Message from the President

I am pleased to present to you our second annual Maine Medical Partners Performance Summary recalling the activities and outcomes for fiscal

year 2010. As we shared with you in our report last year, the organization continues to grow rapidly, both through growth in services and providers currently existing at MMP and through the addition of new practices to our organization (Neurology, Orthopaedic Trauma & Fracture Care and Otolaryngology). MMP now includes a total of 175 physicians across a significant number of specialties and sites serving communities local to Maine Medical Center and throughout the State of Maine.

Much more impressive than the sheer size and scale of MMP, however, is the growing sense of group and culture that has emerged predominantly in 2010. It can be seen through the many committees and efforts that underscore our work this past year. Whether it is the expanding efforts of our clinical planning committee helping us establish priorities for investments in new physician resources, our robust and comprehensive efforts to develop a planning process for primary care, our expanding commitments to quality and quality reporting or our dedicated efforts to meet new standards associated with electronic health records use as well as provider based entity status, our work has been both substantial and impressive.

The growing number of physicians and physician leaders engaged in these certainly underscore our future successes as we rise to meet new challenges in 2011 and beyond. In short, we are effectively blurring the lines between “management” and “physicians” and creating teams that are the measure of our true success.

With that said, we have much more to do and significant opportunities ahead of us. These include active engagement in the MMC-wide physician unification effort and an ever-expanding focus on doing our part to reduce cost and improve value for our most important constituents, our patients. I am confident that our practice is well prepared to excel at these efforts and challenges.

Please take a moment to review these materials so that you can both celebrate your own practices’ achievements and learn more about those of your colleagues. It is that collective sense of our multi-disciplinary group that is most important in defining our real ability to serve our patients. Thank you very much and well wishes for a happy and healthy 2011.

Stephen J. Kasabian PresidentMaine Medical Partners

Message from the CMO

D uring 2010 Maine Medical Partners has experienced dramatic changes that have placed us at the forefront of medical care for

our community. Sometimes these changes have come so quickly that we wonder whether we can respond effectively - but I am happy to report that with the collective effort of all our providers and staff we met the challenge and, at times, exceeded the goals. As we reflect on the year there are a few highlights that deserve special mention.

Our Quality and Patient Safety Committee has further refined their work during this year to not only foster the best in performance improvement but to also facilitate shared metrics for the coming year. Perhaps even more importantly, we are now actively collaborating on how to divide the tasks and share the lessons learned. The net effect is that our goals are now more coordinated than ever and the effort is more evenly allocated. We have also begun work to integrate our Quality and Patient Safety program with the Quality Council at Maine Medical Center to provide additional cohesion around shared goals and to reduce even further any redundancies in our most important efforts.

This past year also witnessed a tremendous effort from our primary care providers and staff to give guidance to the Primary Care Strategic Planning effort. Over the 8 months of active engagement both the strategic priorities and operational imperatives were outlined and forwarded to the consultant that will combine our work with the strategic planning efforts of Maine Medical Center on this topic. In addition, our work will be utilized along with the input of the MaineHealth members and affiliates as we plan for the needs of the entire region. Our highest compliments go out to all of the participants in this crucial endeavor.

Lastly, the combined effort of our medical leaders and practice leadership deserves special mention. Over a series of meetings they have been working to foster the development of better methods to measure and monitor our efforts to build a world-class multi-specialty group. The intent is to use metrics in our everyday work that will move us from doing a good job to doing a great, and ever improving, job. Rather than looking at what might be called “trailing indicators” we are striving, collectively, to use “leading indicators” of patient care access and quality to define our role in the medical community. To see how this has been embraced and carried forward by the participants is very heartening and bodes well for our future.

We have met both anticipated and unanticipated challenges in the past year but thanks to the combined effort of every individual we have truly moved forward into our future. I welcome this opportunity to acknowledge the talent and tenacity that makes us who we are at Maine Medical Partners!

Practice InformationThe team includes seven Board certified general surgeons who have additional qualifications in Surgical Critical Care and fellowship training in the care of the severely injured. A skilled group of specially trained and highly experienced mid-level providers rounds out the clinical team. Together, we fulfil four separate but interrelated missions:

Trauma Care: We operate Maine’s only verified Level 1 trauma center, providing care for injured patients throughout southern Maine and eastern New Hampshire. The trauma center is a critical component of the public safety net and a regional resource. Our trauma physicians hold positions of national leadership and are active in teaching, research, and system development.

