Successfulorgantransplantationonlybecamepossiblewiththeintroductionin1984ofmedicationsableto"tame"ourimmunesystem.Themomentyournewliver,whichcomeswithdifferentDNAandproteins,isplacedintoyourbody,yourbodywageswaronyourimmunesystem.Weprescribemedicationstopre‐ventthecellsofourimmunesystemfromcausingdamage.

Theimmunesystemisahighlyorganizedteamofcellsprotectingourbodiesagainstbacteria,viruses,fungusandfor‐eignproteins.Youhavemanyteamshelp‐ingyouduringyourjourneyoflivingalongandhealthylife:yourhometeam(youandyourcaregivers),themedical,psychosocialandsurgicalteamandtheimmunesystem.Asyouknow,communi‐cationamongteammembersisessentialforagoodoutcome.

Medicationstopreventrejection,alsocalledimmunosuppressants,alterandsilencesomeofthecommunicationsig‐nalsbetweenyourimmunesystem’scells.Theimmunesystem’sdefenseteamhastwobroadcategories:agroundarmyofsoldiersstationedattheborderofyourskinandmucousmembranes(calledtheinnateimmunesystem)andmorespecial‐izedsoldiers,thenavysealsandmarinesthatcanbecalledintoassist(theadap‐tiveimmunesystem).Thegroundarmy(innateimmunesystem)hasbloodcellsthatarecapableofeating,punchingholesand,thereby,killingbacteria,virusesandsomecancercells.Macrophages(sentinelcells),neutrophils(footsoldiers)andnaturalkillercellsaresomeofthemaincellsoftheinnatesystem.Thesecellsarealwaysreadytodefendus.

Theadaptiveimmunesystem(thespe‐cializedsoldiers)containswhitebloodcellscalledlymphocytes.BLymphocytesarecellssecretingantibodiesthatcoatbacteria,virusesororganicmaterialtag‐gingthemfordestruction.Tlymphocytes(Thelpercells,CD4andTkillercellsCD8)assistinkillingvirusesinsidecellsandrecognizeforeignproteinpresentedtothem(proteinfragmentsformyourdonorliver).Theadaptivesystemtakestimeto

mature,hasmemoryandisthesystemmanipulatedbyimmunosuppressivemedicationslikePrograf,Neoral,MyforticandPrednisone.

PrografandNeoralinterferewiththeproductionofasignalingproteincalledInterleukin2.Interleukin2islikeacellphone.ItisusedbyTlymphocytestoalert,activateandtraintheadaptiveim‐munesystem.Ifthereisnocommunica‐tion,thereisnoproductionandexpan‐sionofthesespeciallymphocytes.

PrografandNeoralneedtobetakenevery12hrstoeffectivelyinterferewithcellphoneuse.IfyouonlytakethesemedicationinthemorningandroutinelyforgettheeveningdoseyougiveyourbodyachancetoproducesignalstowakeupthespecialTlymphocytes.Itisalsoimportantthatthedoseofmedicationissufficienttointerferewithcellsignals.

Prograflevelsinthefirstmonthsaftertransplantare7‐10,Neorallevels100‐200.However,yourmedicalteamwilladjustthedosetoyourbodyandyourunderlyingillness.Myforticpreventsthebuildingandassemblyoflymphocytes.Myforticdoesnotdependonadruglevel.Prednisoneinterfereswithboththeinnateimmunesystem(reducingthefunctionofneutrophilsandmacrophages)andadap‐tiveimmunesystem(reducinginflamma‐torysignals).Prednisonebroadlyaffectsalllevelsofimmuneresponsiveness.

Rejection,ortheprocessofattackingyournewliver,hastwotimecourses:acuteandchronicrejection.Acutecellularre‐jectionoccursinthefirstseveralweekstomonthsaftertransplantandisseeninabout25%ofallpatients.Acutecellularrejectiondoesnotgenerallyleadtolossoftheorgan.Acuterejectionisusuallyas‐ymptomaticandonlydetectedbyvigilantbloodtesting,dailyinthehospital,weeklyandthenmonthlyafterdischarge.

ElevationintheASTandALTliveren‐zymefunctiontestsindicateslivercelldeath.Ifthereispersistentelevationintheseliverenzymes,youwillbemoni‐toredwithaliverbiopsy.Theresultsofthebiopsywilldeterminedegreeofrejec‐

tion(inflammation)andismeasuredbyaBanffscore.ThehighertheBanffnumbertheworsetheinflammationorinjury.Thetreatmentofacuterejectionisusuallyashortcourseofintravenousprednisoneandincreaseinyourbaselineimmunosup‐pression.

Chronicrejectionusually,butnotalways,happensafterthefirstyearandisseeninabout5%ofpatients.Itisusuallyaresultof“forgetting"ornottakingyourmedica‐tionasprescribed.Theliverenzymetests,ASTandALTareelevatedinchronicrejec‐tionbutlesssothaninacuterejectionbuttotalbilirubinandalkalinephosphataselevelswillbemuchhigher.Onliverbiopsy,weseelossofbileducts,called"thevanish‐ingbileductsyndrome."Chronicrejection,likeacuterejection,isoftenasymptomatic.Thereisoftentoomuchdamagetoyourliverwhenyouareabletoseethesymp‐toms.Forinstance,whenyoureyesturnyellowmanybileductshavebeende‐stroyedandcannotregenerate.

