in LEAN based Quality Improvement methodologies ; provide population care through their
use of patient registries ; participate in a group model of care in Centering Pregnancy and work with interdisciplinary
teams in complex care clinics for adults and adolescents. This year our educational development focused on
implementing structured curricula in these areas.
Our PGY3 residents continued to work with underserved populations both locally and internationally with groups pf
residents travelling to the Phillipines, Thailand and Tanzania. Residents developed confidence in putting their many
skills as family doctors to work in these resource poor areas. Each returned with an renewed dedication to their specialty
and a commitment to bettering health care for people everywhere.
Both our residents and faculty participated in the Society of Family Medicine’s Annual Educational Conference which
was held in Minneapolis in May of this year. 7 residents presented posters and 11 faculty presented scholarly roundtables,
research seminars, lecture discussions or posters. The residency program also sponsored the Fun Run (also known as the
Marathonaki) which was held in downtoen Minneapolis and scenic Loring Park.
At the end of the academic year, the HCMC Family Medicine and Whittier family were deeply saddened by the sudden
death of our beloved Dr Masha Hemmati. He was a core faculty member who worked with residents in the nursing home,
the clinic and in the hospital -both on the Family Medicine Service and on Labor and Delivery. Dr Hemmati will forever
be remembered for his work ethic, his humor; his dedication to the profession of medicine; his courage in the face of
adversity and his advocacy for his residents Most of all, he is remembered for his fierce commitment to his beloved wife
and son. In his own words. “To Allah we belong and to him we return.” Rest well dear friend and colleague.

Dedicated to our beloved Dr. Masha Hemmati
1955-2016
Dr. Mashallah Hemmati was a Family Physician here at HCMC, joining the department as a resident in 2001. After graduating from the residency program in 2004, he worked at Richfield Clinic and later moved to FMC and Whittier. While maintaining a busy primary care practice, he also took an active role in teaching residents at the hospital, at a local Nursing Home, and in the clinic, with special expertise in minor surgery and urologic issues. Dr Hemmati died of natural causes in June 2016. He is survived by his beloved wife Miriam and his son, Pouja.

The Department of Family Medicine strives to be an educational center of excellence
for family physicians who are competent in caring for people of diverse cultures,
committed to serving their community, and capable of practicing in a wide variety
of settings.
The mission of the Department of Family Medicine is to educate Family Physicians
to become leaders in Family Medicine and Community Health, and to serve our
diverse urban community.
Our values guide and inspire us to do our best as we provide care and medical

PROGRAM EDUCATIONAL GOALS AND PHILOSOPHY
Patient Care Skills
Graduates must be able to collaborate effectively to provide patient care that is
compassionate, appropriate and effective both for the treatment of health problems
and the promotion of health and wellness.

RESIDENCY PROGRAM HIGHLIGHTS
FACULTY Our tireless faculty are the true heroes of this residency program. They are a dedicated
group who do more and more with shrinking resources. They continue to provide
exceptional service in a climate of stringent duty hours and increasing demands as core
faculty.

Research and Scholarly Activity May of our faculty continue to provide mentorship for residents in the ara of research
and scholarly work. The in Minneapolis this year and my of our residents took
advantage of the opporetunity to display scholarly work and posters .

Our residency program had the honor of sponsoring
the Annual Marathonaki put on by STFM every year.
The run/walk was held on Tueday from the Hilton
Minneapolis from Nicollet to Loring and back
through the Sculpture garden. Warmest
congratulations to the residents and faculty who
organized and participated in the glorious event held
on a picture perfect spring morning. Kudos to: Jarrod
Yamanaka, Jon Koretkoff, Charles Anderson, Nancy
Newman, Michelle Karsten and Maria Kaefer.

Outcomes:
33% of the class of 2019 have a tie to Minnesota
Class of 2016 - -3 (27%) took jobs in Minnesota
100% boards pass rate since 2013
Classes of 2016, 2017, 2018 and 2019 are all racially and ethnically diverse
42 % of the interns who entered in 2016 are graduates from US allopathic or osteopathic
schools
The recruitment Committee led by Dr. Susan Hasti will continue its effort to recruit the
candidates who “best fit” our mission, vision and goals. Recruitment efforts are focused
at the National Conference held in Kansas City every year and the Family Medicine
Midwest Conference.
CURRICULUM HIGHLIGHTS
Recruitment was a high priority again for the academic
year. Sustaining program quality and obtaining successful
program outcomes depends in part on the quality of our
residents. The Recruitment committee maintained the same
goals for the 2015-2016 applicant season:
recruit highly qualified applicants,
increase the % of US grads,
restore the gender balance ( has been weighted
toward male),
increase the number of residents who remain to
practice in Minnesota,
and maintain racial and ethnic diversity in our
program as befits our diverse patient population
Centering pregnancy curriculum Staring in the spring of 2013, residents
participated in the existing Centering Pregnancy
groups. PGY2 residents are assigned to each
centering group. In the upcoming academic year
the Centering pregnancy faculty will deploy a
curriculum that prepares residents for skills that
they need to facilitate and implement group
visits based on the Centering model. Faculty
leaders are Michelle Karsten and Susan Hasti.