Surgical Critical Care: The division provides comprehensive multidisciplinary evaluation and management of surgical patients with critical illness, serving as a regional resource for the management of the most critically ill from all over Maine and parts of New Hampshire. Our providers are actively involved in teaching and research, in addition to participating in and providing leadership in national and regional organizations.

Burn Care: The division staffs Maine’s only Burn Center, providing both inpatient and outpatient care to burn patients from throughout the state.

Emergency and Elective General Surgery: Our physicians are all highly skilled and experienced general surgeons, providing surgical care for urgent problems from the Emergency Department, the inpatient service, or patients referred from outside hospitals. In addition, the division operates the surgical clinic at Maine Medical Center, providing surgical care and endoscopy to all members of the community, regardless of ability to pay.

Medical Director’s NoteDuring 2010 our founding member, Dr. David Clark, transitioned to an innovative new role combining full-time funded research with ongoing clinical involvement in trauma care. Dr. Clark’s position has been filled by Dr. Kristen Sihler, previously an Assistant Professor of Surgery at the University of Michigan. Along with her specialty expertise, Dr. Sihler brings an interest in rural surgery and systems of regional emergency coverage. This year is also notable for our successful re-verification as Maine’s only Level 1 trauma center; culminating nearly six months of exhaustive preparation by the entire division.

Practice InformationMaine Medical Partners - Geriatric Medicine physicians are Board certified in geriatrics and specialize in the care of the elderly. Our care team is comprised of physicians and nurse practitioners who provide ongoing, primary care in four area nursing homes and seven assisted living facilities. In addition, our geriatricians are also available at New England Rehabilitation Hospital of Portland, Hospice of Southern Maine, and MMC for inpatient and outpatient Geriatric consultation.

Medical Director’s NoteMMP - Geriatric Medicine has continued to grow over the last year. Our goal remains to provide expert geriatric care to the most vulnerable elderly; provide education to fellows, residents, medical students and the community; and perform research that will continue to help guide our ability to provide expert care in the future. Dr. Sarah Hallen received a Geriatric Academic Career Award (GACA) which provides support for education, research, and professional development.

Our geriatricians help support MMC’s educational mission through teaching of geriatric fellows as well as internal medicine, family medicine and gynecology residents. Geriatricians participated in an education block with the first Tufts Medical School class and look forward to ongoing involvement in this arena. They also participate in interdisciplinary educational opportunities to support nursing, therapeutic recreation, and social work students. In addition, mid-level providers participated for the first time in a preceptorship for a UMass - Boston nurse practitioner student’s clinical experience. Research continues to be an area of development with projects under way surrounding hospital falls prevention, early mobilization of intensive care elders, mortality prediction, cancer treatment disparities in the aged, and feeding tube indications.

Over the next year we will continue to provide high quality geriatric care while exploring ways of improving transitions in care through the use of home visits, ongoing involvement in quality improvement and striving to become productive in research to enhance the knowledge of geriatrics. We will also continue to explore opportunities to disseminate geriatric interdisciplinary education throughout Maine.

Practice InformationThe internists of Maine Medical Partners - Hospital Medicine provide care to adults in the inpatient setting. Our hospitalist physicians are faculty and serve as teaching attendings for the internal medicine residency program. Some of our physicians have prior ties to MMC, while many bring in influences, training and expertise from other institutions.

Medical Director’s NoteIn fiscal year 2010 the group continued to be the major source of clinical activity within Adult Inpatient Medicine providing more than half of the 6,000 discharges. The hospitalist program successfully piloted a new Hip Fracture Co-Management Service with Orthopedics improving outcomes related to time to the OR and length of stay. In addition HM began a new program in the Emergency Room called the AIM Lead Physician facilitating flow in the ER. Hospital Medicine continued to have an enhanced role in teaching, providing all overnight attending coverage for the general medicine teaching teams.

