GENERAL INFORMATION

LOCATIONThe 149th Annual Session of the Ohio Dental Association will take place at the Greater Columbus Convention Center (GCCC) complex, Columbus, Ohio at 400 N. High Street, 43215. The headquarters hotels are the Hyatt Regency Columbus, 350 N. High Street, Columbus, Ohio 43215, (614) 463-1234, and the Hilton Columbus Downtown, 401 N. High Street, Columbus, Ohio 43215, (614) 384-8600; both are connected to the GCCC. Most ODA Annual Session events and courses will be held in the GCCC, the Hyatt or the Hilton. ODA staff will be on-site at the GCCC from Wednesday, September 16 through Sunday, September 20.

FREE CE COURSESThe ODA Annual Session registration fee includes the ability to earn up to 11 free CE credits. Free CE courses are offered on Thursday, Saturday and Sunday of Annual Session – see the CE course listing for details. In addition, attendees have the opportunity to earn CE credits while attending the Table Clinics on Friday in the Exhibit Hall. All of the free courses offered, as well as the Table Clinics, qualify toward the Ohio State Dental Board’s biennium licensure requirement. The registration fee also provides unlimited free entrance to the expansive Exhibit Hall, the Welcome Reception, The Bash!, Friday and Saturday Exhibit Hall events, daily door prizes in the Exhibit Hall, registration incentive drawings for Taylor Swift concert tickets, and the special drawing for two premiere tickets for the September 19, 2015 OSU football game.

NEW DENTIST DISCOUNTODA members who have been practicing for less than 10 years will receive a one-time $50 discount on any paid CE course. The discount will automatically be deducted from the first CE course you add to your registration. The discount is applied to only one course and information provided by each attendee is matched with ODA Membership records to validate the discount.

SPECIAL NEEDSIf you have special needs and require accommodation to fully participate in the conference, please contact the ODA Annual Session department at (614) 486-2700 prior to Annual Session, and the ODA Information Desk located in the Greater Columbus Convention Center (GCCC) Concourse C during the conference. The ODA complies with the Americans with Disabilities Act requirements for accessibility for persons with disabilities.

TAX DEDUCTIONDeductions are permitted for educational expenses - including registration fees, transportation, meals, and lodging. Check with your financial advisor for details.

TWO-HOUR LUNCH BREAKSOn Thursday, Friday and Saturday, most CE courses will break for two hours for lunch at various intervals between 11:30 a.m. and 2:00 p.m., giving attendees the opportunity to spend time in the Exhibit Hall with no need to travel off-site for lunch – concessions and a dining area are located in the Exhibit Hall and in the main corridor outside the Hall.

SEMINAR SEATING & COURTESYPlan to arrive early: seating for all seminars is on a first-come, first-served basis. Also, please set all cell phones and pagers to “silent” during seminars. Seminar room locations will be included in the pre-registration packet mailed to attendees who register by August 3, 2015.

ATTIREBusiness casual dress is recommended for the convention. Dress warmly, in layers, as meeting rooms tend to be cool.

PHOTOGRAPHY RELEASEAnnual Session attendees grant permission to the Ohio Dental Association and their agents to utilize, in perpetuity, the attendee’s image or likeness in an effort to promote the Ohio Dental Association and its events or meetings. Attendees waive any right to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied.

CREDIT HOURSThe number of continuing education (CE) credits for each seminar is equal to the number of course hours for that class. However, not all course topics meet the mandatory requirement for renewal as mandated by the Ohio State Dental Board (OSDB) or other licensing agencies. It is the responsibility of each licensee to understand the specific CE rules of their respective licensure board.

Attendees will receive a code during each course and will enter that information at one of the CE printing stations to print their own personalized CE certificates prior to leaving the meeting. COURSE HANDOUTSIn advance of the meeting, course handout materials will be accessible to course registrants online. These materials may be downloaded and printed. Not all speakers provide handouts for their presentations. Registered attendees will be able to print/download available handouts for their specific courses starting August 17, 2015 and materials will be available online through October 31, 2015. Handouts will not be distributed at or during the courses.

OSDB CONTINUING EDUCATION CRITERIA The Ohio State Dental Board (OSDB) defines acceptable continuing education for all Ohio dental licensees as educational and scientific courses given by board-approved sponsors consisting of activities designed to: review existing concepts and techniques; to convey information beyond the basic dental education; and, to update knowledge on advances in scientific, non-clinical and clinical practice-related subject matter, including ethics, regulatory compliance, risk management, nutrition, and evidence-based dentistry. The objective of continuing education is to improve the knowledge, skills and ability of the individual to provide the highest quality of service to the public and the profession. Ohio licensed dentists must complete 40 hours of acceptable continuing education each biennium. Ohio licensed dental hygienists must complete 24 hours of acceptable continuing education each biennium. Dental licensees are free to select areas of study within the OSDB’s stated categories, not to exceed the maximum number of hours in each category.

OSDB Category A: Includes acceptable continuing education for all Ohio licensees defined as educational and scientific courses presented in a directly interactive presentation format. There is no limit to the number of CE hours a dentist or hygiene licensee can obtain in this category during each biennium.