RESIDENT TRAINING AND EVALUATION IN THE SIMULATION CENTER –
We had our firs simulation Chief this year,. Ben Krage One of the Chief residents
together with Drs Michelle Karsten and Kim Petersen developed a unique curriculum
for Family medicine Residents to be deployed in the Simulation center. Residents have
Sim training every time there is a 5th Wednesday in the month. Our faculty serve as their
preceptors with help from the Educational team in the HCMC Sim Center .

The Community and Population Health Education Committee was formed this
academic year and has been hard at work creating a population health framework that
spans the three training years of residency. The curriculum prepares residents to provide
total health care for all patients whether they are seen in the clinic- the Patient Centered
Medical home; the community in the Community Centered Neighborhood or whether
they are part of the global population. Four themes are embedded in each rotation that
informs population and Community health. They are Health; Patient Centered
Centeredness, Team skills, Scholarly projects.
Health Equity Leaderhsip
Third year residents participate in a capstone curriculum called Baraza. As part of the
Leadership Curriculum, the class has been learning about Advocacy as a tool to
empower our communities and stimulate changes in policies and laws to benefit
underserved population. Advocacy is empathy at work. Residents together with Dr
Svetaz supported the Hennepin County Policy Deparment in a bill to provide
preventative care services to undocumented minors. Residents visited the legislature to
lobby memebers of both houses to support the bill.

International Medicine rotations
Many physicians are interested in providing medical care to people of other countries and
other cultures. The HCMC Residency program is committed to creating opportunities for
international experiences for interested residents and faculty to serve in resource poor,
underserved areas of the world. Our ONE WORLD, ONE HEALTH perspective seeks to
create a passion in our residents for service in underserviced areas through medcial
missions. Through these medical missions, our hope is that we engender in our residents
a responsibility for the health of people not only in our neighborhoods, but the health of
all underserved. Ingrid Wong and Trevor Chan anticipate an extraordinary opportunity in
the Philippines, and Suchithra Narayan is planning a rotation in Chang Mai,Thailand.

The international rotation was remarkable because it incorporated something that we had always wanted to do. To give back to the underserved was not only a privilege but also an unique educational experience in a third world country where there is a lack of medical care. We were able to utilize our practical, broad based family medicine skills to not only educate patients and better their health, but also utilize our procedural skills to provide in a sense, a cure to ailments that disfigured the patients. OTHER NEWS
MDH grant funding to increase physician primary care workforce
The HCMC Family Medicine residency program received funding from the
Minnesota Department of Health to increase the size of the resident class. The
money received will fund the training of two residents who entered in 2016 and will
graduate in 2019. The grant is an incentive to address the projected deficit of
Primary care physicians in Minnesota.

CENTER FOR HEALTH EQUITY LEADERSHIP (CHEL) The Center is conjoint effort among Aqui Para Ti, the Department of Family and
Community Medicine, and the Family Medicine Residency Program. The Center, which
resides in the Department of Family Medicine at Hennepin County Medical Center, has
as its core the achievement of the Aqui Para Ti (“Here for You”) Program, a model
program of integrative care.
CHEL accomplishments
• Created, with a team of 3rd year Family Medicine Residents and Public Health Students,
a White Paper on Equity, used to support 2016 HCMC’s advocacy efforts to expand
Preventive Care access to undocumented families living in MN.
• Prepared a one day Policy Summit, where residents lived advocacy, talking and sharing
that White paper on Equity with MN Legislators.
• Worked with the State Legislature, testifying 3 times at our MN State Legislation, to
pass Health Home model of care and reimbursement as a MN approved model, using
Aqui Para Ti results.
• Helped pass an amendment assuring that Health Home models are used intentionally to
help to remove Health inequities in Minnesota .

This year was marked by a great transformation at APT. The clinic was moved to
Whittier Clinic to be:
in closer contact with our residents,
for our youth and families to have more access to services, and
to bring more cultural concordance to the largest HCMC clinic serving Latino
families.
2016 Society for Adolescent Health and Medicine ( SAHM) Annual Conference,
APT led a two day Conference in Family Centered Adolescent Care,
presented 7 sessions at the National Conference of the largest National and
International conference on Adolescent Health,
2016 Annual Educational Conference of Society of Teachers of Family Medicine
(STFM)
APT faculty and staff presented 8 sessions at the Society of Teachers on Family
Medicine 2016 Annual Spring Conference.
APT was certified as one of the only two Primary Care Behavioral Homes in a pilot
sponsored by the Department of Health Services(DHS). Together with CHEL, Aqui Para
Ti is starting a pipeline for Underserved Teens: “From Clinic to College Pipeline”,

where teens will team up with PG3 residents to be mentored in a healthcare pathway and
to do together Community work.
APT has also published several articles, in Pediatrics, and Family Practice Management,
among others.