We played champion or supporting roles in the adoption and improvement of information technology tools including electronic billing and audits, internal medicine sign-out and discharge summary tab and utilization of SCM’s Rx writer. Hospital medicine has been instrumental in spreading Rx Writer through AIM. In addition Hospitalists have played a key role in improving transitional care of patients from Hospital to Home through participation in Microsystems teams.

As part of our quality measures in the past year, we achieved and maintain excellent performance in medication reconciliation, patient satisfaction, utilization of Rx Writer and oversight and review of Internal Medicine Discharge Summaries. Our focus for FY11 includes CHF core measures, hospital flow including early discharge orders on day of discharge, and reduction in 4 day readmissions.

Practice InformationMaine Medical Partners - Internal Medicine physicians are Board certified internists. Our physicians cover all aspects of adult medical care in both the inpatient and outpatient setting. We offer an integrated approach to patient care, taking each patient’s physical, social, and psychological needs into consideration, while promoting health and preventing disease.

Medical Director’s NoteThis past year has seen continued excellence in MMP - Internal Medicine.

Quality: Our team has excelled in providing high quality care, as demonstrated by our ongoing excellent performance in many quality reporting programs. These include the Maine Medical Center Physician Hospital Organization Clinical Improvement Registry, which reports and rewards on quality measures in the areas of diabetes, cardiovascular disease, asthma, depression and preventive care. We are recognized by NCQA in the areas of diabetes care and heart/stroke care. Our Cape Elizabeth office has recently received recognition as a Bridges to Excellence Physician Office link site. Finally, our Westbrook site was chosen as a participant in the Maine Patient-Centered Medical Home Pilot.

Education: Our physicians continue to be a backbone of the education program for residents in internal medicine at Maine Medical Center. In the inpatient arena, we staff one of the four general medicine teaching services, working closely with residents and medical students caring for hospitalized adult patients on the Adult Inpatient Medicine (AIM) service. In addition, our faculty serve as preceptors and mentors to the internal medicine residents in their continuity clinics, located at the Bramhall campus Outpatient Department as well as at our offices in Cape Elizabeth and Falmouth. Thomas Gearan, MD, who practices at our Falmouth office, was recently named the new Internal Medicine Residency Program Director.

Service: Our physicians are active participants throughout the health care community, contributing to the MMC Center for Performance Improvement, the MaineHealth Ambulatory EMR rollout, MaineHealth clinical integration, the Portland Free Clinic and the development of the MMC/Tufts University Medical School program. Finally, Richard Engel, MD, from our Westbrook office has been serving as the governor for the Maine Chapter of the American College of Physicians, representing our interests throughout the state and at a national level.

Practice InformationMaine Medical Partners - Lakes Region Primary Care offers our community a multi-specialty team that provides medical care for every age. Our practice is comprised of Board certified physicians in both pediatrics and internal medicine. The team is rounded out by a pediatric nurse practitioner and a physician assistant.

Medical Director’s NoteOur office has recently undergone a physical expansion to better serve our patients. The additional space will also allow for future growth, to serve the needs of our community.

Our commitment to medical education remains strong. We serve as a continuity site for the MMC Medicine and Pediatric Residency Program. Students from the University of Vermont spend time with our physicians during ambulatory outpatient pediatric rotations. We look forward to expanding these teaching programs in the upcoming years.

Lakes Region Primary Care was a pilot site for the Maine Health initiative to implement the new ambulatory electronic medical record program, Epic. We have been utilizing the Epic program for 2 years and have enjoyed the benefits this brings to our patients.

We have engaged over the past year in the AAP Chapter Quality Network (CQN) Asthma Pilot Project. The AAP provided the Alabama, Maine, Ohio, and Oregon chapters with tools, resources, and technical support to lead this quality improvement effort. Our participation in this national initiative has assisted us in developing more accurate guidelines for capturing, reporting and utilizing information related to our asthmatic population. We will continue to develop this process and employ the successes with all patients living with chronic illness.

Our providers and staff continue to focus on ways in which we can better serve our patients’ needs. Improvements implemented in 2010 include adding Register Nurses, modifying providers’ schedules to accommodate the acute, same day visits, expanding outpatient office hours to include Saturday appointments and installing a new telephone system.

