Enrolling in Benefits Coverage

New Hires: How to Enroll: You will receive an email invitation to make your benefit elections

from Your Benefits Center within seven days from your date of hire 401(k) enrollment information will sent by Fidelity Investments separately via email separately

Deadline to Enroll: 30 days from your date of hire When Benefits Coverage Begins: First of the month following your hire date.

Medical Plans

Aetna Choice of two high deductible PPO medical plans with a Health Savings

Account (HSA)

2000 Plan with HSA Lower payroll contributions than the HSA 1350 Plan

Deductible and out-of-pocket maximum is higher than the HSA 1350 Plan

1350 Plan with HSA Higher payroll contributions than the HSA 2000 Plan

Deductible and out-of-pocket maximum is lower than the HSA 2000 Plan

Employees (1) enrolled in Medicare; or (2) enrolled in Medicaid; or (3) that have medical coverage through Tricare; or (4) that have received VA benefits within the past three months and do not have a disability rating from the VA are eligible for the 2000 and 1350 medical plans without a Health Savings Account (HSA). These plans have the same provisions as the 2000 and 1350 medical planswith an HSA except there is no HSA with these plans (and consequently, no HSA employer funding). If you meet at least one of the above four criteria, contact Your Benefits Center for more information on the 2000 and 1350 medical plans without an HSA Plans.

The plan covers 100% of all preventive care services, such as annual physicals and screenings, and child immunizations at no cost when using an in-network provider.

You pay the full cost of covered Medical and Rx services up to the deductible amount .You can use money in your HSA to satisfy the deductible.

Once you meet the annual deductible, you share in the cost of Medical and Rx services by paying the co-insurance. You can use money in your HSA to pay these expenses.

You pay co-insurance until you hit the out-of-pocket maximum for the year. Then, the plan pays 100% for covered medical expenses, and you pay nothing.

1. The HSA employer funding is subject to completion of a health assessment and/or a biometric screening. If the employee completes a health assessment, the employer contribution to the employee’s HSA will be: $200 (if the employee is only covering himself/herself under the medical plan – i.e.employee only coverage) or $400 (if the employee is covering himself/herself and other family members under the medical plan—i.e., family coverage). If the employee completes a biometric screening, an additional employer contribution will be made to the employee’s HSA of $200 (for employee only coverage) or $400 (for family coverage). Amounts will be prorated for midyear enrollees.

2. Out‐of‐network expenses are subject to reasonable and customary charges.

2018 HSA Contribution Limits: The IRS sets limits on the amount that may be contributed each year. For 2018,

these limits are: $3,450 for employee only coverage

$6,900 for employee + dependent(s) coverages

If you are age 55 or older, you may make an additional catch-up contribution of $1,000.

Contribution limits includes both employee and employer contributions.

Limit does not include catch-up contributions, which can be made in addition to the IRS-allowed maximum.

Hospital Indemnity Plan (HIP)

Supplements your medical plan and pays a lump-sum cash benefit plus daily benefits directly to you for a covered inpatient hospital stay

You can use the cash benefit to help pay for your medical plan deductible, coinsurance or everyday living expenses

There are two Hospital Indemnity Plan options: Aetna HIP 2000

Employees 50 or older may make additional “catch-up” contributions up to $6,000 for a total of $24,500 in 2018 If you contributed to a prior employer’s plan in 2018, those contributions, along with the

contributions you make to the Corporate Risk Holdings 401(k) Plan count towards the IRS limit

If you reach the IRS maximum limit with your Corporate Risk Holdings 401(k) contributions, the payroll systems will automatically stop your contributions and the corresponding employer match for the remainder of the year Please note: The payroll system will not include any contributions you may have made during the calendar year

Parental Leave New fathers and mothers are eligible for five days of paid leave at 100% of

salary following the birth, adoption or placement of a child.

For new mothers who give birth, paid parental leave may be used during the first week of short-term disability (the waiting period).

Community Service Time-off to volunteer for one day (8 hours) per calendar year (whole of half

day increments).

Employees regularly working 20 hours or more per week are eligible.

The community service organization must be a 501(c)(3) non-profit organization, Company-recognized volunteer activities or a child’s school-related activities.