Whittier Clinic High School
Internship Project Jarrod Yamanaka, M.D., second-year resident at Hennepin
County Medical Center’s Family Medicine Residency
Program, will partner with FAIR School in Minneapolis to
introduce high school students to careers in the medical
profession. Jarrod and his team will give a tour of the clinic
to high school students; work with students in the classroom
as they learn about anatomy and physiology; and select one
high school student for an in-depth summer internship to
help enhance the connection between Whittier Clinic and
its surrounding community. Jarrod’s mentors are Kimberly
Peterson, M.D. and Susan Haddow, M.D.
Rest and Renewal (R&R) Group Visits in an
Underserved Population with Diabetes and
Depression and/or Anxiety Ese Aghenta, M.D., and Randolph Borrero, M.D., second year
residents at Hennepin County Medical Center Family
Medicine Residency Program, will lead a six-week course for
three groups of female patients on Medicare with diabetes
and a mental health diagnosis. Patients will learn therapies
which address stress resilience, sleep optimization, low
glycemic diet, healthy movement, mindfulness mediation,
and connection to self and others. Mentor Kara Parker,
M.D., will oversee Ese and Randolph’s innovation project.

Year of Graduation: 1982
Why I came to HCMC: I chose HCMC because I knew I would be able to truly care for patients
with a variety of conditions including acute trauma and serious illness; with most residents
spending most of their time at HCMC there is an unusual level of support amongst residents of
different specialties. I have been precepting at the HCMC Family Medicine Residency since
1984.
Influential moment and inspirational person: I had the privilege of having Dr. Harley Racer as
a staff preceptor during residency and then teaching alongside him for another 25 years after
graduation. He taught all of us to look beyond the disease to the entire person, and then beyond
the person to the family, and even beyond that to the community. His spirit remains in the many
faculty that follow his example at HCMC's Family Medicine Residency....
What I am proud of: I am proud to work at two institutions that value persons, at any level of
employment or illness, more than economic goals or statistics; HCMC and Mille Lacs Health
System.
Where I practice now: I am now VP of Medical Affairs, but mostly just a regular Family
Physician, at Mille Lacs Health System in Onamia, Minnesota. We have managed to survive the
difficulties of rural medicine and stay independent of the larger systems that are gobbling up
many rural practices. We have grown from two clinics to five, and offer the full spectrum of
Family Medicine to an over 400 square mile area.
What I do outside medicine: The best thing I do is enjoy my three grandkids, who are even
more fun than my four daughters. I go twice a year to Haiti to help staff our Mille Lacs Health
System rural health project, "Santo Clinic"; I am busy with music at our church; enjoy wilderness
canoeing in Canada.
What is best about Minneapolis: Of course, my three grandkids. I also enjoy the jazz clubs,
Guthrie Theater, and the fantastic restaurants on Eat Street right by Whittier Clinic.

I can't believe it has been 5 years since completion of residency, time flies. I am doing great here.
I am currently at Duke Primary Care - Wavery Place in Cary, NC. It’s alot of bread and butter
primary care - DM, HTN, Lipids, Afib, CHF, CAD, gyn and pediatrics. I am also the only one
who does procedures at the office so its fun. I do IUDs, Implanons, toenails, lacerations, I&Ds,
joint injections. I also teach NP and PA students. It’s truly amazing, how much I learned from
our GREAT residency program. You and the rest of the staff should be very proud of having an
excellent program, that makes you ready for success on Day 1 after completion of residency.

My family is doing well. I would love to speak with the residents if I am back in
Minneapolis. One of the aspects about Family Medicine and my training at HCMC was that I was
able to work as 1. Primary care physician 2. Hospitalist 3. Now I'll be starting work as an ER
physician as well. Which other specialty allows one to be able to work in such various positions?
Of course, I could do OB as well, but I hope to never have to do another delivery.

The 2016 Harley Racer Medical Education Award was given to Dr.
Uche Nwaononiwu. The award is presented in the memory of Dr
Harley Racer, past faculty of the residency program. Dr Racer was
dedicated to the creation of family doctors of excellence. He inspired
many of the faculty who were honored to work with him.
Dr Nwaononiwu was recognized for his dedication to research and
scholarly activity and for his creation of an environment of inquiry in