Practice InformationThe Maine Children’s Cancer Program (MCCP) is comprised of four pediatric hematology/oncology physicians, a nurse practitioner, and a team of specialized nurses, social workers and research associates, who offer specialized treatment and a wealth of resources to young patients and their families.

Complementing the outstanding clinical expertise of our staff, MCCP has an exceptionally strong research program. It is a member of the Children’s Oncology Group (COG)—the premier childhood cancer research organization with more than 220 member institutions worldwide. We also collaborate extensively with the Dana-Farber Cancer Institute and St. Jude Cancer Research Hospital to offer our patients the very latest and best therapies.

Medical Director’s NoteIn 2010, we continued our tradition of collaboration with local, regional and national colleagues to enhance and improve care delivery. Examples include: Dr. Chaleff’s work with the neuro-oncology program at Children’s Hospital Boston and regular attendance at the multi-disciplinary tumor board of this group; Dr. Larsen’s work as chairman of the Maine Medical Center’s Institutional Review Board, Medical Director of Oncology Research for the Maine Medical Center Cancer Institute, and study Chair of COG’s AALL0232 High Risk Acute Lymphoblastic Leukemia therapeutic clinical trial; Dr. Rossi’s work to develop the Department of Health and Human Services Section 340B program for Maine Medical Center’s Hemophilia Program; Dr. Speckhart’s efforts to build a Post Therapy Program that oversees the ongoing comprehensive care of patients who have successfully completed cancer therapy and are now facing chronic toxicities associated with their therapy and Christine Shaffer, FNP work with the inpatient pharmacy, nursing staff, and physicians on a number of critical initiatives designed to improve both safety and the quality of the care we deliver to our patients.

Our program holds a biweekly Tumor Board Conference in association with the Pediatric Oncology Program at the University of Vermont via teleconference. We have recently added the program at Eastern Maine Medical Center and we actively engaged in discussions with Boston Floating Hospital to add them on as well.

Finally, we were awarded a $100,000 competitive grant from the Hyundai Hope on Wheels Program to develop a telemedicine component for our program. In the upcoming year, we will seek to establish teleconferencing capabilities at key clinical sites in the state and to develop portable teleconferencing systems to be used directly by children and families in their homes.

Practice InformationMaine Neurology joined Maine Medical Partners in March 2010 following 23 years of private practice. The decision to forge this alliance and join this multi-specialty medical group was in keeping with our shared values and philosophy of medical care. MMP Neurology is the largest neurological practice in Maine, offering a comprehensive range of services for adults and children.

Our practice offers the clinical expertise of 12 board certified Neurologists, most with additional subspecialty certification, and three nurse practitioners. Specialty care is available for Epilepsy, Memory Disorders, Movement Disorders, Multiple Sclerosis, Neuromuscular Disease, Pediatrics, Peripheral Neuropathy/Disc Disease/Nerve Injury, Sleep, Stroke, Vertigo and Dizziness.

We have an in house MRI unit to ensure timely access and state of the art imaging for our patients. Our registered MRI technologists skillfully perform vascular, neurological and spinal imaging. Our MRI department provides services to more than 250 patients each month and has recently achieved three-year accreditation by the American College of Radiology (ACR).

Earlier this year the END/Vascular Lab earned Accreditation by the Intersocietal Commission for the Accreditation of Vascular Laboratories (ICAVL). The lab attained this recognition for its commitment to providing a high level of patient care and quality testing for the diagnosis of vascular disease. The Electrodiagnostic Lab is one of two accredited EEG labs in the state.

Medical Director’s NoteWe anticipate that we will be expanding our neurology team by five physicians over the next five years. We are currently recruiting for Pediatric Neurology, Movement Disorder, Stroke, Critical Care and Neuromuscular Disease.

Practice InformationMaine Medical Partners - Neurosurgery & Spine offers the clinical expertise of fellowship-trained and Board certified neurosurgeons. Our specialists encompass all aspects of brain and spine treatment, and each physician is on the leading edge of his or her respective specialty.

Medical Director’s NoteRobert Ecker, MD, an endovascular neurosurgeon, joined MMP - Neurosurgery & Spine in August of 2010. His addition to the group brings comprehensive endovascular diagnosis and treatment to Maine Medical Center and benefit patients throughout northern New England.

