Por  favor  de  recoger  el  paquete  de  inscripción  en  la  escuela  que  le  corresponde  a  su  hijo.  

Cuando  regrese  el  paquete  de  inscripción  a  la  escuela  que  le  corresponde,  también  necesitara  traer:    

 •   Cartilla  de  vacunas  del  niño  •   Acta  de  nacimiento  del  niño  •   Comprobante  de  domicilio    

       (Eje:  Factura  de  luz  o  renta–que  muestre  su  nombre  y  domicilio)    •   Contactos  de  emergencia    

       (2  nombres,  domicilio  y  número  de  teléfono)

Please  pick  up  a  registration  packet  at  your  child’s  boundary  school  

When  returning  the  completed  registration  packet  to  your  child’s  boundary  school  you  will  also  need  to  bring:  

 

•   Child’s  immunization  record  •   Child’s  birth  certificate  •   Proof  of  residence    

           (Ex:  Utility  or  rent  bill  –  showing  name  and  address)  •   Emergency  Contacts                (2  names,  addresses  and  phone  numbers)