Generally,acuterejectioncanbewelltreatedbyaddingandadjustingim‐munusuppressionanddoesnotcauseper‐manentdamagetotheliver.Unfortunately,chronicrejectionisdifficulttotreatandoftenleadstocirrhosisanddeathinmanypatients.Itisimportanttoknowthatchronicrejectiondoesnotjusthappen.Chronicrejectiondoesnothappen"outofnowhere."Ifyouhaveyourbloodtestsregularly,communicatewithyourmedicalandpsychosocialteam,anddon'trepeat‐edlyforgetorrunoutofyourmedication,thisfearedcomplicationdoesnothavetohappen.Ifyouhavecirrhosisasaresultofchronicrejectionduetonottakingyourmedicationornon‐compliance,asecondlivertransplantislikelynotanoptionatourcenter.

SedebetomarPrografyNeoralcada12horasparainterferirenformaefectivaenelusodelteléfonocelular.Sisolotomaestosmedicamentosporlamañanayseolvidaenformarutinariadeladosisdelatarde,ledaalcuerpolaoportunidaddeproducirseñalesquedespiertenloslinfocitosTespe‐ciales.Tambiénesimportantequeladosisdemedicamentoseasuficienteparainter‐ferirenlasseñalesdelascélulas.

LosnivelesdePrografdurantelosprimerosmesesdespuésdeltrasplantesonde7a10,losnivelesdeNeoral,de100a200.Sinem‐bargo,suequipomédicoajustaráladosisasucuerpoysuenfermedadsubyacente.ElMyforticevitalaconstrucciónyelensambledelinfocitos.ElMyforticnodependedeunniveldefármaco.Laprednisonainterfieretantoenelsistemainmunitarioinnato(reduciendolafuncióndelosneutrófilosymacrófagos)comoenelsistemainmunitarioadaptativo(reduciendoseñalesinflamato‐rias).Laprednisonaafectaampliamentetodoslosnivelesderespuestainmunitario.

Elrechazooprocesodeatacarelnuevohígadotienedosetapas:rechazoagudoycrónico.Elrechazocelularagudosepro‐duceenlasprimerassemanasamesesdespuésdeltrasplanteyseveenalrededordel25%delospacientes.Elrechazocelularagudo,porlogeneral,nollevaalapérdidadelórgano.Elrechazoagudoesnormal‐menteasintomáticoysolosedetectamedi‐anteanálisisdesangredemonitoreo,diaria‐menteenelhospital,semanalmenteyluegomensualmentedespuésdelalta.

UnaelevaciónenlosanálisisdefuncióndelasenzimashepáticasASTyALTindicalamuertedelascélulashepáticas.Sihayunaelevaciónconstantedeestasenzimashepáticas,selerealizaráunabiopsiadehígadoparamonitorearlo.Losresultadosdelabiopsiadeterminaránelgradoderechazo(inflamación),elcualsemidemedianteelpuntajedeBanff.Cuantomayoresel

númeroBanff,peoreslainflamaciónole‐sión.Porlogeneral,eltratamientodelre‐chazoagudoesunaadministracióncortadeprednisonaporvíaintravenosayunau‐mentoenlainmunosupresiónbasal.

Porlogeneral,aunquenosiempre,elre‐chazocrónicoseproducedespuésdelprimerañoyseveenalrededordel5%delospa‐cientes.Normalmente,eselresultadode“olvidar”onotomarsusmedicamentoscomoseleindicó.LosanálisisdeenzimashepáticasASTyALTsonelevadosenelre‐chazocrónicoperomenoresencomparaciónconelrechazoagudo,aunquelosnivelestotalesdebilirrubinaydefosfatasaalcalinasonmuchomayores.Enunabiopsiadehígado,vemosunapérdidadeconductosbiliares,loquesedenomina“síndromededesaparicióndeconductosbiliares”.Elre‐chazocrónico,comoelrechazoagudo,esamenudoasintomático.Amenudohaymuchodañoenelhígadoparacuandosepuedenverlossíntomas.Porejemplo,cuandoseponenamarilloslosojos,muchosconductosbiliaressehandestruidoynopuedenregenerarse.

Generalmente,elrechazoagudopuedetratarsecorrectamenteagregandoyajustandolainmunosupresión,ynocausadañospermanentesalhígado.Desafor‐tunadamente,elrechazocrónicoesdifícildetratar,yamenudoprovocacirrosisylamuerteenmuchospacientes.Esimportantesaberqueelrechazocróniconosucedeporquesí.Elrechazocróniconosucede“delanada”.Siserealizaanálisisdesangredemaneraregular,secomunicaconsuequipomédicoypsicosocial,ynoseolvidarepetida‐mentedetomarsusmedicamentosniselesacaban,estatemidacomplicaciónnodebesuceder.Siustedtienecirrosiscomocon‐secuenciadeunrechazocrónicopornotomarlosmedicamentosopornocumplirconlasindicaciones,probablementenotengalaopciónderealizarseunsegundotrasplantedehígadoennuestrocentro.