Quality: Working with the Neurosciences Institute, we have helped create a balanced scorecard for gathering and reporting data on surgical inpatient activity at MMC. The group is currently in the process of implementing a Web-based patient health questionnaire for the outpatient environment that will provide valuable data with respect to the etiology and efficacy of surgical and non-surgical treatments for spine patients.

Research: We are committed to supporting research and development in the neurosciences through our involvement in a number of grant-funded research studies and databases. We are the only group in the country to participate in clinical trials for a new medication designed by a leading drug manufacturer to aggressively treat malignant brain tumors. The practice has also established a number of databases to collect retrospective data on brain tumor, aneurysm, epidural steroid injection, burst fracture, and odontoid fracture patients. The valuable data and information generated from these efforts will be used to improve the group’s ability to meet the treatment needs of patients.

The group’s priorities for fiscal year 2010 supported the continued ability to meet or exceed the goals defined by the MMC Neurosciences strategic plan. These priorities include improving on evidence based clinical treatment algorithms and surgical patient selection criteria analysis, and sharing best practices among providers. The formalization of the group’s shared decision making processes, in collaboration with the CORE team, and the design and implementation of a SDM pilot study was also a priority for FY10.

Practice InformationThis division of Maine Medical Partners provides orthopaedic trauma and fracture care to both adults and children. The team includes two Board certified orthopaedic surgeons specializing in management of complex fractures and fracture related problems. Also included in the team are two highly trained and experienced mid-level providers. The surgeons provide guidance and care to physicians and patients throughout the state of Maine and New Hampshire. We provide emergency room coverage and physician requested consultations 24 hours per day, 365 days per year.

This newly created division is part of Maine’s only Level 1 trauma center.

The office provides both operative and non surgical care for fractures. Our long time cast technician also fabricates custom made orthotics that are fitted and available for patients to take home the same day of their appointment.

Medical Director’s NoteThe goal of this division is to continue to develop and expand its leadership role as a regional resource for orthopaedic injuries. We are working to develop high efficiency operating rooms. We continue to be active in the AO Foundation as faculty and chairmen of numerous AO courses. We have also coauthored a chapter in a book published by the American Academy of Orthopaedic Surgeons (AAOS) titled Orthopaedic Knowledge Update and revised the AO Foundation’s online surgical reference section on Fixation of Tibial Shaft Fractures.

In support of the programmatic goals, we are actively recruiting a third Board certified orthopaedic surgeon and two additional experienced mid-level providers.

Practice InformationOur team is comprised of three practicing surgeons, one emeritus surgeon, two nurse practitioners, and three full-time office staff working to provide outstanding surgical care to the children of Maine, southern Vermont and southern New Hampshire. Services include minimally invasive surgery of the chest and abdomen, bariatric surgery consultation, neonatal surgery, gastric and intestinal surgery.

Medical Director’s NoteEducation, Research and Publication: Pediatric Surgery participates in a national children’s surgical database, Pediatric NSQIP (National Surgery Quality Improvement Project). Albert Dibbins, MD, is the Physician Champion for this effort. Dr. Baird Mallory and Jeannie Ross, PNP started a high school student led trauma prevention program. Drs. Dibbins and Mallory retrieved data to establish a 30 year Pediatric Surgery data base for future outcome studies. Dr. Mallory submitted one manuscript for publication and Janice Dudley, PNP and Dr. Ian Neilson did an institutional presentation this year. Pediatric Surgery hosted a state wide nursing educational day and a state wide physician educational day on Pediatric Surgery at last fall’s AAP meeting. Participants in leading sessions included Janice Dudley, PNP, Jeffrey Halter, MD, Baird Mallory, MD, Ian Neilson, MD and Jeannie Ross, PNP. Pediatric Surgery participated in the launch of an institute wide Vascular Access Team with Dr. Mallory’s involvement in planning and implementation.

Quality: Several quality initiatives were completed this year in the areas of Emergency Preparedness Education and Patient Satisfaction with Care. Targeted efforts included: developing and implementing a system for post-op follow up phone calls, OR and NICU briefings to improve communication, the development and implementation of patient pre and post operative education packets and IBD Outcomes Analysis. To enhance their quality improvement knowledge base and skill set, the team participated in a six months intensive Clinical Microsystems quality improvement training program.

