Goal #1:

To create a system where those receiving health services and supports can move as smoothly and seamlessly as possible between different areas of the health system, and that access is

improved.

Goal #2To ensure effective and efficient

delivery of supports to keep people healthy, in the areas of health promotion, injury and illness

prevention, screening and early detection, vaccinations, home visits, nutritional counseling, home care,

health information and more.