WhenIfirstattendedtheEducationalWorkshopsIstartedoutasablankpage.MyinnocencebroughtmetherebelievingthatIwouldbeoneofthefirsttogetaliver.ManyattendeeshadchronicHepatitisC,exces‐sivealcoholuse,fattyliverdiseaseandotherdiseasesthatIlookedatasself‐abuse.Ihadledahealthyandcleanlife‐stylesothereforeandwithoutquestionIshouldbeatthetopofthelist.Surprise!!Notso!!

Ilearnedthatnoonevoluntarilychoosestodestroytheirliverwhileengagingintheselifestyles.Dr.Silviataughtmethatthereshouldbenojudgment,nopunish‐ment,nofingerpointing.Wewereallsickanddyingso,therefore,wewouldallbelistedunderthesameModelforEnd‐stageLiverDisease(MELD).

ThevariousandsometimesdifficultmedicalproceduresusedtoevaluatemystatustestedmypatienceanddrovemetoeducatemyselfonmydiseaseandwhyIwasputthroughthistorment.Iwasnowopeninganewbookontheworldof

medicine.Mycuriositywasbeingsatis‐fied,myknowledgewasgrowingandIwasnowdevelopingarespectforthosewhocuredthediseased,comfortedthesickandwhoperformlife‐savingopera‐tions.Thatadmirationgrowsbytheday.IoftenfeeldwarfedwhenIaminthepres‐enceofthestaffatTheCenterForLiverDiseaseandTransplantation.Howfortu‐

nateIhaveexperiencedthisfeelingofawe.

Onequieteveningthatphonecallcameinformingmethattherewasaliveravail‐able.MywifeandIwererelaxed,incon‐trolandabitanxioustohavetheopera‐tion,recoverquicklyandreturntoanor‐mallife.Theoperationandquickrecov‐erycameoffwithoutmuchfanfare.Wewerebothsurprisedandfeltveryblessed.ThelovingeffortsofmycaregiverbroughtmetoaplacewhereIthoughtIwouldpickuponmylifeasbefore.HowfortunatethatIwasonceagaininvolvedwithlovingfamily,friendsandenjoyingmyfivegrandchildren.InsideIstruggledwithfeelingsthatIsomehowhadchangedbutcouldnotreallyidentifythisunknownME.Iwasexperiencingmoresensitivity,aneedtoshowmyoverwhelmingapprecia‐tionandstrugglingwiththerealitythatsomeonediedinorderformetolive.HowdoIthankmydonorfamilyandhonormydonor?Iwrotemy“ThankYou”lettertothedonorfamilywithinonemonth.

Inthefirstyear,IwasaskedtocometotheworkshopsforthenewpatientstotellmystoryandIdidforseveralyears.JohnRicewasafellowrecipientwhojoinedmeandwegottoinspireeachotheronhowtoreachouttothosewhowerestrugglingthroughthepre‐transplantprocessandneededtotalktousaboutourexperi‐ences.Theseveryrewardingtimesand

MedicationlistItisveryimportanttohaveanaccuratemedicationlistsoyouknowthecorrectwaytotakeyourmedica‐tions.Medicationchangeshappenfrequentlyafteralivertransplantanditisimportanttostayuptodateonallchangestomakesureyoudonothaveoutdateddirections.Tohelpyoustaycompliantwithmedica‐tions,amedicationlist(MyMedSchedule®)willbegiventoyoubeforeyouaredischargedfromthehos‐pital.YoushouldALWAYSbringyourmedicationlistwithyoutoalltransplantclinicappointments,soyoucantakenotesonanychangestoyourmedications.Thismedicationlistcanbeavailableonlineat(www.mymedschedule.com)andthroughtheMyMed‐Schedule®mobileappifyousetupanaccountwiththeMyMedSchedule®program.Thisallowsyoutohaveaccesstoyourlistwheneverandwhereveryouaretohelpyouwithcompliance.

Idon'tknowtherightwordstosumupmystory.Icanonlyreflectonthedivineinterventionthatsavedmylifeandpermittedmetogrowandreflectonwhathasbeensomeofmybestyears.Ilookforwardtocelebrating50yearsofmarriagetomywifeandcaregiverMary.IcannowmarvelatthegrowthanddevelopmentofmyfivegrandchildrenwhoweretoddlerswhenIfirstgotsickandarenowincollegeandhighschool.Thedesiretowatchthemgrowwasamajorfactorinmyrecoveryandinkeepingmyselfhealthy.Ofcourse,TheCenterforLiverDiseaseandTransplantationwherethosetalentedhands,brilliantmindsandfiercelydedi‐catedhumanshaveproventhat"AmazingThingsAreHappeningHere."