Division expansion: Pediatric Surgery initiated recruitment for a fourth surgeon and a part time medical assistant to join their team. In addition, we are proud to announce that Dr. Mallory became a member of the NACHRI, Chiefs of Surgery and Directors of Children’s Hospitals Association.

Practice InformationOur physicians provide state-of-the-art treatment for neonates to older adults. We treat a wide variety of urological conditions including urological cancers, stones, urinary incontinence, male infertility and impotence, urinary infections, and congenital anomalies in infants and children, among others.

Medical Director’s NoteWe are the regional leaders in the surgical management of prostate cancer, with the most experience in da Vinci® robotic-assisted laparoscopic prostatectomies. Our surgeons recently performed our 1000th robotic procedure. This comes on the heels of being selected as one of five model prostate cancer programs in the nation by the Association of Community Cancer Centers. Our surgeons also perform a variety of complex genito-urinary laparoscopic and open procedures. Early in 2010, we went through our first residency match process and received 150 applicants for the single PGY-1 position. In July, we welcomed our first PGY-2 resident as part of the ACGME-approved residency in urology.

Program expansions have included the employment of our seventh partner, Dr. Peter Steinberg, who joined us after completing a laparoscopic and endourologic fellowship at Montefiore Medical Center. We have also hired a part-time office-based urologist in late 2010, and eagerly await the arrival of two additional colleagues in mid-2011, who will bring further expertise in infertility and robotic surgery.

Research: The Maine Cancer Foundation-funded grant entitled “Predicting Quality of Life Outcomes after Prostate Cancer Treatment” has Moritz Hansen, MD as co-investigator with Amy Haskin, MD of the MMC Center for Outcomes Research and Evaluation (CORE). Translational research efforts are being supported by linking the MMC prostate cancer clinical database to available de-identified tissue bank specimens. This will allow important collaborations in urologic oncology between basic scientists at the Maine Medical Center Research Institute and the clinicians at MMP - Urology. Research interests remain active in techniques of risk communication, shared decision-making, cost effectiveness, and learning rate analysis. MMC was also recently awarded a NCCCP grant by the National Cancer Institute with funding for a state-wide bladder cancer initiative led by Dr. Craig Hawkins.

Program Growth: The Maternal-Fetal Medicine division performed over 42,000 diagnostic studies, 5,000 consults, assumed the total care of high-risk obstetrical patients, and handled over 400 transports from hospitals throughout Maine and New Hampshire. With the growing demands of the practice, Rebecca Hunt, MD was hired to help support the obstetric service, and to develop a family planning resource for the greater community. We established a telemedicine obstetrics center at St. Mary’s Regional Medical Center to provide patient ultrasounds and consultation services to their physicians. We also expanded the number of patients seen for ultrasound and consultations in the Waterville community. Locally, regionally and nationally, MMP - Women’s Health continues the tradition of leadership in health care for women.

Service ExcellenceChampions of ChangeThis year, a quality improvement training program was designed, implemented and completed in the second week of January 2011. This program is called Champions of Change.

The Champions of Change program was designed to teach 32 participants across 16 sites the skills they need to conduct quality improvement projects. The program participants were Patient Service Representatives, Clinical Managers, Medical Assistants, Nurses and Practice Managers.

These participants became the quality improvement champions within their sites and now guide others in how to apply the tools of quality improvement to any process.

The MMP Quality Program framework for FY11 was strategically aligned with the Champions of Change program. With this new framework, we have projects with a clear patient centered focus, greater collaboration within and between sites, greater knowledge of quality improvement methodologies and a measurable increase in efficiencies.

Care TransitionsIn early 2010, we developed and implemented a Care Transitions Program with the goal of promoting a safe and effective transition for patients from the inpatient setting to home. Specific areas of focus include:

• Reduce rehospitalization and avoidable Emergency Department visits• Reduce medication discrepancies• Increase follow-up with the PCP post discharge• Increase patient awareness of complications and when to seek treatment

The program was launched in May 2010 for MMP patients discharged from the Medicine Service to home. In September, this was expanded to included patients discharged from the Surgical Service to home.
