This post offers the opportunity to join the AHP Out Patient - Department of Nutrition and Dietetics and to support the Eating Disorder service. The main focus is to provide dietetic support, advice and intervention for both adults and young people with complex Eating Disorders. The post requires an experienced Dietitian with relevant or transferable skills that will enhance both the specialist and support aspects of this post.

The post will be based in Stirling Community Hospital, although a degree of flexibility will also be required. The postholder will work with other members of the department to ensure a whole department approach and contribute to our annual objectives and staff development programme.

2. JOB PURPOSE To develop and deliver the specialist nutrition and dietetic services* within

range of clinics and community settings. To coordinate, plan and evaluate all activities necessary for the

organisation and progression of the Nutrition and Dietetic services within a Clinical Governance framework.

To contribute to service specific and wider strategic issues including policy development, implementation and service planning.

To utilise expert knowledge of current research, guidelines and practice in order to ensure that all service users have access to relevant specialist, evidence based treatments that maximise outcomes.

To progress service through leadership and multi agency partnerships and networks.

To act as source of clinical expertise in addition to providing training and support for colleagues, other health care professionals, care and education staff on relevant complex aspects of the service.

To participate in the delivery of the departmental clinical placement programme for pre-registration dietetic students

3. ORGANISATIONAL POSITION – appendix 14. DIMENSIONS

NHS Forth Valley has a population of 280,000 and health services are delivered by 1 Acute DGH,

1 CHP, Integrated teams, 4 Community Hospitals and 56 General Practices and 70 Care Homes

Forth Valley Dietetic Services are delivered from both the Acute and Out Patient Service AHP

Care Groups using joint care pathways.

*This Locality Post will work closely with the Band 7 Dietetic Advanced Practitioner in Eating Disorders and will be supported within the North team structure. The main focus of the post is a combination of the following specialist roles.

In addition, the post holder will contribute to :

Staff : Support and guidance of other staff members in specialist area

Supervision of junior, assistant and clerical staff as required

Students: Participation in integrated clinical placement programme including 6xA , 4 x B and 4 x C placements (12 week clinical placements)

Practice Participation in practice improvement, joint CE meetings andDevelopment : Clinical Governance activities as required

Staff Support and development of training and educational requirements for dietetic

Training : and other staff groups including HCSW, care assistants, local authority and voluntary sector staff.

out patient dietetic services using experience, high level skills and breadth of knowledge required for this expert area of practice

To act as an expert/specialist source of information on food and health issues for those working with groups or individuals within the designated setting e.g. other dietetic colleagues, partners in Health, Local Authority, the voluntary sector, as well as providing nutritional/dietetic advice to projects for National organisations such as NHS Scotland and the Nutritional Diet Sheet Initiative.

To initiate and participate in research, audit and evaluation of health promotion activities especially in relation to nutritional issues within the setting.

Contribute to implementation and monitoring of clinical governance aspects of specialist services

Take lead on clinical audit for specific specialities while ensuring that support and sharing mechanisms are in place for other less experienced staff

Develop and maintain networks with local partners and other specialists in same area of expertise

Support, supervise and contribute to development of junior and support staff

7. ASSIGNMENT AND REVIEW OF WORKAccountability is directly to the Team Lead for all clinical activities and support will be available through Lead for Eating Disorders Service and all activities will be monitored through the departmental performance framework including regular objective monitoring and annual activity planning.The post holder will be supported by the Dietetic Coordinator and/or Clinical Lead in designated strategic and operational aspects that are determined by service demands, constraints and the local implementation of national guidelines and recommendations. As a senior member of the dietetic department and range of multi-disciplinary teams, the post holder will make high level decisions relating to dietetic conditions, patient care plans, inclusion of other health professionals and seeking peer support only as and when necessary. There will be a frequent requirement to use discretion and professional judgement, concerning the appropriateness of specific individual dietary treatments and safety aspects of care and delivery that can impact on patient, self and other staff. In addition there is a requirement to prioritise own workload including competing demands from clinical and non clinical commitments, decisions regarding progress of patient referrals and the necessity to reinforce appropriate referral mechanisms based on available dietetic resource.Clinical supervision of junior, assistant staff, clerical staff and students will be required on an on-going basis and support for other specialist and community staff will be required through ad hoc support mechanisms and programmed training. 8. COMMUNICATION AND WORKING RELATIONSHIPSThe post holder will develop systems and encourage an environment that requires cooperation from individuals, colleagues, support staff and external agencies. A full range of advanced interpersonal, educational and motivational skills are required to maximise patient care and support junior staff through the performance framework. Techniques are required which permit the acquisition and assessment of information that will impact on the success of goal setting and therapeutic intervention while ensuring health literacy. The requirement to regularly interpret and deliver confidential information that relates to sensitive and possibly contentious matters and situations will be necessary. Regular exposure to distressing and demanding situations are necessitated by the chronic nature and sensitive needs of the caseload and differing opinions of associated care team. The ability to deal appropriately with range of debilitating disorders, terminal care, frustrations, emotional or aggressive reactions is therefore required. Effective communication through listening, questioning and presentation techniques, while participating in team interactions are essential elements which facilitate care delivery and change management while progressing departmental and personal development. Opportunities to promote the profession and nutritional care should be identified, evaluated and implemented appropriately.In addition, there is a requirement to engage with other health professionals and external agencies through a complex network of contacts and ability to understand a range of roles, terminology and use of a varying communication formats in order to maximise patient care, ensure service equity while coaching and supporting range of staff.