HARGREAVES, DDS, PHD Dr. Hargreaves is currently professor and chair of the department of endodontics, and professor in the departments of pharmacology, physiology and surgery at the University of Texas Health Science Center at San Antonio. He received his DDS degree from Georgetown University School of Dentistry, PhD in physiology

from the Uniformed Services University of the Health Sciences in Bethesda, and Certificate in Endodontics from the University of Minnesota. He is a Diplomate of the American Board of Endodontics. His area of research focuses on the pharmacology of pain and inflammation. The research approach of Dr. Hargreaves and his colleagues include clinical trials, animal studies and in-vitro tissue studies. He has received the National Institutes of Health Method to Extend Research in Time Award for pain research, and two International Association for Dental Research Distinguished Scientist Awards—for pulp biology in 1999, and pharmacology, toxicology and therapeutics in 2000. STEPHEN POSS, DDSDr. Stephen Poss is a graduate of the University of Tennessee and maintains an aesthetic-based practice in Brentwood, Tennessee. SASHA ROSS, DMD, MSDr. Sasha Ross grew up in Cleveland, Ohio where she graduated from Orange High School. She completed her Bachelor of Arts degree at Northwestern University in Evanston, Illinois as a chemistry major and anthropology minor. She then trained at Harvard School of Dental Medicine (HSDM) in Boston,

Massachusetts where she earned her DMD degree. Immediately after HSDM, Sasha enrolled and earned her Certificate in Periodontics in the College of Dentistry at the University of Illinois at Chicago (UIC). While at UIC, she also obtained a Masters of Oral Sciences. Her research has been presented at various international conferences and was published in the Frontiers of Oral Biology. In 2012, Sasha earned her Diplomate of the American Board of Periodontology. Sasha currently holds a seat on the Board of Trustees for the Greater Cleveland Dental Society (GCDS). She served as a co-chair for the GCDS Women in Dentistry Dinner in 2013 and 2014. She is also an active member of the American Dental Association, Ohio Academy of Periodontists, Alpha Omega Dental Fraternity, and the Harvard Alumni Association of Cleveland. SHAUN A. ROTENBERG, DMD, MSDr. Rotenberg is an Assistant Professor for the Division of Periodontology at the Ohio State University College of Dentistry in Columbus, Ohio. He also maintains a Private Practice in the Columbus area focusing on periodontal therapy, implant therapy, and periodontal plastic surgery.

Dr. Rotenberg serves on the Board of Directors of the Columbus Dental Society and is President of the Alpha Omega Columbus Alumni Chapter. Dr. Rotenberg, a Diplomate of the American Board of Periodontology, received his dental degree from the Medical College of Georgia and achieved his certificate in Periodontics and Master of Science degree from the Ohio State University. Medicine degree at the University of Pennsylvania. In addition to clinical periodontics and implant dentistry, Dr. Serra has also completed research investigating the link between inflammatory periodontal disease, gingival overgrowth and certain medications commonly prescribed to treat high blood pressure, seizures and reduce the risk of rejection in organ transplant patients. WILLIAM YANT, DDSDr. Bill Yant maintains a general dentistry practice in the mountains of western Maryland. He graduated from the Baltimore College of Dental Surgery, University of Maryland in 1992. At that time he joined a group dental practice and is now the senior partner with two other general dentists. Living in a rural community,

the practice strives to offer state of the art dentistry. Dr. Yant has completed the Dawson Academy and is an alumni member. He has completed externship in dental implants at West Virginia University and an extensive periodontal surgical continuum for the general dentist at the University of Pittsburgh dental school. He has been involved with CAD/CAM dentistry since CEREC 2 and has served for many years as a regional trainer for CEREC in the Pittsburgh branch area. REGISTRATION FORMYou can register your entire office on one registration form and pay for everyone with one form of payment! It’s simple: Fill out the primary registrant’s information including the name of your dental practice. Then list the name of everyone who is registering – including the dentist – and their course and event choices. If you need to add more courses or events than there is room for on one line, just take up two lines for one registrant. You can send in as many forms as you need – just photocopy the form. In selecting courses, please select the appropriate fee for each attendee: M=ODA/ADA Member Dentist; NMD=Non-member Dentist; S/O=Hygienists, Dental Assistants, Office Managers, Front Desk staff & all other registrants. Remember, you must register for any free CE course that you want to attend, but you do not need to register for the free events. A ticket for the Table Clinics, the BASH!, Exhibit Hall door prizes for dentists and staff, the special drawing for two premiere tickets for the September 19, 2015 OSU football game and any appropriate give-a-ways will automatically be included with your registration*. Registration fees are waived for dental hygiene and post-secondary dental assisting classes attending Annual Session as a group. Every group of five students must be accompanied by an Instructor or appropriate chaperone while in the Exhibit Hall. The pre-registration fee for post-secondary student dental assistants attending individually is $5, and increases to $15 on-site. RETIRED LIFE MEMBER REGISTRATIONODA members in the Retired Life category do not pay a registration fee if registered before the pre-registration deadline. A discounted $35 registration fee applies after the pre-registration deadline of August 3, and that fee increases to $45 on-site. Dentists in this category can audit any CE lecture course at no charge, space permitting; but to receive CE credit for the course, the member must pay the posted course fee.

SPOUSE REGISTRATIONSpouses of ADA member dentists who do not work in the dental office may register for free. MEMBERSHIP OFFERNon-member dentists may apply a portion of the registration fee toward 2016 ODA membership dues. CANCELLATION POLICYFor a cancellation refund, written requests must be received by the ODA no later than September 1, 2015. A $20 service fee will apply for each cancellation. Refunds, no matter when submitted, will be processed two to four weeks after the 2015 Annual Session. Refunds will be made via the same method of payment, when possible. The Ohio Dental Association reserves the right to cancel or reschedule Annual Session due to unforeseen circumstances. Registrants will be notified.
