The EH&S Training (JHQ) Database

To access the EH&S Training Database:

- Go to: https://ehswprod.lbl.gov/EHSTraining/Jhq/EHSLogin.asp - Enter LDAP username and password - Navigate by using the JHQ Menu (on the left hand side)

To complete or update a Job Hazard Questionnaire (JHQ): - Select “Take JHQ” (Supervisors can “login as a Proxy User” for their employee) - Select the proper JHQ (Labwide, LSD or PGF)

- Complete the JHQ - Verify and Submit JHQ by clicking the button at the end of the questionnaire.

To View a Training Profile: - Select “REPORT”, “VIEW TRAINING PROFILE”. • By Division – view a list of all employee training profiles for a Division• By Supervisor - view a list of all employee training profiles for a given supervisor • By Individual – View an individual’s training profile
